Determining the best method for assessing microbiological contamination on flexible endoscopes without a working channel: a pilot study.

BACKGROUND: Flexible endoscopes (FEs) without a working channel can become extensively contaminated. Adequate reprocessing is critical and the quality of these processes can be verified by microbiological sampling. This study aimed to determine the ability to detect colony-forming units (CFUs) after sampling used FEs without a working channel for four different sampling techniques. METHODS: FEs without a working channel were collected after clinical use. Manual pre-cleaning with tap water was performed. A sample was collected from the distal 8-10 cm and tip of the FE by one of four sampling t

The role of rapid and advanced microbiological methods in critical care: 2025 Emanuele Russo Delphi consensus.

SCOPE: Interpretation of rapid and advanced microbiological test results remains nonstandardized, with no existing reference guidelines. This study aimed to analyse the existing evidence and provide expert guidance on the use of these techniques in critically ill patients. METHODS: A Delphi consensus process was conducted by a multidisciplinary panel of experts, including microbiologists, infectious disease specialists, intensivists, surgeons, and pulmonologists. Sixteen prioritized key questions were addressed via literature reviews and two Delphi rounds. Consensus was reached when 70% of th

Executive summary of the consensus statement of the Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC), Spanish Society of Neurology (SEN), Spanish Society of Immunology (SEI), Spanish Society of Pediatric Infectology (SEIP), Spanish Society of Rheumatology (SER), and Spanish Academy of Dermatology and Venereology (AEDV), on the diagnosis, treatment and prevention of Lyme borreliosis.

The diagnosis of Lyme borreliosis (LB) is based on the epidemiological history, clinical manifestations and microbiological findings in the early disseminated and late phases of the disease. Related to this fact, microbiological diagnostic techniques have recently appeared. Far from facilitating the diagnosis and the clinical-therapeutic management of LB patients, they are generating confusion. Herein, experts and representatives of Spanish Scientific Societies [Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC), Spanish Society of Neurology (SEN), Spanish Society of Immu

Efficacy of the FilmArray blood culture identification panel for direct molecular diagnosis of infectious diseases from samples other than blood.

Molecular-based techniques reduce the delay in diagnosing infectious diseases and therefore contribute to better patient outcomes. We assessed the FilmArray blood culture identification (BCID) panel (Biofire Diagnostics/bioMérieux) directly on clinical specimens other than blood: cerebrospinal, joint, pleural and ascitic fluids, bronchoscopy samples and abscesses. We compared the results from 88 samples obtained by culture-based techniques. The percentage of agreement between the two methods was 75 % with a Cohen κ value of 0.51. Global sensitivity and specificity using the FilmArray BCID pane

Optimal microbiological sampling for the diagnosis of osteoarticular infection.

Infection is a dire complication afflicting every field of orthopaedics and traumatology. If specific clinical, laboratory and imaging parameters are present, infection is often assumed even in the absence of microbiological confirmation. However, apart from confirming infection, knowing the exact infecting pathogen(s) and their antimicrobial susceptibility patterns is paramount to help guide treatment. Every effort should therefore be undertaken with that goal in mind. Not all microbiological findings carry the same relevance, and knowing exactly how and where a sample was collected is key. Se

Two-step method for isolating Cryptococcus species complex from environmental material using a new selective medium.

Cryptococcosis is an opportunistic infection caused by the Cryptococcus species complex. An outbreak of cryptococcosis caused by Cryptococcus gattii (AFLP6/VGII) in North America has indicated the need for studies of this organism and its environmental niche. Difficulties in isolating the Cryptococcus spp. because of the overgrowth of filamentous fungi onto culture media and its low fungal population size under natural conditions limit studies of these pathogenic yeasts. We designed a selective medium that inhibits the growth of environmental filamentous fungi but does not inhibit that of Cryp

Efficient methods for large-area surface sampling of sites contaminated with pathogenic microorganisms and other hazardous agents: current state, needs, and perspectives.

The recovery operations following the 2001 attacks with Bacillus anthracis spores were complicated due to the unprecedented need for large-area surface sampling and decontamination protocols. Since this event, multiple reports have been published describing recovery efficiencies of several surface sampling materials. These materials include fibrous swabs of various compositions, cloth wipes, vacuum socks, and adhesive tapes. These materials have reported recovery efficiencies ranging from approximately 20% to 90% due to the many variations in their respective studies including sampling materia

Prokaryotic names: the bold and the beautiful.

