PURPOSE OF REVIEW: Although mitochondrial diseases impose a significant functional limitation in the lives of patients, treatment of these conditions has been limited to dietary supplements, exercise, and physical therapy. In the past few years, however, translational medicine has identified potential therapies for these patients.
RECENT FINDINGS: For patients with primary mitochondrial myopathies, preliminary phase I and II multicenter clinical trials of elamipretide indicate safety and suggest improvement in 6-min walk test (6MWT) performance and fatigue scales. In addition, for thymidine k…

Histological mitochondrial changes are generally found to be associated with late onset myofibrillar myopathies (MFMs). How these changes contribute to the pathogenesis of MFMs is unknown. Mitochondrial changes, including COX-deficient fibers (n = 8), biochemical activities of respiratory chain complexes (n = 7), and multiple mtDNA deletions by long-range PCR (n = 9) were examined in patients with genetically confirmed MFMs [MYOT (n = 2), DES (n = 1), ZASP (n = 2), FLNC (n = 4)] and compared with age and sex matched normal controls (n = 27) and patients with a mitochondrial disorder with multi

Mitochondrial myopathies cover a diverse group of disorders in which ragged red and COX-negative fibers are common findings on muscle morphology. In contrast, muscle degeneration and regeneration, typically found in muscular dystrophies, are not considered characteristic features of mitochondrial myopathies. We investigated regeneration in muscle biopsies from 61 genetically well-defined patients affected by mitochondrial myopathy. Our results show that the perturbed energy metabolism in mitochondrial myopathies causes ongoing muscle regeneration in a majority of patients, and some were even a…

In this paper, we have described mitochondrial cytochemistry (NADH oxidase, cytochrome c oxidase), in the light and electron microscopic studies of the experimental mitochondrial myopathies. DNP, oleate and crotoxin were employed to produce mitochondrial changes in the rat skeletal muscle. The DNP and oleic acid lesions showed strong NADH oxidase and cytochrome c oxidase activities in mitochondria--both at the light microscopic as well as at ultrastructural level. However, the crotoxin lesions showed marked reduction of both the enzyme activities in histochemistry and electron microscopy. Musc…

OBJECTIVE: To investigate the symptomatic, biochemical, and pathological characteristics of mitochondrial myopathy and mitochondrial encephalomyopathy.
METHODS: Physical examination, electromyography, electroencephalography, cranial CT or MRI, serum enzymological examination, and light microscopy and electron microscopy of muscle biopsy specimens were made among twenty-one in-patients with the diagnosis of mitochondrial myopathy and mitochondrial encephalopathy. All the patients were followed up for more than 5 years.
RESULTS: Four patients died of lung infection, epilepticism or multiple or

PURPOSE OF REVIEW: Treatment options for mitochondrial myopathies remain limited despite rapid advances in the understanding of the molecular basis of these conditions. Existing therapies continue to be evaluated and novel treatment strategies are starting to appear on the horizon.
RECENT FINDINGS: Exercise training continues to show promise as a method of improving exercise tolerance and enhancing oxidative capacity. Coenzyme Q10 deficiency appears to be a relatively common finding in mitochondrial disorders and is likely to benefit from exogenous supplementation. Large-scale randomized clin…

Mitochondrial myopathies are heterogeneous group of clinical disorders that can affect multiple systems besides skeletal muscles. The mitochondrial abnormalities in the skeletal muscles are morphologically identified by the presence of characteristic Ragged-red fibers (RRF) in the cryostat sections of the muscle stained with modified Gomori's trichrome stain. In this retrospective study, clinical and histopathological features in six patients with mitochondrial myopathies have been analysed. The utility of histochemical methods in confirming the diagnosis of mitochondrial myopathy has been emp…

By means of the modified trichromic method of Gomori, introduced by Engel and Cunningham in 1963 [1], it has been possible to demonstrate the presence of ragged-red fibers (RRF) in the mitochondrial myopathies. At present, to study these disorders in muscle biopsies, one may also use morphological, ultrastructural, histochemical, fluorescent, immunological and molecular biology methods. However, the alterations thus demonstrated in striated muscle are not always present in all these diseases due to mitochondrial alterations. At the same time, the multisystemic nature of these diseases also imp…

Peripheral neuropathy has attracted relatively little attention in mitochondrial myopathy. However, mitochondrial myopathies are clinically heterogeneous disorders that can affect multiple systems including peripheral nerves other than the skeletal muscle. In addition to the survey of the literature, we studied 6 cases of mitochondrial myopathy with peripheral neuropathy; 3 cases of oligo-systemic involvement confined mainly to skeletal muscles and peripheral nerves, and 3 cases of multi-systemic involvement diagnosed as myoclonus epilepsy with ragged-red fibers (MERRF) or mitochondrial myopat…

Increased susceptibility to apoptosis has been shown in many models of mitochondrial defects but its relevance to human diseases is still discussed. We addressed the presence of apoptosis in muscle from patients with mitochondrial DNA (mtDNA) disorders. Taking advantage of the mosaic pattern of muscle morphological anomalies associated with heteroplasmic mtDNA alterations, we have used an in situ approach to address the relationship between apoptosis and respiratory defect, mitochondrial proliferation and mutation load. Different patterns of mitochondrial morphological alterations were provide

OBJECTIVES: Idiopathic inflammatory myopathies (IIMs) are a heterogeneous group of acquired muscle disorders characterised by immune-mediated muscle damage and systemic involvement. Increasing evidence highlights mitochondrial abnormalities as a key contributor to muscle weakness, inflammation, and disease progression. This review aims to summarise current knowledge on the mechanisms, histopathological features, and clinical implications of mitochondrial dysfunction in IIMs, as well as to discuss emerging therapeutic strategies targeting mitochondrial impairment.
METHODS: A narrative review o

Mitochondria, the energy factories of human organisms, can be the cause of a variety of genetic disorders called mitochondrial myopathies. Mitochondrial diseases arise from genetic alterations in either mitochondrial DNA (mtDNA) or nuclear DNA (nDNA) and can manifest with great heterogeneity, leading to multiorgan dysfunction. The purpose of this article is to concisely review the pathophysiology, genetics and main clinical features of mitochondrial myopathies, focusing mainly on the treatment and management of these disorders. Currently, a particular treatment for mitochondrial myopathies doe…
