Relevance of molecular medicine to clinical obstetrics and gynecology.

Understanding molecular biology can improve the clinical acumen of the practicing obstetrician/gynecologist. An area of basic research now becoming clinically relevant involves the G proteins and G protein-coupled receptors. Clinicians already manipulate G protein-coupled receptors in their daily practice. Examples include the administration of oxytocin (oxytocin receptors), beta-2 tocolytic agents (beta 2-adrenergic receptors), GnRH agonists (GnRH receptors), exogenous gonadotropins (FSH and LH receptors), and bromocriptine (dopamine receptor). Clinically important disorders presenting to the

Illuminating the Invisible: Translational Molecular Imaging in Cardiovascular Medicine.

Cardiovascular disease remains the predominant cause of global morbidity and mortality, with recent epidemiological trends indicating a resurgence in adverse outcomes. Conventional imaging modalities-including coronary computed tomography angiography, echocardiography, and cardiac magnetic resonance imaging-are indispensable for anatomic and functional assessment, yet they lack the sensitivity to detect subclinical molecular perturbations that precede overt pathology. The advent of molecular imaging, leveraging nuclear, magnetic resonance, computed tomography, contrast-enhanced ultrasound, and

Driving Precision Medicine in Pancreatic Cancer With Molecular Genetics.

Pancreatic ductal adenocarcinoma (PDAC) remains one of the most lethal malignancies with dismal 5-year survival rates. Characterized by its complex tumor microenvironment and resistance to conventional therapies, PDAC has historically been difficult to treat effectively. Recent advances in molecular profiling have revolutionized our understanding of the genetic landscape of pancreatic cancer. This review examines how molecular genetics is driving precision medicine approaches in pancreatic cancer, with particular focus on KRAS-targeted therapies. Understanding these developments is crucial for

Precision medicine using molecular-target drugs in psoriatic arthritis.

Psoriatic arthritis (PsA) presents various clinical manifestations, including skin lesions, peripheral arthritis, axial involvement, enthesitis, nail involvement, dactylitis, and uveitis. In addition, it causes a high incidence of lifestyle-related diseases and an increase in cerebrovascular and cardiovascular events. As the pathology of PsA has been clarified, molecular-targeted drugs targeting tumor necrosis factor-α, interleukin (IL)-17A, IL-17A/F, IL-17 receptor, IL-12/23(p40), IL-23p19, Cytotoxic T-lymphocyte Antigen-4 (CTLA-4), Janus kinase, and phosphodiesterase-4 have been developed an

Current status and perspectives in ceramide-targeting molecular medicine.

Ceramide is not only structurally but also functionally a key molecule in diverse kinds of sphingolipids. In the past decade, ceramide has been shown to be of crucial significance in several cell functions including apoptosis, cell growth, senescence, and cell cycle control. Among them, the role of ceramide in apoptosis induction has extensively been studied, and ceramide-targeting molecular medicine for apoptosis-based diseases such as malignant tumors, atherosclerosis and neurodegenerative disorders appears to come out to the clinical field. We here describe the recent advances in research o

Molecular medicine of TFF-peptides: from gut to brain.

TFF-peptides (i.e. TFF1, TFF2, TFF3; formerly P-domain peptides, trefoil factors) have been established as secretory products typical of the gastrointestinal tract. Their synthesis has recently been recognized in a number of mucin-producing epithelial cells, for example, of the respiratory tract, the salivary glands, the uterus and of the conjunctiva. They have a pivotal role in maintaining the surface integrity of these delicate epithelia as constituents of mucus gels as well as by their anti-apoptotic properties and their motogenic activity modulating cell migratory processes. The latter is

Molecular dynamics simulation in traditional Chinese medicine research: from molecular mechanisms to multiscale validation.

Molecular dynamics (MD) simulation is a computational technique based on Newtonian mechanics and statistical physics that tracks molecular motion and ligand-target recognition at atomic resolution. In recent years, with advances in computational resources and continuous optimization of simulation algorithms, the application of MD simulations in traditional Chinese medicine (TCM) research and development has expanded steadily. Because TCM involves chemically diverse constituents, multiple targets, metabolic transformation, and pathway-level regulation, its mechanisms are difficult to interpret

Molecular and Biomarker Signatures of Trauma: The State of Precision Medicine.

