A selective sac extraction method: another minimally invasive procedure for inguinal hernia repair in children: a technical innovation with satisfactory surgical and cosmetic results.

PURPOSE: To achieve satisfactory surgical and cosmetic results with minimal surgical invasiveness without laparoscopic assistance in childhood inguinal hernia repair, a novel technique, the selective sac extraction method (SSEM), was devised. The technical feasibility of this method was retrospectively examined. METHODS: The method was applied to 162 consecutive hernia repairs in 153 patients, 79 boys and 74 girls, excluding patients with incarcerated or irreducible hernia, sliding hernia of the ovary (hernia with palpable ovary at repair), or hernia associated with an undescended testis. Her

Surgical Performance Determines Functional Outcome Benefit in the Minimally Invasive Surgery Plus Recombinant Tissue Plasminogen Activator for Intracerebral Hemorrhage Evacuation (MISTIE) Procedure.

BACKGROUND: Minimally invasive surgery procedures, including stereotactic catheter aspiration and clearance of intracerebral hemorrhage (ICH) with recombinant tissue plasminogen activator hold a promise to improve outcome of supratentorial brain hemorrhage, a morbid and disabling type of stroke. A recently completed Phase III randomized trial showed improved mortality but was neutral on the primary outcome (modified Rankin scale score 0 to 3 at 1 yr). OBJECTIVE: To assess surgical performance and its impact on the extent of ICH evacuation and functional outcomes. METHODS: Univariate and mult

A novel technique for transdiaphragmatic latero-pericardial cardiophrenic lymph node excision using the minimally invasive surgical access procedure in patient with advanced stage ovarian cancer.

This study reports the first case of transdiaphragmatic lateropericardial cardiophrenic lymph node excision using the GelPOINT™ mini access platform in a patient with advanced-stage ovarian cancer. A 69-year-old woman with high-grade serous epithelial ovarian cancer. Cardiophrenic lymph node dissection is vital in advanced ovarian cancer surgery, as enlarged nodes are linked to poor prognosis. No clear guidelines exist for operating on patients with enlarged cardiophrenic lymph nodes [1,2]. These nodes are categorized by location relative to the heart: anterior, median (lateropericardial), and

The minimally invasive surgical local osteo-enhancement procedure (LOEP) to deliver a resorbable, tri-phasic calcium-based implant material to address bone loss and strengthen the proximal femur.

With global population growth and advancing age, fragility fractures present a major healthcare challenge that current approaches have not resolved. Although pharmacological agents have been shown to reduce fragility fracture risk, there remain unmet needs in clinical care, especially for patients at imminent risk of hip fracture, given the delay between treatment initiation and observed protective effect. This gap suggests a need for novel approaches, including hip procedures that strengthen bone locally and quickly. The purpose of this report is to describe a procedural technique that has be

[Cardiac surgery 2.0].

Minimally invasive approach has become key in cardiac surgery over the last decade and nowadays involves all areas of expertise, as it was initially developed for mitral valve surgery and has quickly expanded also to surgery for the treatment of aortic valve disease and to myocardial revascularization. Compared to standard sternotomy, the most evident difference is minimalization of surgical incision, which reduces chest trauma with several benefits for patients. Minimally invasive cardiac surgery requires dedicated instrumentation/devices as well as an additive learning curve. Different perfu

The "folded" glove-finger in minimally invasive surgery: easy, quick and cheap specimen retrieval.

Laparoscopic and thoracoscopic surgery often require extraction of surgical specimens. The use of standard retrieval bag can be expensive, especially in case of multiple specimens during the same operation. We describe the use of a folded glove-finger as cheap alternative and easily reproducible means of specimen extraction. The medium finger of a glove, cut at its base, was used to locate several types of specimens. The edge of the glove was folded to allow an easier insertion and retrieval of the specimens. Using this technique in the last 5 years, we have retrieved hundreds of different typ

[Current Status and Future Prospects of Minimally Invasive and Function-Preserving Surgery].

