Ambulatory blood pressure monitoring in atrial fibrillation.

Ambulatory blood pressure monitoring (ABPM) devices are increasingly used in the assessment of hypertension, but patients with atrial fibrillation are usually excluded because device accuracy in this patient group is unproved. The present study investigates the clinical use of the Spacelabs 90207 oscillometric ABPM device in outpatients with chronic atrial fibrillation and suggests that such devices can be used in clinical practice to assess blood pressure in stable outpatients with chronic atrial fibrillation.

Ambulatory electrocardiographic monitoring in healthy newborn infants.

Twenty-four hour ambulatory electrocardiographic monitoring is a useful method to document dysrhythmias and to assess treatment response. Various studies have been done in the pediatric age-group to determine normal heart rate values. In this study we determined the heart rate and rhythm patterns of 25 healthy newborn infants whose ages ranged between 3-10 days (mean 6.5 days). There were 15 males and 10 females. The maximum heart rate in these infants was 175-231 beats/min (207 +/- 14), minimum heart rate 60-121 beats/min (93 +/- 16) and the average heart rate was 130-161 beats/min (143 +/- 9

Ambulatory blood pressure monitoring in secondary hypertension.

The term 'secondary hypertension' includes a variety of aetiological processes with no obvious common feature apart, perhaps, from the loss of normal blood pressure regulatory factors. It has been suggested that all forms of secondary hypertension, including renal/renovascular, accelerated phase, glucocorticoid and mineralocorticoid excess, phaeochromocytoma and toxaemia in pregnancy are associated with the loss of the normal nocturnal fall in blood pressure. The evidence for this view appears strongest for glucocorticoid-induced hypertension, but for most other forms the available data are co

Ambulatory blood pressure monitoring in clinical trials.

Monitoring ambulatory blood pressure, instead of taking pressure readings in hospital, avoids the so-called white-coat effect and allows more readings to be obtained over a longer period of time. It improves the accuracy of the blood pressure estimate and increases the statistical power of therapeutic trials for hypertension. Subjects with white-coat or office hypertension can be detected by ambulatory blood pressure monitoring and excluded from clinical trials. In 23 studies, including a total of 3304 normotensive subjects, the 24-h ambulatory blood pressure averaged 118/72 mmHg; the daytime

Ambulatory blood pressure monitoring in clinical practice.

Ambulatory blood pressure monitoring provides a more reliable estimate of blood pressure and its diurnal variability than casual blood pressure measurement. However, the data supporting the link between cardiovascular risk and blood pressure are based almost entirely on casual, or office-based, blood pressure readings, and this form of measurement continues to be the primary factor in clinical decision making. In recent years, compelling evidence from research centers around the world has shown that ambulatory blood pressure measurements correlate more closely with the target organ complicatio

Ambulatory blood pressure monitoring in hypertensive adolescents.

The purpose of this study was to determine the ability of ambulatory blood pressure monitoring to identify youths with chronic blood pressure elevation. Nineteen adolescent boys were studied, ten had 5-year average systolic or diastolic pressures above the 95th percentile, nine had normal pressure. A Del Mar Avionics Pressurometer III system recorded an average of 121 readings on each subject. The coefficients of variation for pressure were similar for hypertensive and normotensive individuals. During classes, eight of the ten hypertensive youths had elevated pressures in over half of the meas

Clinical use of ambulatory blood pressure monitoring.

This paper describes the alerting reaction and blood pressure rise induced in the patient when blood pressure measurement is taken by the doctor, emphasizing that this may be responsible for important errors in estimating the real untreated or treated blood pressure in hypertension. It then examines the advantages offered by ambulatory blood pressure monitoring. It is shown that when performed intra-arterially this approach may be useful in the diagnosis of pheochromocytoma. However, its wider use as a means to better discriminate between normotensive and borderline or mild hypertensive subjec

Ambulatory blood pressure monitoring in primary care.

The accurate assessment and appropriate management of blood pressure (BP) is critical for primary care physicians. Traditional office-based BP measurement has limitations that can be addressed, in part, through the use of ambulatory BP monitoring (ABPM). Because BP readings are provided at specific time intervals throughout a 24-hour period, ABPM represents a better picture of the normal fluctuations in BP levels associated with daily activities, including sleep. Blood pressure values obtained by 24-hour ABPM are a better predictor of cardiovascular risk than office-based BP measurements, and

Ambulatory long-term pH monitoring in pigs.

BACKGROUND: pH monitoring has been established as the "gold standard" in the diagnosis of gastroesophageal reflux. Evaluation of experimental antireflux therapy should therefore also include this technique, but a suitable technique in an experimental model did not exist so far. The aim of our study was to establish a reliable method for the evaluation of an experimental reflux model in pigs. METHODS: A total of 33 German Landrace pigs with an average body weight of 56 (50.2-67.2) kg were included. pH monitoring was performed before and after open cardiomyotomy in each animal. All manipulation

Ambulatory blood pressure monitoring in clinical practice.

