Management of the second and third metatarsal in moderate and severe hallux valgus.

Hallux valgus is a common disorder of the forefoot that results from medial deviation of the first metatarsal and lateral deviation and/or rotation of the great. The management of hallux valgus is usually focused on the malalignment of the first ray, however, some patients report that pain at the plantar aspect of second and/or third metatarsal head(s) is more severe than the first metatarsophalangeal joint. In order to alleviate the metatarsalgia, we developed an oblique sliding osteotomy to manage the second metatarsal. This article reports the results of 17 feet that were treated with obliq

Length of the first metatarsal and hallux in hallux valgus in the initial stage.

The aim of this study was to confirm whether the length of the first metatarsal and the length of the hallux are greater than normal in the initial phase of the hallux valgus deformity. In a sample of 152 radiographs (98 of normal feet and 54 of incipient hallux valgus feet), the length of the first metatarsal and the hallux was measured according to methods previously described. Comparisons were made between normal and hallux valgus feet, and between male and female feet. The results show significant differences between the two groups in the first metatarsal (P<0.0001) and hallux (P<0.001). I

Medial deviation of the first metatarsal in incipient hallux valgus deformity.

BACKGROUND: The aim of this study was to determine whether excessive medial deviation of the first metatarsal (excessive intermetatarsal angle) is present in the initial phase of hallux valgus. METHODS: The intermetatarsal angle between the first and second metatarsals (1-2 IMA) was radiographically studied in 49 normal feet and in 49 feet with mild hallux valgus deformity. RESULTS: The results demonstrated a statistically significant difference in the mean intermetatarsal angle between the two groups (8.76 degrees in normal feet; 9.98 degrees in affected feet). However, we believe that is n

Distal metatarsal osteotomy for hallux valgus in the middle-aged patient.

Thirty-eight distal metatarsal osteotomies for hallux valgus in middle-aged patients between the ages of 50 and 67 years were performed over a period of five years. None of the patients could be rated with an excellent result. Good results were noted in only 52.6%. Poor results were attributable to imperfect surgical technique, severity of preoperative deformity, osteoporosis, and preexisting osteoarthritic changes at the first metatarsophalangeal joint.

Proximal spherical metatarsal osteotomy for the foot with severe hallux valgus.

BACKGROUND: A proximal spherical metatarsal osteotomy was devised to correct not only varus deviation of the first metatarsal, but also dorsiflexion. We expected to increase the medial longitudinal arch by adding plantar flexion at the osteotomy site. To investigate the limitations of this procedure for feet with severe hallux valgus, a followup study was performed on patients with preoperative hallux valgus angles greater than or equal to 40 degrees. MATERIALS AND METHODS: Forty-eight feet in 37 patients (10 male, 27 female) (60 years; range, 20 to 84 years) were investigated. Mean followup

Second Metatarsal Fracture During Lapidus Hallux Valgus Correction: A Case Report.

CASE: A 67-year-old woman with rheumatoid arthritis underwent Lapidus hallux valgus correction. Initial realignment was unsatisfactory, so the K-wire through the first and second metatarsals was removed. As the clamp was repositioned, the second metatarsal fractured through the K-wire hole. The Lapidus procedure was completed, and then, the fracture was fixed with a percutaneous K-wire. The tarsometatarsal fusion and second metatarsal fracture healed. CONCLUSION: Because second metatarsal structural integrity is imperative for use of the clamp, bone quality and K-wire holes from re-reduction

Distal Oblique Metatarsal Osteotomy for Hallux Valgus Deformity: A Clinical Analysis.

We compared the outcomes of the distal oblique metatarsal (DOM) osteotomy, which is parallel to the articulation surface of the proximal phalanx, with those of the chevron osteotomy and evaluated whether displacement and shortening of the first metatarsal have any effect on the incidence of metatarsalgia and patient satisfaction. Patients treated with the DOM osteotomy (n = 30) or distal chevron osteotomy (n = 31) were evaluated retrospectively. The chevron and DOM osteotomies both provided significant improvement in the first intermetatarsal angle (p < .001), hallux valgus angle (p < .001), d

Bipartite hallucal sesamoid bones: relationship with hallux valgus and metatarsal index.