The keywords of surgical treatment in the 21st century are "minimally invasive" and "function preserved". "Laryngeal function" is important in the head and neck region. The goal of laryngeal function preservation surgery is to be able to talk on "telephone" (vocalization function), to be able to "eat out" (chewing/swallowing function), "bath to shoulder" (nasal respiration function). Minimally invasive surgery and function-preserving surgery for head and neck cancer have been developed mainly for transoral surgery. In Japan, oral endoscopic surgery is the main surgical method. Transoral videol

Key-lock type mini-sternotomy: an alternative approach for adult cardiac operations.

An alternative technique for minimally invasive aortic valve replacement and atrial septal defect repair is described. After a 5-cm skin incision, a key-lock type sternotomy is made. Excellent exposure of the right atrium and aortic valve was achieved. The configuration of the mini-sternotomy (or the lock) limits the movement of the sternal surfaces (or the key) on the lateral and craniocaudal directions.

Minimally invasive periodontal therapy: will periodontal therapy remain a technologic laggard?

Minimally invasive therapeutic approaches have become the standard of care for many medical procedures. In contrast, the use of minimally invasive techniques in non-surgical and surgical periodontal therapy has not progressed to the same extent. This commentary explores some of the technologic forces that influence the acceptance of minimally invasive therapeutic modalities. There is adequate science to support the development and clinical use of minimally invasive periodontal treatment but the technology to perform minimally invasive procedures is not currently available. Potential explanatio

Minimally Invasive Esophagectomy: A Consensus Statement.

BACKGROUND: Minimally invasive esophagectomy (MIE) is increasingly performed in various ways. The lack of international definitions and nomenclature makes accurate comparison of outcomes difficult. METHODS: An international, multispecialty consensus-writing committee constructed definitions and nomenclature for MIE. After a PubMed search, vetting, and review with all authors, a consensus was reached. RESULTS: The proposed definition for MIE is an operation "that removes part or all of the esophagus, does not retract, lift, spread or remove any part of the chest or abdominal wall and the surg

Direct mini-incision parathyroidectomy.

We describe our technique for performing a mini-incision parathyroidectomy in patients with hyperparathyroidism. Since our procedure differs from conventional parathyroidectomy in requiring a 2- or 3-cm skin incision and no raising of skin flap, this technique resulted in a good cosmetic status and less invasiveness. Hypesthesia or paresthesia in the neck and discomfort while swallowing related to a large skin incision and raising of skin flap are minimized. Although the cosmetic results of endoscopic techniques are better than those of our procedures, endoscopic instruments remain traumatic,

How I do it: transnasal retraction during transoral robotic oropharyngeal resection.

Collapse of the resection plane presents a frustrating problem during transoral robotic resection, in a situation already typified by limited vision and access for instruments. We present a quick and cost-effective retraction technique to effectively mitigate this issue and increase the ease and reliability of robotic oropharyngeal resection. This technique utilises a simple transnasal apparatus to create greater exposure of the resection plane. A Y-suction catheter is inserted into the oropharynx via the nasal cavity. A silk suture is then used to attach it to the oropharyngeal resection spec

Periocular plastic surgery.

BACKGROUND: Good vision depends on the normal anatomy and function of the eyelids and orbital structures. The goals of periocular ophthalmic plastic surgery are the anatomical and functional preservation and restoration of the lids, orbits, and periorbital structures when they are affected by congenital or acquired malpositions, defects and mass lesions. In this region, functional and esthetic considerations are closely linked. METHOD: This review is based on selected articles retrieved by a PubMed search, the guidelines of the German Ophthalmologists' Association (Bundesverband der Augenärzt

Clinical application of flexible and variable transparent endport technology in minimally invasive neuroendoscopic keyhole surgery: a practical new technology.

With the development of endoscopic technology, neuroendoscopy utilizes its advantages such as good illumination, close range observation, and flexible degrees of freedom. Neuroendoscopic surgery can achieve advantages such as minimally invasive, high clearance rate, low incidence of complications, good brain tissue protection, and fewer surgery-related injuries. However, minimally invasive endoscopic surgery also has inherent limitations, since its narrow surgical channels are prone to collapse and require special instrument support. By summarizing the literature on the use of endoport technol
