This article weighs three important moral and ethical considerations for leaders in healthcare: their obligations to society, their privilege as leaders and how value goes beyond a simple cost analysis. Leaders highly motivated by the bottom line have avoided the long overdue action on moral and ethical considerations critical to a more just and fairer society. Leaders are now being tasked to develop strategies for health equity, anti-oppression, anti-racism, social justice, diversity, equity and inclusiveness, community engagement, the social determinants of health and environmental accountab

Do we have a moral obligation to genetically cure embryos rather than selecting between them? Such an obligation would be an ethical argument for human germline gene editing (hGGE) to avoid the inheritance of genetic conditions instead of using pre-implantation genetic diagnosis (PGD). In this article, the intuition that we do have such a moral obligation is critically evaluated. The article first develops a theoretical framework for discussing the ethical questions of hGGE. This framework is based on an exploration of the phenomenology of the germline, from both biological and philosophical p

Most opposition to induced abortion turns on the belief that human fetuses are persons from conception. On this view, the moral status of the fetus alone requires those in a position to provide aid-gestational mothers-to make tremendous sacrifices to benefit the fetus. Recently, critics have argued that this pro-life position requires more than opposition to induced abortion. Pro-life theorists are relatively silent on the issues of spontaneous abortion, surplus in vitro fertilisation human embryos, and the suffering and death of born persons due to lack of access to food, shelter and medical

Moral distress has been documented in clinical nursing, but far less attention has been given to morally troubling experiences within academic healthcare settings. This study examines how conflicts between professional values and organizational culture may give rise to moral ambiguity, evolve into moral distress, and contribute to moral injury over time. Guided by a critical, transformative theoretical framework, this longitudinal autoethnography explores these experiences across a 45-year nursing career in academic settings. Data were generated through sustained self-observation and reflexive

The introduction of conscience clauses after the 1973 US Supreme Court decision in Roe v. Wade allowed physicians and nurses to opt out of medical procedures, particularly abortions, to which they were morally opposed. In recent years pharmacists have requested the same consideration with regard to dispensing some medicines. This paper examines the pharmacists' role and their professional and moral obligations to patients in the light of recent refusals by pharmacists to dispense oral contraceptives. A review of John Rawls's concepts of the "original position" and the "veil of ignorance", alon

This article considers strategies for illuminating health systems' structural violence toward people experiencing homelessness and for resisting incursion of moral injury to health professional learners. This article also canvasses the nature and scope of educators' obligations to teach in patient-focused ways that motivate equity and students' capacity to serve some of the country's most vulnerable residents in clinical settings or on the streets.

Such prominent authors as Tina Rulli have argued in favour of pro tanto duties to adopt instead of procreating. This article interrogates-and then subsequently extends support for-such a view. Specifically, this paper develops and defends a conditional moral obligation to adopt rather than procreate for agents who intend to become parents by drawing on antinatalist philosophy. It argues that even non-antinatalists should recognize what I call "antinatalistic pressures": reasons arising from the harms and risks of existence that shift the justificatory burden onto would-be procreators. I formul

Cure occupies a prominent place among the goods of medicine, yet its limits raise a fundamental question about medicine's constitutive end. Chronic illness, disability, dementia, frailty, and dying reveal circumstances in which medicine retains substantial obligations despite the absence of cure. These cases invite renewed reflection on what medicine is fundamentally for. This article argues that care constitutes medicine's moral foundation and the broader framework within which cure derives its meaning and purpose. Drawing primarily on Edmund Pellegrino's philosophy of medicine, together with

Judith Jarvis Thomson famously argued that abortion is permissible even if we accept that a fetus qualifies as a person and possesses a right to life. The current paper presents two arguments that undermine Thomson's position. First, the paper sketches a contractarian argument that explores Thomson's violinist thought experiment from behind a veil of ignorance, which suggests that if we had an equal likelihood of being an unwanted fetus and a pregnant woman, it would be rational for us to oppose abortion. Second, the paper discusses the hypothetical self-aborting fetus, a thought experiment th

