In the present article the interaction of 1- isopropylamino-3-[4-(2-methoxyethyl)-phenoxy]-2-propanol (metoprolol, Beloc) with human erythrocytes is reported. Metoprolol is well known and acts mainly as a selective beta-1-antagonist. While erythrocyte aggregation, suspension osmolality and pH-values were unchanged, red cell deformability was markedly improved. Investigation into the furosemide-sensitive fluxes resulted in an inhibition of sodium efflux and in a constant water content of human erythrocytes. These in vitro results suggest that metoprolol induced improvement of red cell deformabi

The antiarrhythmic effect of metoprolol after chronic treatment has been evaluated by 24 hour Holter monitoring in 16 patients with several premature ventricular beats and in 14 patients with chronic atrial fibrillation and compared with the effect of a placebo. Metoprolol induced a mean decrease of heart rate of 14.5% in the 16 patients and reduced the mean value/min of the ventricular premature beats of the 51.2%. In addition the use of metoprolol abolished the ventricular premature beats in 9 cases, while in two cases there was an increase of their frequency. In the 14 patients with chronic

BACKGROUND: Metoprolol is recommended for therapeutic use in multiple cardiovascular conditions, thyroid crisis, and circumscribed choroidal hemangioma. A detailed systematic review on the metoprolol literature would be beneficial to assess all pharmacokinetic parameters in humans and their respective effects on patients with hepatic, renal, and cardiovascular diseases. This review combines all the pharmacokinetic data on metoprolol from various accessible studies, which may assist in clinical decision making.
METHODOLOGY: The Google Scholar and PubMed databases were searched to screen articl

The cardiac electrophysiological effects of metoprolol were studied in ten patients (pts) aged 22-51 years undergoing intracardiac stimulation studies for paroxysmal palpitations. The following measurements were made: (1) basic sinus cycle length (SCL); (2) sinus node recovery time (SRT) following overdrive pacing; (3) atrio-His (A-H) and His-ventricular (H-V) conduction intervals during regular atrial pacing; (4) effective refractory periods of the atria (AERP), A-V node (AVERP) and ventricular myocardium (VERP); and (5) A-V nodal functional refractory period (AVFRP). All measurements were re

Induction of microsomal enzymes with barbiturates in rats has little effect on the metabolism of metoprolol, compared with propranolol and alprenolol, which undergo extensive hepatic extraction in animals and man. Our study was designed to examine whether the metabolism of metoprolol is inducable by barbiturate in man. In 8 healthy subjects the area under the plasma concentration/time curve after 0.1 gm metoprolol was reduced by a mean of 32% after treatment with 0.1 gm pentobarbital at bedtime for 10 days. There was considerable interindividual variability in the reduction after pentobarbital

A crossover comparison of metoprolol and hydrochlorothiazide has been performed in 20 patients with mild hypertension. Both drugs caused almost identical statistically significant reduction in blood pressure of about 20 mm Hg systolic and 15 mm Hg diastolic. The side effects during active therapy were few and mild, but 5 patients experienced subjective symptoms during the first few days following abrupt withdrawal of metoprolol, namely general malaise, palpitations, headache, sweating and tremor. The symptoms were more pronounced in the standing position and disappeared at once on resumption o

1. Metoprolol was metabolized in rat liver microsomes in vitro by O-demethylation with subsequent oxidation and by aliphatic hydroxylation of the methoxy-ethyl substituent and by oxidative deamination of the propanolisopropylamine side-chain. The same routes of metabolism in the rat in vivo were revealed from urinary metabolites. Eight metabolites were identified by g.l.c.-mass spectrometry by comparison with synthetized reference compounds. 2. Metoprolol binds to cytochrome-P-450 eliciting a type I difference spectrum with KS = 23 +/- 2-0 muM. The apparent Michaelis-Menten constant Km = 39 +/

INTRODUCTION: Metoprolol is a widely used beta-adrenergic blocker that is commonly prescribed for a variety of cardiovascular syndromes and conditions. While central nervous system adverse effects have been well-described with most beta-blockers (especially lipophilic agents such as propranolol), visual hallucinations have been only rarely described with metoprolol.
CASE PRESENTATIONS: Case 1 was an 84-year-old Caucasian woman with a history of hypertension and osteoarthritis, who suffered from visual hallucinations which she described as people in her bedroom at night. They would be standing

Acute β-blockade with metoprolol has been associated with increased mortality by undefined mechanisms. Since metoprolol is a relatively high affinity blocker of β(2)-adrenoreceptors, we hypothesized that some of the increased mortality associated with its use may be due to its abrogation of β(2)-adrenoreceptor-mediated vasodilation of microvessels in different vascular beds. Cardiac output (CO; pressure volume loops), mean arterial pressure (MAP), relative cerebral blood flow (rCBF; laser Doppler), and microvascular brain tissue Po(2) (G2 oxyphor) were measured in anesthetized mice before and

Metoprolol succinate is a widely used medication for the management of hypertension, heart failure, and angina. We report the case of a 36-year-old woman who developed erythema multiforme after administration of a low dose of this drug. She also presented with pruritic burning pain throughout her body accompanied by chills. While erythema multiforme has been reported with carvedilol, this is the first observation of metoprolol succinate causing this and physicians should be aware of this potential, yet rare, side-effect.