INTRODUCTION: Resuscitation and prognostication of patients after traumatic injury rely primarily on broad clinical and laboratory measures that may overlook subclinical microcirculatory and metabolic derangements. Emerging data show that individual responses to injury are heterogeneous, and characterizing such responses using biomarkers is a growing priority. Recent literature has increasingly explored the use of high-throughput "-omics" methodologies to define biologic patterns after injury. This review summarizes existing literature on blood-based biomarkers after traumatic injury to descri

Molecular vehicles for therapeutic delivery of lactate in exercise and medicine.

Once thought of as a dead-end metabolite, the product of anaerobic, oxygen-limited metabolism, we now know that lactate is produced under fully oxygenated conditions, such as after consuming dietary carbohydrate and during physical exercise. Via shuttling mechanisms lactate is a preferred fuel energy source for red skeletal muscle, heart and brain. Consequently, lactate is being recognized as a major means of delivering non-insulin dependent carbohydrate-derived carbon around the body. Such recognition has led to consideration of developing enteral and parenteral means of lactate delivery. Ent

Carbon monoxide: from the origin of life to molecular medicine.

Carbon monoxide, long considered only as a toxic gas, has recently been shown to mediate potent anti-inflammatory and other salutary effects in rodents when it is used at low doses. Carbon monoxide is one of the products of the degradation of heme by heme oxygenase 1. Until recently, these beneficial effects of carbon monoxide were shown only when it was given before a stress stimulus. Hagazi and colleagues have recently shown that this substance is effective even when it is given after a disease process has started. The effects of low doses of carbon monoxide are complemented by the productio

Medicinal chemistry and molecular pharmacology of GABA(C) receptors.

GABA(C) receptors belong to the nicotinicoid superfamily of ionotropic receptors that include nicotinic acetylcholine receptors, bicuculline-sensitive GABA(A) receptors, strychnine-sensitive glycine receptors and 5HT3 serotonin receptors. The GABA(C) receptor concept arose from medicinal chemical studies of a conformationally restricted analog of GABA. Receptors matching the predicted properties of GABA(C) receptors were cloned from the retina. Post cloning studies revealed the unique physiology and pharmacology of these relatively simple homomeric receptors. Three subtypes of GABA(C) receptor

Precision Medicine in Astronauts: Principles and Targets in Molecular Assessment.

The field of space medicine has served the population of professional astronauts well over the many decades of human space travel, which is reflected in the considerable success of astronauts executing missions and returning to Earth in good health (rare comorbidities notwithstanding). At the same time, there are considerable gaps in our biomedical knowledge that will only scale as less fit individuals embark upon spaceflight, and we enter more extreme environments such as the Moon and Mars for longer duration missions. One means to partially fill this knowledge gap is to expand the practice o

Molecular drug targets for scabies: a medicinal chemistry perspective.

Sarcoptes scabiei is a causative organism for scabies that affects an estimated global population of 300 million and remains a disease of significant concern. Recently, a number of potential drug targets were identified for scabies, including hydrolytic enzymes, inactivated paralogues of hydrolytic enzymes, inhibitors of host proteolytic enzymes and other proteins of interest. These discoveries remain confined to academic laboratories and institutions, failing to attract interest from researchers in commercial drug development. Here, we summarize the latest developments in the scabies mite bio

The Molecular Mechanisms of Thalidomide Teratogenicity and Implications for Modern Medicine.

Thalidomide is a teratogen that affects many organs but primarily induces limb truncations like phocomelia. Rodents are thalidomide resistant. In the 1950s, this has led to misinterpretations of animal tests and to the fatal assumption that the drug was safe for pregnant women to use against morning sickness. The result was one of the biggest scandals in medical history: 10.000 and more infants with birth defects in Europe. Nonetheless, thalidomide still has its place in modern medicine as it has strong therapeutic potential: it has been approved by the FDA for multiple myeloma and erythema no

Molecular Imaging and Precision Medicine in Dementia and Movement Disorders.

Precision medicine (PM) has been defined as "prevention and treatment strategies that take individual variability into account." Molecular imaging (MI) is an ideally suited tool for PM approaches to neurodegenerative dementia and movement disorders (MD). Here we review PM approaches and discuss how they may be applied to other associated neurodegenerative dementia and MD. With ongoing major therapeutic research initiatives that include the use of molecular imaging, we look forward to established interventions targeted to specific molecular pathophysiology and expect the potential benefit of MI
