Bubbles in the bleb--troubles in the bleb? Molteno implant and intraocular tamponade with silicone oil in an aphakic patient.

A 14-year-old aphakic girl who had had previous bilateral glaucoma surgery with a Molteno implant underwent pars plana vitrectomy and silicone-oil tamponade for proliferative-vitreoretinopathy retinal detachment in both eyes. The filtering bleb of the left eye was functional for 5 months before becoming available for histologic examination. We found numerous foreign-body granulomas coating the inner surface of the bleb, as well as intracellular and extracellular deposits of emulsified silicone oil in the wall of the bleb. In the fellow eye, the filtering bleb remained functional despite repeat

[Molteno implant and refractory glaucoma. Evaluation of postoperative IOP control and complications with a modified surgical procedure].

BACKGROUND: Management of refractory glaucoma is difficult because classic medical and surgical treatments are ineffective in controlling highly elevated intraocular pressure (IOP). Drainage implants are an alternative to cyclodestructive procedures in refractory glaucoma. The double-plate Molteno implant, a tube linked to two polypropylene plates, allows aqueous humor drainage from the anterior chamber towards the posterior subconjunctival spaces. The main postoperative complication is severe hypotony, potentially responsible for a decrease in visual acuity in cases of advanced glaucoma. Surg

Which is better? One or two? A randomized clinical trial of single-plate versus double-plate Molteno implantation for glaucomas in aphakia and pseudophakia.

PURPOSE: Previous studies have suggested that primary double-plate Molteno implantation may be beneficial. Therefore, the authors performed a randomized clinical trial to evaluate the relative effectiveness and safety of single- versus double-plate Molteno implantation. METHODS: From March 1988 to February 1990, 132 patients who underwent Molteno implantation for medically uncontrollable non-neovascular glaucomas in aphakia or pseudophakia were randomly assigned to receive either single- or double-plate implants. RESULTS: The 1- and 2-year life-table success rates (success [survival] defined

Corneal melting after avulsion of a Molteno shunt plate.

PURPOSE: We report a case of Molteno shunt avulsion after blunt trauma resulting in corneal melting. The implant's connecting tube had been placed above the superior rectus muscle. METHODS: Case report. RESULTS: After blunt trauma, the secondary plate of a double-plated Molteno implant became completely dislodged anteriorly onto the patient's cornea. The corneal tissue entrapped beneath the avulsed plate was found to have extensive melting requiring emergency explantation of the Molteno implant and corneal grafting. CONCLUSIONS: Corneal melting after Molteno shunt avulsion is a novel findin

Management of the Molteno silicone tube in corneal transplant surgery.

A method to prevent tube-corneal touch in patients with Molteno implants who require corneal transplants is presented. A double-armed 10-0 Prolene suture is passed from limbus to limbus so that within the anterior chamber the suture acts as a splint for the silicone tube of the Molteno implant, keeping it away from the cornea. This suture has been used in six patients, all aphakic or pseudophakic, requiring corneal transplants for bullous keratopathy. In all six patients, the silicone tube was successfully splinted away from the endothelium of the corneal transplant.

Use of neodymium: YAG laser to open an occluded molteno tube.

A patient with refractory glaucoma secondary to trauma and repeat penetrating keratoplasty had Molteno implant surgery to control elevated intraocular pressure. Six weeks after surgery intraocular pressure rose abruptly, and a fibrinous membrane was seen covering and occluding the tip of the Molteno tube. The Q-switched neodymium (Nd):YAG laser was used successfully to open the membrane, with subsequent renewal of aqueous flow and control of intraocular pressure. Bevel-up cutting of the tube tip facilitates Nd:YAG laser opening of such an obstruction. We describe this use of the Nd:YAG laser a

Glaucoma tube shunts: 2026 Update.

PURPOSE OF REVIEW: To provide an update on new glaucoma drainage devices and implants. RECENT FINDINGS: Glaucoma drainage implant devices are currently indicated for medically or surgically uncontrolled glaucoma, especially in eyes at high risk for trabeculectomy failure. The devices used can differ, but many offer similar long-term outcomes. Due to the differences in devices, some of the side effect profiles may differ. SUMMARY: Long-term outcomes show that nonvalved devices (Ahmed ClearPath, Baerveldt, Molteno, Paul Glaucoma Implant) achieve greater intraocular pressure reduction and lower

Surgical technique: complex glaucoma case requiring Molteno drainage tube extension.

A 42-year-old man has been under long-term follow-up since he was a child for congenital glaucoma and buphthalmos in both eyes. His left eye best corrected visual acuity (BCVA) was counting fingers, due to end-stage glaucoma. He was on maximal medical therapy with an intraocular pressure (IOP) maintained at mid to low twenties. His right eye, the only seeing eye, had a BCVA of 6/9. This eye had undergone multiple glaucoma laser and surgical procedures, including an initial first Molteno drainage device inserted superonasally that failed in April 2003 due to fibrotic membrane over the tube open

Aqueous shunt implantation in glaucoma.

Aqueous shunts or glaucoma drainage devices are increasingly utilized in the management of refractory glaucoma. The general design of the most commonly-used shunts is based on the principles of the Molteno implant: ie. a permanent sclerostomy (tube), a predetermined bleb area (plate) and diversion of aqueous humour to the equatorial region and away from the limbal subconjunctival space. These three factors make aqueous shunts more resistant to scarring as compared to trabeculectomy. The two most commonly used shunts are the Ahmed Glaucoma Valve, which contains a flow-restrictor, and the non-va

Performance of a new, low-volume, high-surface area aqueous shunt in normal rabbit eyes.

PURPOSE: This experimental study was conducted to report perfusion characteristics of small diameter, cylindrical aqueous shunts in normal rabbit eyes and to test the hypothesis that decreasing bleb diameter would decrease capsular fibrosis, as evidenced by a thinner capsule forming around the implant. These two properties increase hydraulic conductivity of the fibrous membrane forming around the device, resulting in a more effective filtering shunt. METHODS: Cylindrical latex tubes with the distal portion of the sidewall removed were implanted under the conjunctiva. The proximal, intact end

The choice of surgical procedure in congenital, infantile, and juvenile glaucoma.

Trabeculotomy is successful in patients with an angle anomaly involving only the trabecular meshwork, but in those involving the trabecular meshwork with iris and/or cornea trabeculotomy combined with trabeculectomy is recommended. In cases of failed trabeculotomy or trabeculectomy, a Molteno implant can be tried, although it does not work well in patients with congenital glaucoma. If all else fails, a cyclodestructive procedure is indicated and contact Nd:YAG laser cyclophotocoagulation is recommended.
