Abnormal monitoring of inner speech: a physiological basis for auditory hallucinations.

Auditory verbal hallucinations ("voices") are thought to arise from a disorder of inner speech (thinking in words). We examined the neural correlates of tasks which involve inner speech in subjects with schizophrenia who hear voices (hallucinators), subjects with schizophrenia who do not (nonhallucinators), and normal controls. There were no differences between hallucinators and controls in regional cerebral blood flow during thinking in sentences. However, when imagining sentences being spoken in another person's voice--which entails both the generation and monitoring of inner speech--halluci

Growth and Financial Outcomes of a Remote Physiologic Monitoring Service for Hypertension in the Primary Care Setting.

BACKGROUND: Remote physiologic monitoring (RPM) presents a promising solution for hypertension management through efficient blood pressure monitoring outside of the traditional clinic setting utilizing monitors that transmit readings automatically to the healthcare team. Barriers to adoption of RPM include addressing the challenge of managing continuous streams of data, patient capability, and financial sustainability. The impacts of different RPM models on enrollment and financial outcomes are unknown. OBJECTIVE: The primary objective of this study was to describe the operationalization and

Physiologic perfusion monitoring methods during endovascular revascularization for atherosclerotic peripheral arterial disease: protocol for a systematic review.

BACKGROUND: Endovascular therapy is a fundamental treatment for peripheral arterial disease. However, the success rate of endovascular therapy remains poor, as a third of patients with critical limb ischemia ultimately require a major amputation for gangrene despite endovascular treatment. This failure rate has prompted investigation into methods of determining physiologic procedural success before and after treatment, before clinically apparent outcomes occur such as gangrene. The aim of this systematic review is to evaluate if in patients undergoing endovascular surgery for lower extremity a

Novel technique for retroperitoneal implantation of telemetry transmitters for physiologic monitoring in Göttingen minipigs (Sus scrofa domesticus).

Telemetric monitoring of physiologic parameters in animal models is a critical component of chemical and biologic agent studies. The long-term collection of neurobehavioral and other physiologic data can require larger telemetry devices. Furthermore, such devices must be implanted in a location that is safe, well-tolerated, and functional. Göttingen minipigs (Sus scrofa domesticus) present an ideal large animal model for chemical agent studies due to their relatively small size, characterized health status, and ease of training and handling. We report an effective approach to implanting a nove

Use of sensory-evoked potentials recorded from the human occiput for intraoperative physiologic monitoring of the spinal cord.

STUDY DESIGN: This is a report of a prospective case series. OBJECTIVE: The purpose of this study was to determine whether evoked potential activity recorded from occipital scalp electrodes in humans is similar to that recorded in animals, and to evaluate the independence of this activity from the classical somatosensory-evoked potential. SUMMARY OF BACKGROUND DATA: Intraoperative somatosensory-evoked potentials can be of limited usefulness in predicting spinal cord injury because they are transmitted primarily through the dorsal columns, and therefore do not reflect integrity of important v

STREAM: A data-driven framework for physiological state monitoring in ICU patients.

Intensive care unit (ICU) monitoring systems face a critical gap: translating continuous physiological data into interpretable patterns that support clinical assessment. Existing approaches rely on static thresholds or severity scores that fail to capture dynamic disease progression patterns and provide limited insight into why patients change over time. We developed STREAM (State Trajectory Representation and Evolution-Aware Monitoring), which models each patient as a point in a multidimensional physiological space and applies geometric analysis of routine ICU data to track physiological inst

Radar HRV Monitoring With Physiological Prior Inspired Deep Neural Networks.

Radar sensing has emerged as a promising solution for the contactless monitoring of Heart Rate Variability (HRV), a crucial indicator of the cardiovascular and autonomic nervous systems. However, due to signal noise and interference that easily obscure heartbeat details, along with variations in heartbeat across different physiological conditions, existing methods remain restricted to laboratory settings with healthy subjects and fail in real-world scenarios involving more complex physiological conditions. In this study, we propose a physiological prior-inspired deep learning framework for rob

Remote Monitoring of Physiology in People Living With Dementia: An Observational Cohort Study.

