Human milk banks (HMBs) are an essential clinical service, providing safe, screened donor human milk (DHM) to clinically vulnerable infants, which can enable mothers to establish their own milk supply. While HMB processes share similarities with other medical products of human origin (e.g., blood, platelets), HMBs remain marginalised services with limited resilience to external or internal pressures. As demand for DHM increases, understanding how to mitigate these vulnerabilities is essential. In November 2022, a workshop organised by the Human Milk Foundation brought together UK milk bank lea

Many infants are nurtured with milk supplied by human banks, whose bacteriological and physical-chemical profiles are a major issue. We investigated the bacteriological and physical-chemical characteristics, as well as genotypic and phenotypic and profiles of Staphylococcus species isolated from 240 samples of breast milk from a bank in a teaching hospital. Dornic acidity of milk revealed that 95.4% (229/240) had acceptable limits (< 8.0 oD). Caloric intake showed a wide variation in cream content (4%), fat (4%) and energy values (559.81 Kcal/L). Staphylococcus (105/186 or 56.5%) and Enterobac

BACKGROUND: The importance of breast-feeding for very low birthweight (VLBW) infants has been pointed out. Some overseas studies suggested that the standardization of enteral nutrition (EN) leads to improved prognosis in VLBW infants. In Japan, however, physicians in charge of infants are responsible for making nutrition management decisions on an individual basis. We conducted an online survey to clarify the course of nutrition management of VLBW infants currently implemented in Japan.
METHODS: We mailed a notice to 300 representative neonatologists throughout Japan requesting their particip

BACKGROUND: Donor Human Milk (DHM) is recommended for preterm infants when a mother's own milk is unavailable. This study explored Saudi mothers' knowledge, attitudes, and concerns regarding human milk banking (HMB) and donation, to identify barriers to acceptance and to inform future implementation.
METHODS: A cross-sectional survey of 835 mothers across Saudi regions assessed support for HMB and its association with sociodemographic factors, feeding practices, breastfeeding duration, and knowledge of the benefits of breast milk. Attitudes toward religious permissibility and willingness to a

BACKGROUND: The World Health Organization (WHO) strongly recommends the use of donor human milk (DHM) for low birth weight infants when mother's own milk is unavailable or insufficient. However, the use of DHM requires the availability of human milk banks (HMBs), the majority of which are in middle and high-income countries. Developing countries offer multiple opportunities and challenges regarding the establishment and operationalization of HMBs. This study describes the experience in setting up the HMB in Uganda at St. Francis Hospital Nsambya.
METHODS: The establishment of the first HMB in

Iran established its first human milk bank (HMB) in 2016 to address high rates of neonatal mortality and prematurity-related complications. A national network of 19 HMBs was developed using a sustainable model that integrated local technological innovation and religious consensus. By partnering with domestic knowledge-based companies to manufacture automated pasteurizers, the program successfully bypassed international trade sanctions that made imported equipment cost-prohibitive. Furthermore, the program secured official Fatwas from high-ranking Shia authorities and a formal letter of consent

Feed intolerance and necrotising enterocolitis (NEC) are challenges while treating sick neonates. These can be reduced by giving human milk, but adequate availability of mother's own milk or pasteurised donor human milk (PDHM) is a challenge in neonatal setups, like ours, without an attached human milk bank. Hence, this quality improvement initiative was taken to improve donor human milk collection in our urban tertiary-care teaching hospital, to at least 500 mL per week in 4 weeks. After analysing the problem, our quality improvement team identified a shortage of human milk donation that was

BACKGROUND: Human milk banking has been promoted to provide donated breast milk for at-risk children whose mothers cannot breastfeed them but this effort was hindered by the advent of HIV epidemic.
OBJECTIVE: To estimate the seroprevalence of HIV, syphilis and hepatitis B in the blood of human milk donors registered in a major maternity hospital in the northern region of the Santa Catarina State, Brazil.
METHODS: A retrospective study included serological tests for HIV, syphilis and hepatitis B screening of milk donor candidates in the 2005-2015 period. The 95% confidence intervals were calc

