Intraoperative electroencephalography (EEG) monitoring under pediatric anesthesia has begun to attract increasing interest, driven by the availability of pediatric-specific EEG monitors and the realization that traditional dosing methods based on patient movement or changes in hemodynamic response often lead to imprecise dosing, especially in younger infants who may experience adverse events (e.g., hypotension) due to excess anesthesia. EEG directly measures the effects of anesthetics on the brain, which is the target end-organ responsible for inducing loss of consciousness. Over the past ten

OBJECTIVE: To evaluate the ability of intraoperative neurophysiologic monitoring (IONM) during aortic arch reconstruction with hypothermic circulatory arrest (HCA) to predict early (<48 hours) adverse neurologic events (ANE; stroke or transient ischemic attack) and operative mortality.
METHODS: This was an observational study of aortic arch surgeries requiring HCA from 2010 to 2018. Patients were monitored with electroencephalogram (EEG) and somatosensory evoked potentials (SSEP). Baseline characteristics and postoperative outcomes were compared according to presence or absence of IONM change

Surgical manipulation during skull base surgeries places various cranial nerves (CN) at risk, including the nerves innervating the extraocular muscles. It could be very challenging for the surgeon to identify these cranial nerves due to the distortion of the normal anatomy by the tumors. Despite the recent advancement in technology, surgeries involving the third, fourth, fifth, and sixth cranial nerves still carry a risk of temporary or permanent paralysis of the muscles supplied by these cranial nerves. Intraoperative Neurophysiological Monitoring (IONM) with spontaneous and triggered electro

This case illustrates the benefits of utilizing intraoperative neurophysiological monitoring (IONM) for preventing injury to sensory/motor pathways of the spinal cord during a cordotomy procedure to relieve pain. Cordotomy has been used effectively in the treatment of visceral pain but comes with a high risk of damaging motor and sensory pathways due to close proximity of lesion. The subject is a 47-year-old female with a pancoast tumor of the left lung, left brachialplexopathy, and severe neuropathic pain syndrome, refractory to medical therapy. A palliative cordotomy procedure was elected fo

BACKGROUND: Intraoperative neural monitoring (IONM) has increasingly garnered the attention of the surgeons performing thyroid and parathyroid surgery around the world. Current studies suggest a majority of general and head and neck surgeons utilize neural monitoring in their thyroid surgical case load in both the US and Germany.
PURPOSE: We aim to present an up-to-date review of the application of IONM specifically focusing on its utility in thyroid cancer surgery. Neural monitoring is discussed particularly as it relates to neural prognosis, the issues of staged thyroid surgery for thyroid

A 54-year-old man presented with an intracranial schwannoma of the hypoglossal nerve between the medulla and the left hypoglossal canal. The condylar fossa approach was used with intra-operative electromyography (EMG) monitoring of the lower cranial nerves. The tumor was then removed carefully without decreasing the tongue EMG responses. EMG monitoring enabled us to remove the tumor while maintaining the function of the hypoglossal nerve. Tongue EMG was easily recorded by stimulating the hypoglossal nerve fibers, which was useful in identifying the hypoglossal nerve and evaluating its function

Selective dorsal rootlet rhizotomy (SDR) is a neurosurgical procedure designed to reduce spasticity in the legs, although preserving motor and sensory function, of appropriately selected children with spastic quadraparesis. This is accomplished by neurophysiologicially guided (e.g., selective) severing of specific dorsal rootlets in the cauda equina. This decreases facilitatory input to spinal anterior motor neurons, thereby reducing spasticity in the legs. This first portion of this article discusses the neurophysiologic intraoperative monitoring techniques during SDR, with the understanding

The surgical and endovascular treatment of cerebrovascular disorders (CVDs) in children, such as cerebral arteriovenous malformations, cavernous malformations, and moyamoya disease, have become commonplace and routine in many centers. As in the adult population, these procedures carry the risk of intraoperative cerebral ischemia. Therefore, similar strategies used to reduce the risk of cerebral ischemia in adults should be used in children. Unfortunately, there are no published studies on the intraoperative use of available techniques to identify, prevent, or potentially reverse cerebral ische

Intraoperative auditory monitoring in acoustic neuroma surgery is used to improve residual hearing. Three techniques are available: monitoring of brainstem auditory evoked potentials, of electrocochleography or of direct eighth nerve compound action potential. The three techniques with their advantages and disadvantages are discussed. The current trend is to monitor brainstem auditory evoked potentials using digital filtering or to monitor the eighth nerve compound action potential.