BACKGROUND: Internet of Things (IoT) technology enables physiological measurements to be recorded at home from people living with dementia and monitored remotely. However, measurements from people with dementia in this context have not been previously studied. We report on the distribution of physiological measurements from 82 people with dementia over approximately 2 years. OBJECTIVE: Our objective was to characterize the physiology of people with dementia when measured in the context of their own homes. We also wanted to explore the possible use of an alerts-based system for detecting healt

[Investigation on new paradigm of clinical physiological monitoring by using wearable devices].

As a low-load physiological monitoring technology, wearable devices can provide new methods for monitoring, evaluating and managing chronic diseases, which is a direction for the future development of monitoring technology. However, as a new type of monitoring technology, its clinical application mode and value are still unclear and need to be further explored. In this study, a central monitoring system based on wearable devices was built in the general ward (non-ICU ward) of PLA General Hospital, the value points of clinical application of wearable physiological monitoring technology were ana

Use of near-infrared spectroscopy as a physiologic monitor for intra-abdominal hypertension.

BACKGROUND: Intra-abdominal hypertension (IAH) is increasingly recognized as an important parameter in critically ill (ICU) patients. IAH affects perfusion to all abdominal components including the abdominal wall (AW). Near infrared spectroscopy (NIRS) measures changes in three chromophores including oxygenated hemoglobin (HbO2), deoxygenated hemoglobin (Hb), and cytochrome aa3 (Cyt), providing information concerning dysoxia. We sought to examine whether NIRS measurement of the AW was safe, and correlated with intra-abdominal pressure (IAP) in ICU patients. METHODS: A NIU-Pro001 system record

Physiology-guided mechanical ventilation: Monitoring, proportional assist, and bounded automation.

Mechanical ventilation has evolved into a complex intervention that influences lung injuries, respiratory muscle function, and hemodynamic stability. Although lung-protective strategies improve outcomes in acute respiratory distress syndrome, bedside management remains limited by incomplete monitoring of key physiologic variables, including lung stress, inspiratory effort and regional ventilation. This constrains decision such as positive end-expiratory pressure titration and ventilatory assist targeting. Emerging technologies aim to address these gaps by improving physiological assessment and

Cardiovascular Physiology During Mechanical Circulatory Support: Implications for Management and Monitoring.

Background/Objectives: Mechanical circulatory support (MCS) is increasingly utilized for the management of acute decompensated heart failure (HF) and cardiogenic shock (CS). The primary goals of MCS are to restore systemic perfusion, reduce cardiac workload, and support end-organ function. A thorough understanding of cardiovascular physiology in patients supported by MCS is essential for clinical decision-making. This review summarizes current evidence on the physiological effects of various MCS devices, key monitoring techniques, patient management, and explores the emerging role of artificia

Evaluation of Radar-Based Cardiorespiratory Monitoring Under Different Physiological Conditions.

The use of Frequency-Modulated Continuous Wave (FMCW) radar for contactless monitoring of cardiac and respiratory activity has demonstrated significant potential for clinical applications. However, its performance in nonphysiological frequency ranges remains underexplored. This study evaluated the accuracy of a FMCW radar system operating at 122.5 GHz for monitoring heart rate (HR) and respiratory rate (RR) across physiological and nonphysiological conditions. The estimation of heart and respiration rates, as well as the separation of signals, was conducted using Maximum Likelihood Estimation

Factors Influencing Use of Continuous Physiologic Monitors for Hospitalized Pediatric Patients.

OBJECTIVES: Continuous physiologic monitors (CPMs) generate frequent alarms and are used for up to 50% of children who are hospitalized outside of the ICU. Our objective was to assess factors that influence the decision to use CPMs. METHODS: In this qualitative study, we used group-level assessment, a structured method designed to engage diverse stakeholder groups. We recruited clinicians and other staff who work on a 48-bed hospital medicine unit at a freestanding children's hospital. We developed a list of open-ended prompts used to address CPM use on inpatient units. Demographic data were