Numerous pharmacogenomic (PGx) associations of pharmacokinetics and pharmacodynamics of commonly prescribed medications have been reported. Yet, the evidence supporting their association on survival remains underexamined. We sought to investigate the CYP2D6-metoprolol association and determine whether the established variability in drug exposure and hemodynamic response would translate into differences in mortality across metabolizer status in the Montreal Heart Institute Hospital Cohort. Single random plasma samples were collected from 996 patients receiving metoprolol tartrate. Bioanalytical

The study investigates the prognostic role of treatment with carvedilol as compared to metoprolol in patients with ventricular tachyarrhythmias. A large retrospective registry was used including consecutive patients on beta-blocker (BB) treatment with episodes of ventricular tachycardia (VT) or fibrillation (VF) from 2002 to 2015. Patients treated with carvedilol were compared to patients with metoprolol. The primary prognostic outcome was all-cause mortality at three years. Secondary endpoints comprised a composite arrhythmic endpoint (i.e., recurrences of ventricular tachyarrhythmias, approp

β-blockers are commonly used therapies after acute myocardial infarction and in the management of congestive heart failure and hypertension. We report a case of a middle-aged woman with a history of mild hypertension who was placed on metoprolol succinate. Before initiation of the β-blocker, her triglyceride level was in the borderline-high range (150-199 mg/dL). On treatment, her triglyceride levels exceeded 1000 mg/dL. She developed fatigue and mild abdominal discomfort but without biochemical evidence of pancreatitis. After discontinuation of metoprolol succinate, her triglyceride levels re

Atenolol (CAS 29122-68-7) and metoprolol (CAS 37350-58-6) are beta 1-selective adrenoceptor antagonists without intrinsic sympathomimetic activity. beta-Receptor blockers influence carbohydrate- and lipid metabolism. Liver is a central organ of these processes. The metabolic fate of a single oral atenolol and metoprolol dose and their actions on carbohydrate- and lipid parameters have been investigated. Healthy male rats received a dose reducing heart beat/min by 25% (atenolol: 6 mg/kg; metoprolol: 10 mg/kg). About 9% of atenolol is metabolized by cytochrome P-450 (P-450). P-450-dependent func

The hypertensive effect of urapidil, a new antihypertensive agent that acts via central and peripheral alpha-adrenoceptors, has been compared with that of metoprolol in 40 patients with mild essential hypertension. Blood pressure was significantly reduced by both drugs, while the heart rate was reduced only after metoprolol. The increases in systolic blood pressure and heart rate caused by three progressive work loads of bicycle exercise were not affected during urapidil, whereas both were reduced by metoprolol. A slight reduction in forced expiratory volume was observed in some patients durin

BACKGROUND: Beta-blockers have been the initial treatment for symptomatic obstructive hypertrophic cardiomyopathy (HCM) despite limited evidence of their efficacy. Aficamten is a cardiac myosin inhibitor that reduces left ventricular outflow tract gradients, improves exercise capacity, and decreases HCM symptoms when added to standard medications. Whether aficamten as monotherapy provides greater clinical benefit than beta-blockers as monotherapy remains unknown.
METHODS: We conducted an international, double-blind, double-dummy trial in which adults with symptomatic obstructive HCM were rand

BACKGROUND: Severe coronavirus disease-2019 (COVID-19) can progress to an acute respiratory distress syndrome (ARDS), which involves alveolar infiltration by activated neutrophils. The beta-blocker metoprolol has been shown to ameliorate exacerbated inflammation in the myocardial infarction setting.
OBJECTIVES: The purpose of this study was to evaluate the effects of metoprolol on alveolar inflammation and on respiratory function in patients with COVID-19-associated ARDS.
METHODS: A total of 20 COVID-19 patients with ARDS on invasive mechanical ventilation were randomized to metoprolol (15 m

BACKGROUND: Extensive clinical experience on metoprolol has been gained in different cardiovascular conditions.
REVIEW: This paper discusses the use of metoprolol, with a focus on the extended-release formulation, in clinical practice in the light of existing evidence and current recommendations. This is a narrative review based on existing evidence and clinical experience of the author. Evidence on the use of metoprolol in heart failure, ischemic artery disease, arterial hypertension, and atrial fibrillation is well-established.
CONCLUSIONS: Metoprolol still appears to be a suitable pharmac
