The prevalence in active duty military service members of 30-day DSM-IV psychiatric disorders, including posttraumatic stress disorders and major depressive disorder, is greater than among sociodemographically-matched civilians. Only 23-40% of returning military who met strict criteria for any mental health problem in 2004 had received professional help in the past year. One-fourth of Regular Army soldiers meet criteria for a 30-day DSM-IV mental disorder, two-thirds of whom report a pre-enlistment age of onset. Both pre- and post-enlistment age of onset are predictors of severe role impairmen

This study examined the gambling of a cohort of U.S. Air Force recruits (N = 31,104) and the relationship between their gambling and health-risk behaviors. Participants provided self-report data regarding gambling and health-related behaviors. Results suggest that 10.4% of participants gambled weekly or more often, 6.2% reported gambling problems, and 1.9% acknowledged loss of control over gambling. Men were more likely than women to report weekly gambling and possible problematic gambling. Minorities, compared to Caucasians, were more likely to experience gambling problems and report loss of

Multiple strategies have been proposed to improve health care in the United States. These include the development of communities of solution (COSs), implementation of patient-centered medical homes (PCMHs), and lengthening family medicine residency training. There is scant literature on how to build and integrate these ideal models of care, and no literature about how to build a model of care integrating all 3 strategies is available. The Military Health System has adopted the PCMH model and will offer some 4-year family medicine residency positions starting in 2013. Lengthening residency trai

The Defense Health Board conducted a year-long examination of mental health accession screening and related issues. In its August 2020 report, Examination of Mental Health Accession Screening: Predictive Value of Current Measures and Processes, the Board recommends a paradigm shift in how mental health impacts on readiness are understood and addressed. This shift can only occur with the development and implementation of a research plan that follows cohorts of military personnel from recruitment through their military career. The following article describes this research plan as an excerpt of t

BACKGROUND: Healthy People (HP) 2010 is a national health promotion and disease prevention initiative of the U.S. Department of Health and Human Services. The HP 2010 report highlighted a range of racial/ethnic disparities in dental health. A substantial portion of these disparities appear to be explained by differences in access to care. Members of the U.S. military have universal access to care that also has a compulsory component. The authors conducted a study to investigate the extent to which disparities in progress toward achievement of HP 2010 objectives were lower among the military po

INTRODUCTION: Musculoskeletal injuries are an endemic amongst U.S. Military Service Members and significantly strain the Department of Defense's Military Health System. The Military Health System aims to provide Service Members, military retirees, and their families the right care at the right time. The Military Orthopedics Tracking Injuries and Outcomes Network (MOTION) captures the data that can optimize musculoskeletal care within the Military Health System. This report provides MOTION structural framework and highlights how it can be used to optimize musculoskeletal care.

Background: Military academy cadets face training, study, and adaptational pressures that elevate the risk for mental health problems and functional impairment. Existing tools are either generic symptom checklists or narrow resilience scales; none integrates environmental risk, individual vulnerability, and protective resources within a unified hierarchical model relevant to this population. Methods: We developed and psychometrically evaluated the Military Cadets' Mental Health Risk Scale (MCMHRS). After item generation and two-round Delphi review (n = 15), a 64-item pool was administered to 6

PURPOSE: Healthcare provision in the United Kingdom (UK) falls primarily to the National Health Service (NHS) which is free at the point of access. In the UK, there is currently no national marker to identify military veterans in electronic health records, nor a requirement to record it. This study aimed to compare the sociodemographic characteristics and recorded mental health diagnoses of a sample of veterans and civilians accessing secondary mental health services.
METHODS: The Military Service Identification Tool, a machine learning computer tool, was employed to identify veterans and civ

Chlamydia psittaci infection among humans (psittacosis) and pet birds (avian chlamydiosis), also known as parrot disease, parrot fever, and ornithosis, is a zoonotic bacterial disease. Humans most often become infected by inhaling the organism when urine, respiratory secretions, or dried feces of infected birds are dispersed in the air as very fine droplets or dust particles. C. psittaci infection of humans can cause influenza-like symptoms, such as fever of abrupt onset, pronounced headache, and dry cough, and can lead to severe pneumonia and non-respiratory health problems. Infection can als