OBJECTIVE: The objective was to relate the incidence of the partition of the hallucal sesamoid bones to the size of the first metatarsal and the hallux valgus deformity. MATERIALS AND METHODS: In a sample of 474 radiographs, the frequency of appearance of bipartite sesamoids was studied. The length and relative protrusion of the first metatarsal, and the hallux abductus angle, were measured and compared between the feet with and without sesamoid partition. RESULTS: The results showed that 14.6% of the feet studied had at least one partite sesamoid, that the sesamoid most frequently divided w

Distal soft tissue procedure and proximal metatarsal osteotomy in hallux valgus.

The results of a distal soft tissue procedure and a proximal metatarsal osteotomy in patients with symptomatic hallux valgus deformity were reviewed. The series consisted of 33 patients (47 feet; mean age of patients, 44 years). The average followup period was 48 months. At followup, 41 feet (29 patients, 85%) were free from pain at the first metatarsophalangeal joint. In six feet (four patients), the pain was improved but persisted. The mean hallux valgus angle was 38 degrees before surgery and 13.8 degrees after surgery. The mean intermetatarsal angle was 17.7 degrees before surgery and 7 de

Hallux valgus correction with proximal metatarsal osteotomy: two-year follow-up.

We evaluated the results of 33 feet in 23 patients who underwent a basilar crescentic osteotomy with a modified McBride procedure with a minimum 24-month follow-up. The average hallux valgus improved from 37.5 degrees to 13.8 degrees and the intermetatarsal 1-2 angle from 14.9 degrees to 4.7 degrees. The angle of declination of the first metatarsal was found to have dorsiflexed an average of 6.2 degrees. Unfortunately, osteotomies secured with staples dorsiflexed to a greater degree. Bilateral foot surgery produced results similar to those with unilateral procedures. Four of our patients devel

Silastic implant arthroplasty with proximal metatarsal osteotomy for painful hallux valgus.

First metatarsophalangeal joint arthrosis may be associated with hallux valgus deformity. In an attempt to address the pain and deformities of this disorder, silastic implant arthroplasty combined with soft tissue balancing and proximal first metatarsal osteotomy has been performed on 17 feet in 11 patients. A retrospective review of the results at an average followup of 29.5 months is presented. Subjective pain relief has been satisfactory, and loss of correction of deformity has been minimal. Infection remains as a significant postoperative complication. This technique may be a treatment opt

Hallux valgus with second metatarsalgia: Is second metatarsal Weil osteotomy necessary?

BACKGROUND: The role of concomitant Weil osteotomy to address second toe metatarsalgia during hallux valgus correction is unclear. We aimed to critically analyse outcomes of an additional Weil osteotomy versus isolated Scarf osteotomy.in the management of hallux valgus and second metatarsalgia. METHODS: Patients with second toe metatarsalgia who underwent first metatarsal Scarf osteotomy for hallux valgus were enrolled retrospectively. Demographics, radiographic measurements and functional outcomes were assessed at baseline, 6-months and 2-years postoperatively. Between-group significance was

Percutaneous double metatarsal osteotomy for correction of severe hallux valgus deformity.

Double osteotomy of the first metatarsal is an option in treatment of severe hallux valgus deformity. Good short-term results have been reported with percutaneous surgery in hallux valgus with moderate deformity. We report short-term results with percutaneous double osteotomy of the first metatarsal in severe deformities. This is a prospective study of 6 patients with severe hallux valgus deformity who were treated with percutaneous double osteotomy of the first metatarsal (proximal closing wedge and distal chevron osteotomy) in 2008. They were assessed preoperatively and one year and two year

Quantitative morphological evaluation of the first metatarsal head in juvenile hallux valgus.

BACKGROUND: Hallux valgus, a common deformity of the forefoot, is characterized by lateral deviation of the hallux at the metatarsophalangeal joint, often requiring surgery. Juvenile hallux valgus, defined by its development during childhood or adolescence, has a high postoperative recurrence rate. However, its etiology remains unclear. Several studies have found that the radius of curvature of the first metatarsal head is smaller in individuals with adult hallux valgus than in healthy controls, possibly contributing to progression of deformity or recurrence. However, to our knowledge, no deta