Molecular genetic engineering technologies such as CRISPR/Cas9 have made the accurate and safe genetic engineering of human embryos possible. Further advances in genomics have isolated genes that predict qualities and traits associated with intelligence. Given these advances, prospective parents could use these biotechnologies to genetically engineer future children for genes that enhance their intelligence. While Julian Savulescu's Principle of Procreative Beneficence (PPB) argues for the moral obligation of prospective parents to use in-vitro fertilization and preimplantation genetic diagnos

We respond to David Wilkinson's arguments against our view of the ethicality of doctors' strikes and our claim that the 2023-2024 UK doctors' strikes are morally permissible and arguably supererogatory.Wilkinson proposes that in specialist outpatient settings, striking doctors should help arrange their own cover to prevent disproportionate harm to patients and to abide by the principles of non-maleficence and fiduciary duty. This hasn't happened during the 2023-2024 UK doctors' strikes; therefore, in his view, these strikes are morally impermissible. We reject Wilkinson's proposal on the groun

The dead donor rule (DDR) originally stated that organ donors must not be killed by and for organ donation. Scholars later added the requirement that vital organs should not be procured before death. Some now argue that the DDR is breached in donation after circulatory determination of death (DCDD) programs. DCDD programs do not breach the original version of the DDR because vital organs are procured only after circulation has ceased permanently as a consequence of withdrawal of life-sustaining therapy. We hold that the original rendition of the DDR banning killing by and for organ donation is

Roughly 80,000 U.S. prisoners are held in solitary confinement at any given time. A significant body of research shows that solitary confinement has severe, long-term effects, and the United Nations has condemned the practice of solitary confinement as torture. For years, prisoners have been organizing hunger strikes in order to protest solitary confinement. But such action is not without consequences, and some inmates have suffered serious injury or death. The question I raise in this paper is whether we ought to force-feed hunger striking prisoners when serious harm is imminent. Both the Wor

Recently, debate over whether health care providers should have a protected right to conscientiously refuse to offer legal health care services-such as abortion, elective sterilization, aid in dying, or treatments for transgender patients-has grown exponentially. I advance a modified compromise view that bases respect for claims of conscientious refusal to provide specific health care services on a publicly defensible rationale. This view requires health care providers who refuse such services to disclose their availability by other providers, as well as to arrange for referrals or facilitate

The intrapartum management of pregnancy complicated by fetal hydrocephalus with macrocephaly often confronts the physician with a complex array of moral obligations to both mother and fetus that may sometimes come into direct conflict. The authors analyze the component beneficence-based and autonomy-based obligations of these conflicts and offer strategies for resolving them for each of three clinical subgroups. For isolated fetal hydrocephalus, we argue that cesarean section should be performed except when the woman with full informed consent unwaveringly refuses. For fetal hydrocephalus with

Conscience is typically invoked in healthcare to defend a right to conscientious objection, that is, the refusal by healthcare professionals to perform certain activities in the name of personal moral or religious views. On this approach, freedom of conscience should be respected when the individual is operating in a professional capacity. Others would argue, however, that a conscientious professional is one who can set aside one's own moral or religious views when they conflict with professional obligations. The debate on conscientious objection has by and large crystallised around these two

The article examines five controversial views, expressed in Jonathan Swift's A Modest Proposal, Helga Kuhse and Peter Singer's Should the Baby Live? The Problem of Handicapped Infants, Alberto Giubilini and Francesca Minerva's "After-birth abortion: why should the baby live?", Julian Savulescu's "Procreative beneficence: why we should select the best children", and the author's "A rational cure for prereproductive stress syndrome". These views have similarities and differences on five levels: the grievances they raise, the proposals they make, the justifications they explicitly use, the justif

BACKGROUND AND AIMS: The World Health Organization (WHO) Framework Convention on Tobacco Control (FCTC) seeks to realize the right to health through national tobacco control policies. However, few states have met their obligations under Article 14 of the FCTC to develop evidence-based policies to support tobacco cessation. This article examines how human rights obligations could provide a legal and moral basis for states to implement greater support for individuals to overcome their addiction to tobacco.
ANALYSIS: The United Nations (UN) has a well-established legal framework for promoting hu