INTRODUCTION: The immediate access to detailed patient data is vital for effective medical care in military and emergency scenarios, enhancing diagnosis, treatment, and monitoring of military personnel. The integration of electronic health records (EHRs) is urgently needed in military healthcare systems, despite the distinct hurdles involved. Current literature on EHR use in military healthcare is lacking and disjointed. This study aims to bridge this gap through a systematic review, offering a thorough examination of the advantages, obstacles, and recommended strategies for implementing EHRs

INTRODUCTION: Female aviators and aircrew face unique occupational exposures, including noise, whole-body vibrations, toxic chemicals, intermittent hypoxia, and high gravitational forces. We evaluated associations between maternal occupations as aviators and aircrew during pregnancy and adverse pediatric health outcomes.
METHODS: We conducted a retrospective cohort study of children born to female U.S. military officers from October 2002 to December 2019. Exposure was defined as the mother serving in an aviation or aircrew occupation at time of birth. Adverse health outcomes were identified b

INTRODUCTION: The aviation occupational environment may expose a developing fetus to intermittent hypoxia, high gravitational force, toxic materials, loud noise, high frequency vibrations, and galactic cosmic radiation. These exposures in animal models are associated with adverse neonatal outcomes. We sought to investigate whether a maternal military aviation career was associated with adverse neonatal health outcomes.
METHODS: We performed a retrospective cohort study of female officer's children born in the Military Health System from October 2002 to December 2019. Female fixed-wing aviatio

Blood transfusion is required in a number of emergency settings and the French military health service (FMHS) has issued specific guidelines for the treatment of war casualties. These guidelines take into account European standards and laws, NATO standards, and also public sentiment regarding transfusion. These guidelines reflect a determination to control the process and to avoid the improvisation frequently associated with wartime transfusion. The evolution in warfare (terrorism and bombing more frequent than gunshot) and the wide use of body armor have deeply changed the clinical presentati

Ciguatera toxin is a marine neurotoxin produced by microorganisms that becomes concentrated in predatory fish. Toxicity in humans results from the ingestion of contaminated fish harvested in tropical waters. Clinical manifestations of illness include the rapid onset of gastrointestinal symptoms and neurological abnormalities. Because of the rapid onset of symptoms and the potential for case clusters from a common source ingestion of contaminated fish, there is the potential that ciguatera poisoning may initially mimic illnesses caused by antipersonnel biological and chemical agents. We present

OBJECTIVE: This study examined the mental health and resilience of combatants and their partners during the 2023 Israel-Hamas war, with a focus on self-compassion as a protective factor. While soldiers' mental health has been widely studied, the psychological burden on their partners, especially during active conflict, is less understood. The transition of reservists from civilian to combat roles, coupled with the upheaval faced by their families, provides a unique opportunity to assess resilience in this dyadic context.
METHOD: A total of 134 couples (N = 268), combatants serving in Gaza and

OBJECTIVES: Although studies have described inequities in spinal cord stimulation (SCS) receipt, there is a lack of information to inform system-level changes to support health care equity. This study evaluated whether Black patients exhaust more treatment options than do White patients, before receiving SCS.
MATERIALS AND METHODS: This retrospective cohort study included claims data of Black and non-Latinx White patients who were active-duty service members or military retirees who received a persistent spinal pain syndrome (PSPS) diagnosis associated with back surgery within the US Military

INTRODUCTION: Cystic fibrosis (CF) is the most common life-threatening genetic illness in the United States. People with CF as well as their caregivers are up to three times more likely to report experiencing symptoms of depression and anxiety than those without CF. In 2016, the Cystic Fibrosis Foundation and the European Cystic Fibrosis Society came together to form the International Committee on Mental Health in CF and released guidelines outlining behavioral health (BH) screening recommendations for patients with CF and at least one primary caregiver. This study sought to characterize the r