For its specific qualitative characteristics human donor milk (DM) is the main alternative to preterm infants nutrition and growing. How several studies suggest child's physical and mental development is influenced by breastfeeding that prevents the necrotizing enterocolitis (NEC), retinopathy of prematurity (ROP), and sepsis common in preterm newborns. Our research was conducted in NICU's Human Milk Bank (HMB) "Allattiamolavita." Our study was based on macronutrients analysis (MA) of 100 DM samples taken until 10 days since childbirth and analyzed by spectroscopic infrared innovative method (

BACKGROUND: Banked human milk (BHM) has inherent infectious risks, even when pasteurized. Because of the ubiquity of Bacillus cereus in the environment and its ability to resist the Holder pasteurization process, there is a concern that BHM might lead to severe B. cereus infections.
OBJECTIVE: We reviewed observed and published cases to determine the potential causal role of BHM as the source of these infections.
METHODS: Two infants in the province of Québec (Canada) developed a B. cereus neonatal infection, and both had received BHM. We conducted bacteriological studies to compare clinical

BACKGROUND: Although human breast milk improves child survival, evidence on its acceptability among healthcare professionals in low-income settings such as Ethiopia remains limited. This study assessed the acceptability of human milk banking (HMBs) and identified factors influencing healthcare professionals’ acceptance of HMBs in public hospitals in eastern Ethiopia. METHODS: A facility-based, multicenter, and mixed-methods study was conducted in four public hospitals from August to September 2024. The quantitative component used simple random sampling to select 361 healthcare professionals, w

We assessed the potential effectiveness of human milk banking and lactation support on provision of human milk to neonates admitted in the newborn unit (NBU) at Pumwani Maternity Hospital, Kenya. This pre-post intervention study collected data from mothers/caregivers and their vulnerable neonates or term babies who lacked sufficient mother's milk for several reasons admitted in the NBU. The intervention included establishing a human milk bank and strengthening lactation support. Preintervention data were collected between 5 October 2018 and 11 November 2018, whereas postintervention data were

BACKGROUND: Although exclusive human milk feeding is beneficial for very low birth weight infants, rates remain suboptimal globally. This hybrid type 1 effectiveness-implementation trial evaluated structured lactation support combined with human donor milk banking in neonatal intensive care units (NICUs), primarily assessing effectiveness, with secondary assessment of implementation and economic outcomes.
METHODS: We conducted a stepped-wedge cluster-randomized trial in 15 German Level I NICUs (April 2022-March 2024). The Neo-MILK intervention comprised structured lactation counselling, multi

BACKGROUND: Human milk banks (HMBs) offer the best feed for neonates after mother's own milk (MOM), especially when MOM is insufficient. Although HMBs are founded on standard protocols, contamination and wastage of milk due to positive milk cultures remain a problem. Present study was planned as a quality improvement (QI) initiative to reduce culture rates at the HMB.
AIMS STATEMENT: The aim is to reduce the milk-culture positivity rates at the HMB by 50% or more, over a span of 3 months.
MATERIALS AND METHODS: A QI initiative was undertaken at the milk bank of a tertiary-care hospital in We

OBJECTIVE: We examined the effect of two strategies to prevent necrotizing enterocolitis (NEC) in extremely low birth weight (ELBW) infants-adherence to a standardized feeding protocol and use of a hospital-based milk bank to provide exclusive preterm human milk feedings.
STUDY DESIGN: We conducted a single-center observational study from 2010 to 2015. Infants received preterm human milk, initially trophic feeds from days 7 to 14 after birth, followed by advancement of 15 mL/kg/day to reach a goal of 180 mL/kg/day. Fortification was used selectively for weight gain < 15 g/kg/day. We determine