Intraoperative recurrent laryngeal nerve identification is sometimes difficult in reoperative cervical dissection or operation for inflammatory thyroid disorders. Three modalities have been described to intraoperatively assess nerve function: vocal cord visualization with fiberoptic bronchoscopy or direct laryngoscopy, electromyelographic surveillance of arytenoid muscle function, and cord function assessment with an electromyelogram-electrode endotracheal tube. Our study focused on patients requiring cervical dissection for thyroid or parathyroid disease in which intraoperative recurrent lary

Intraoperative auditory monitoring is a useful adjunct that is currently evolving. Near-field monitoring techniques such as electrocochleography and direct eighth nerve compound action potential are being used more frequently. The use of these two techniques is compared in 26 patients undergoing hearing preservation acoustic neuroma resection. Overall, 9 (35%) of the 26 patients had their hearing preserved. Three (23%) of the 13 with electrocochleography monitoring and 6 (46%) of the 13 with direct compound action potential monitoring had hearing preserved after surgery. Although there was a s

A threefold intraoperative monitoring of facial nerve, auditory nerve and vestibular nerve function was performed in 14 cases of retrosigmoidal neurectomy. The facial nerve was monitoring with a pressure transducer placed against the cheek (Opalarm system). The auditory nerve was monitored with acoustically (click) evoked early potentials and the vestibular nerve was monitored with electrically evoked vestibular potentials obtained by direct stimulation (biphasic current pulses of 0.75-mA p-p, 100 us, 20/s) of the exposed vestibular nerve in the cerebellopontine angle before, during and after

Intraoperative facial nerve monitoring provides a potentially useful adjunct to recent surgical advances in neurotology and neurosurgery. These measures further aid the surgeon in preserving facial nerve function by enhancing visual identification with electrical monitoring of mechanically evoked facial muscle activation. Facial nerve monitoring in neurotologic surgery may achieve the following goals: (1) early recognition of surgical trauma to the facial nerve, with immediate feedback made available to the surgeon through monitoring of mechanical activation; (2) assistance in distinguishing t

We believe that our initial experience establishes the fact that ABRs can be routinely and reliably performed in an operating room environment. There was no added risk to the patient, and operative delays were minimal. We did note transient fluctuation in latency values up to 1.5 msec. These changes would revert to baseline levels within five to ten minutes. Changes noted during drilling were probably related to the random noise produced. Case 3 was worrisome in that hearing was lost after maintenance of the ABRs during the surgical procedure. Evidently the vestibule was damaged, and a labyrin

Craniovertebral junction (CVJ) surgery encompasses a wide spectrum of neurosurgical procedures ranging from transoral approaches for CVJ bone anomalies to surgery for intramedullary tumours. Intraoperative neurophysiological monitoring (IONM) has been increasingly used in recent years because of its ability to prevent neurological complications during surgery. In CVJ surgery the risk of neurological injuries is related first to the positioning of the patient and then to the surgical procedure. Application of IONM during the positioning of the patient permits fast recognition of impending cause

Multimodal intraoperative neurophysiologic monitoring is a reliable tool for detecting intraoperative spine injury and is recommended during surgery for degenerative cervical myopathy (DCM). Somatosensory evoked potential (SEP) can be used to monitor spine and peripheral nerve injury during positioning in surgery for DCM. Compensation technique for transcranial evoked muscle action potentials (tcMEPs) should be adopted in intraoperative monitoring during surgery for DCM. Free-running electromyography is a useful real-time monitoring add-on modality in addition to SEP and tcMEP.

BACKGROUND: In spinal deformity surgery, iatrogenic spinal cord injury is the most feared complication. Intraoperative monitoring (IOM) of the spinal cord assesses its functional integrity and allows significant reduction of the rate of spinal cord injury.
HYPOTHESIS: In case of severe IOM alert, lesional level diagnosis constitutes supplementary and useful information.
MATERIAL AND METHODS: This study was retrospective and monocentric. In our institution, 1062 pediatric spinal deformity surgeries have been monitored since 2004. We review the records of the six patients who presented a sever
