Many drugs have been found to increase or decrease the clearance of theophylline, probably by interaction with one or more of the variants of the cytochrome P450 drug-metabolising system. Theophylline may be particularly susceptible to alteration of its clearance because of the particular form(s) of the P450 system involved, because its metabolism is saturable, and/or because 90% of its elimination is via metabolism. Its clearance has been found to be decreased (typically by around 25%, but often by far more) by erythromycin, troleandomycin (triacetyloleandomycin), roxithromycin, enoxacin, cip

A single dose of 300 mg of ethmosine per os has been found to exert an antiarrhythmic action 1-2 h after its administration in 60% of the patients and may be used to terminate episodes of ventricular arrhythmias. 3-4 h after the beginning of its administration, a complete antiarrhythmic effect was observed in giving a "loading" dose of 500-600 mg of ethmosine at the beginning of the treatment followed by maintenance therapy (200 mg given every 8 hours). To treat patients with ventricular rhythm disturbances, ethmosine may be used in a daily dose of 600-900 mg without side effects.

The electrophysiological effects of intravenously administered ethmozin (1 mg kg-1) on sinus node function were examined in 32 patients. Based on their clinical data and the results of the initial study, patients were subdivided into group A (n = 12), those with sick sinus syndrome and sinus node recovery time (SNRT) greater than 1500 ms, and group B (n = 20), those without overt sinus node dysfunction and a normal SNRT. The mean sinus cycle length did not change significantly in either group after ethmozin . SNRT was prolonged by ethmozin in group A whereas in group B the increase of SNRT was

A hundred and twenty two patients with premature ventricular contractions and paroxysmal tachycardias were comprehensively studied prior to and following treatment regimen with ethacizin, befol, sodium succinate and their combinations. Besides its significant antiarrhythmic activity, ethacizin displayed a number of adverse cardiac effects, such as excessive negative inotropic and dromotropic ones. Befol and sodium succinate in combination with ethacizin can reduce or even eliminate these side effects in rest and during simulated mental stress and graded exercise. The antiarrhythmic effect of t

Voltage-clamp studies of myelinated nerve fibers that are designed to determine structural criteria regarding selective drug blocking of open and inactive states of the sodium channel are described. A series of phenothiazines were studied. It was shown that two of these drugs (ethmozine and ethacizine, at 5 microM) require open channels for blocking action and the other two (chlorpromazine and chloracizine, at 5 microM) can effectively block inactive channels. A size criterion, which looks at the spanning width at the aromatic end of these molecules, can explain this qualitative difference in

Ventricular fibrillation (VF) is a major cause of sudden cardiac death in humans. Currently used antiarrhythmic drugs are aimed at preventing initiation of VF by decreasing the incidence of arrhythmias which can lead to VF. This approach today seems to be insufficient. On the basis of reports that VF can terminate spontaneously in various mammals, and even in humans, we propose pharmaceutical enhancement of self-ventricular defibrillation as a new therapeutical approach. Data obtained over the last decade indicate that a high cardiac extraneuronal noepinephrine level during VF facilitates self

Changes in contraction amplitudes, contraction peak time and relaxation time were evaluated at 50% and 80% of contraction peak, with adrenaline or strophanthin acting on the myocardium pretreated with antiarrhythmic substances, using atrial specimens from patients with acquired heart diseases and septal defects. Etmozin and ethacizine were shown to have a negative inotropic effect, reducing contraction intensity by about 50%. Adrenaline and strophanthin are capable of restoring myocardial contractility in the presence of antiarrhythmic effects without actually inducing spontaneous auto-arrhyth

The results of treating 95 patients suffering from various disorders of cardiac rhythm with ethmosin are discussed. Sixty-eight patients received oral ethmosin in a daily dose of 600 mg, 12 of them were treated for a long time (85 +/- 39 days on the average), the others for no more than 3--7 days. Fifty patients received a daily dose of 150 mg ethmosin in a single intravenous injection. The therapeutic effect was considered positive if the number of extrasystoles reduced by no less than 50% with complete removal of group extrasystoles and paroxysms of tachycardia according to the data of daily