In recent years, names of ∼170 new genera and ∼1020 new species were added annually to the list of prokaryotic names with standing in the nomenclature. These names were formed in accordance with the Rules of the International Code of Nomenclature of Prokaryotes. Most of these names are not very interesting as specific epithets and word elements from existing names are repeatedly recycled. The rules of the Code provide many opportunities to create names in far more original ways. A survey of the lists of names of genera and species of prokaryotes shows that there is no lack of interesting names

A new culture medium for recovering the agents of Cryptococcosis from environmental sources.

The isolation of Cryptococcosis agents from environmental samples may be difficult due to the presence of groups of fast-growing fungi. We propose a new culture medium based on a modification of Dichloran Rose-Bengal Chloramphenicol Medium (DRBCm) to detect colonies of Cryptococcus neoformans. Our results indicate that DRBCm is superior to the classical Bird Seed Agar in its ability to detect colonies of C. neoformans.

Chronic Osteomyelitis of the Jaw: Pivotal Role of Microbiological Investigation and Multidisciplinary Management-A Case Report.

A 15-year-old girl with a history of recurrent painful orofacial swelling was diagnosed on the basis of clinical findings, histopathological examination and imaging modalities as having primary chronic osteomyelitis of the jaw. Initial microbiological samples were performed but were inconclusive. She received multiple empirical antibiotic therapies and NSAIDs for 3 years without complete remission. Only MALDI-TOF (Matrix-Assisted Laser Desorption/Ionization-Time Of Flight) analysis after additional multiple microbiological bone samples with adequate techniques yielded the final diagnosis of ba

Microbial Composition of Carious Dentin and the Impact of Minimally Invasive Excavation Techniques: A Narrative Review.

Dental caries is a biofilm-mediated dysbiotic disease characterized by ecological shifts within the oral microbiome and progressive demineralization of dental hard tissues. The microbiological composition of carious dentin and the impact of minimally invasive excavation techniques on residual microbial communities remain subjects of ongoing investigation due to methodological heterogeneity and inconsistencies among published studies. This narrative review aimed to summarize current evidence regarding the microbial ecology of carious dentin, compare culture-based and molecular methods for micro

[Managing lower respiratory tract infections in immunocompetent patients. Definitions, epidemiology, and diagnostic features].

Particular attention is given to lower respiratory tract infections because of their frequency and potential severity. These infections represent a major cause of death worldwide and pneumonia remains the first cause of death from infectious origin in France. Three nosological entities are usually described according to the anatomic localization of the infectious process: acute bronchitis, pneumonia, and bronchopneumonia (if the infection involves the bronchial tree and the lung parenchyma). If bronchial infection occurs within the context of a chronic respiratory tract disease, it is called a

Differences of lung microbiome in patients with clinically stable and exacerbated bronchiectasis.

BACKGROUND: Molecular-based diagnostic techniques can compensate for the inherent limitations of culture-based microbiology and provide a more comprehensive description of an entire community of bacteria at a particular anatomical site. Using culture-independent DNA-based molecular techniques, the aim of the present study was to characterize, differentiate, and compare the composition of lower airway bacterial microbiome between clinically stable and acutely infected patients with bronchiectasis experiencing exacerbation. METHODS: Patients with clinically stable bronchiectasis and those exper

Skin and soft tissue infections: swab or biopsy? A diagnostic dilemma.

PURPOSE OF REVIEW: Skin and soft tissue infections (SSTIs) are frequent clinical conditions in which microbiological investigation is selectively indicated. When an etiological diagnosis is pursued, the optimal sampling method - superficial swab vs. deep tissue biopsy or aspirate - remains controversial. This review critically examines the diagnostic performance, clinical impact, and limitations of different sampling strategies in SSTIs. RECENT FINDINGS: Evidence consistently shows that deep tissue biopsy or aspirate provides a higher microbiological yield and better correlation with clinical

The biopsy dilemma in spinal infections: timing of antibiotics and diagnostic yield.

PURPOSE: Establishing a microbiological diagnosis is critical in managing spinal infections, yet culture yields from biopsy are inconsistent and may be influenced by prior antibiotic therapy. This systematic review aimed to evaluate the impact of pre-biopsy antibiotic exposure on microbiological yield and to compare diagnostic performance across biopsy modalities. METHODS: Following PRISMA guidelines, five databases (Ovid MEDLINE, EMBASE, Web of Science, Cochrane Library, Scopus) were searched through June 2025. [Microbiology of periodontitis. 1. The infective nature of periodontitis].

In the last years, guided tissue regeneration (GTR) and implantology have opened new possibilities for practical periodontologists in the treatment of advanced cases. For both techniques the successful therapy of the local infection of the pocket is a prerequisite. In the near future rapid tests for microbiological diagnoses and slow release devices for antibiotics will be available to achieve this aim easier. The following article will present the microbiological aspects of periodontitis and plaque control and show their relevance for the practitioner.