Ambulatory blood pressure (ABP) monitoring has become increasingly more available in routine clinical practice in Canada. The ABP is more reliable and more reproducible than office readings, and is a better predictor of target organ damage. Normal values for ABP have been established using both cross-sectional and longitudinal outcome data. Abnormal mean 24 h and awake ABP values should exceed 135/85 mmHg and 140/90 mmHg, respectively. ABP recordings are useful in making a diagnosis of hypertension by identifying people with high office but normal ABP values. ABP monitoring can also be perform

Clinical value of ambulatory blood pressure monitoring.

UNLABELLED: Ambulatory blood pressure monitoring (ABPM) has now become an established clinical tool. It is appropriate to take stock and assess the situation of this technique. UPDATE ON EQUIPMENT: Important improvements in equipment have occurred, with reductions in weight, in awkwardness and in noisiness of the machines, better acceptability and tolerance by the patients, and better reliability. Validation programmes have been proposed and should be referred to. Limitations of the technique persist with intermittent recording in current practice. The reproducibility is limited in the short-t

Orthostatic hypotension and ambulatory blood pressure monitoring.

Orthostatic hypotension has a number of causes, many of which are easily reversible. However, characteristics such as standing hypotension in conjunction with supine hypertension are not always easily detected in the clinic or office setting. Ambulatory blood pressure monitoring may be a valuable technique for diagnosis and for the assessment of therapeutic effects in patients with orthostatic hypotension.

Clinical uses of ambulatory blood pressure monitoring.

In the past, the diagnosis and management of hypertension has been based on office blood pressure. However, office blood pressure is not always a true reflection of a patient's blood pressure profile. Since ambulatory blood pressure monitoring permits a large number of readings to be taken in the patient's usual environment, it may provide a more representative blood pressure profile. Indeed, ambulatory blood pressure has been better correlated than office blood pressure with the target-organ complications of hypertension. Office or white-coat hypertension (elevated blood pressure only when me

Ambulatory Cardiac Monitoring in Infants with Supraventricular Tachycardia.

Supraventricular tachycardia (SVT) is a frequent cause of tachyarrhythmia in infants < 1 year of age and ambulatory cardiac monitoring is an important tool for diagnosis and follow-up of these patients. We retrospectively reviewed 594 infants (mean age 4.05 months, SD 3.55; 54% M) who underwent ambulatory cardiac monitoring (69% 24 h Holter, 31% extended monitor) through the Pocket ECG system (MediLynx) between January 2016 and July 2020. 170 patients who had the ICD-10 code I47.1 for SVT used at enrollment were analyzed separately. 49 (8.3%) patients had sustained SVT or non-sustained SVT (nS

Ambulatory blood pressure monitoring in children undergoing polysomnography.

Ambulatory blood pressure monitoring (ABPM) is recommended for the diagnosis of hypertension in children at high risk, such as children with obesity or obstructive sleep apnea (OSA). Nocturnal hypertension is highly predictive of cardiovascular outcomes. ABPM allows for early detection of nocturnal hypertension in children. Although OSA is the most common sleep disorder associated with hypertension, studies have also shown an increase in cardiovascular risk in adult patients with other sleep disorders; therefore, there is an imperative need to provide early diagnosis in children at high risk.

Ambulatory blood pressure monitoring in solid organ transplantation.

Solid organ transplant recipients are at an increased risk for hypertension and cardiovascular disease. To assist in their management, 24-h ambulatory blood pressure monitoring (ABPM) has become increasingly used in both clinical research settings and practice. ABPM has been used to better define post-transplant hypertension incidence and prevalence in different solid organ transplantation populations. ABPM provides additional information on cardiovascular risk beyond that obtained by clinic-based readings, based on its ability to assess 24-h blood pressure (BP) load, detect nocturnal non-dipp

[Ambulatory monitoring of blood pressure--focus on childhood].

Conventional office measurements of blood pressure (BP) are limited in their ability to assess long term hemodynamic load on cardiovascular system. The ambulatory blood pressure monitoring (ABPM) offers the advantage of providing multiple blood pressure measurements from a subject's normal environment during his normal activities, thereby revealing important patterns of blood pressure during the whole 24-hour period. Results of ABPM have much better correlation with end organ damage than casual BP and provide more sensitive data of ultimate prognosis. There is much better long-term reproducibi

Some people's blood pressure is elevated simply by being in a clinical environment. Twenty-four hour monitoring gives a more accurate average blood pressure reading. The initial costs of ABPM are recouped in less than two years.