An electrophysiologic study of ethacizin's mechanisms of action was carried out in patients with paroxysmal atrioventricular nodal tachycardia. Tachycardia was controlled by 0.5 mg/kg ethacizin in all patients. No patients demonstrated induced tachycardia in the presence of the drug, and 55% developed a complete retrograde atrioventricular block. The assessment of the preventive effect of oral ethacizin administration showed that paroxysms of tachycardia could not be provoked by esophageal electrostimulation of the heart in 87% of the patients. In the same patients, stable antiarrhythmic effec

The action of two antiarrhytmic drugs, moracizine (MOR, CAS 31883-05-3) and ethacizine (ETHA, CAS 33414-33-4) on receptors of potential-operated CA-channels has been investigated. ETHA binding to verapamil receptors was more effective than that of MOR (IC50 = 0.53 +/- 0.08 mumol/l, respectively). The Hill coefficient for ETHA binding was similar to that of verapamil (0.64 +/- 0.09 and 0.60 +/- 0.10, respectively). Interaction of ETHA and MOR with dihydropyridine receptors in concentrations up to 10 mumol/l was similar that of verapamil, however, MOR was less potent. MOR and ETHA did not intera

The authors observed 127 patients with different disorders of the cardiac rhythm treated with ethmozine, cordarone and rhythmodan. To assess the efficacy of the therapy circadian ECG was recorded on a magnetic tape with a portable cardiomonitor, exercise tests (bicycle ergometry, treadmill and walking under ECG control) were conducted. Good effect was seen after a course of treatment in patients with ventricular and atrial extrasystolae, paroxysms of supraventricular, ventricular tachycardia and auricular fibrillation. Cordarone was found effective in the Wolff-Parkinson - White syndrome and p

Fifty-six patients with frequently occurring paroxysms of atrial fibrillation were examined for the clinical efficacy of a new antiarrhythmic drug etacyzine (a diethylamine analog of etmozin). The use of the drug in doses of 50 to 70 mg intravenously and in doses in 150 to 200 mg per os a day removed and prevented the occurrence of atrial fibrillation paroxysms in 60 to 65% of cases, whereas etmozin in doses of 50 mg intravenously and 600 to 800 mg per os day did not produce any positive anti-arrthythma effect in the patients. The prolonged use of etacyzine (up to 1 year and over) exerted a be

Electrophysiologic effects of intravenous ethmozin (1.5 to 2 mg/kg) were evaluated in 16 patients (10 with Wolff-Parkinson-White [WPW] syndrome and six with concealed accessory pathway [AP]) with ventricular preexcitation syndrome. Ethmozin terminated induced supraventricular tachycardia (SVT) in 9 of 14 patients and atrial flutter with anterograde conduction 2: 1 over AP in one patient. The drug prevented induction of sustained SVT in 8 of 14 patients (four with WPW syndrome and four with concealed AP). The drug significantly lengthened the cycle length of induced SVT in WPW syndrome (381 +/-

OBJECTIVE: To review the literature regarding the use of antiarrhythmic agents in the management of atrial flutter (AF), atrial fibrillation (Afib), junctional ectopic tachycardia (JET), and atrial ectopic tachycardia (AET) in infants and children. To discuss the advantages and disadvantages of specific agents in each type of arrhythmia in an effort to develop treatment guidelines.
DATA SOURCE: A MEDLINE search encompassing the years 1966-1996 was used to identify pertinent literature for discussion. Additional references were found in the articles, which were retrieved via MEDLINE.

Antiarrhythmic treatment is based on the hypothesis that ventricular premature beats (VPBs), in the presence of underlying cardiac disease and impaired ventricular function, may predispose to sudden cardiac death. The effectiveness of treatment, however, has not been proven. For acute treatment of paroxysmal ventricular tachycardia, on comparison of the effectiveness of lidocaine and ajmaline, some new aspects have been rendered. VENTRICULAR PREMATURE BEATS (VPB): Isolated VPBs can be found in 40 to 75% of healthy subjects; if their number is substantial, investigation is warranted. For VPBs w

Alzheimer's disease (AD) is a heterogeneous neurodegenerative disease driven by pathological depositions of proteins that accumulate over decades. Compelling genetic and neurobiological evidence suggests that amyloid accumulation in the brain initiates and drives early-stage AD. Measurement of fibrillar amyloid has been pivotal to the development and approval of disease-slowing treatments. Various biomarkers of AD pathophysiology provide evidence of target engagement and downstream effects on disease progression, and their use as surrogate endpoints may help identify and expeditiously bring ne
