The role of physiotherapy after total knee arthroplasty in patients with haemophilia.

With the availability of clotting factor concentrates, advances in surgical techniques, better implant design, and improvements in postoperative management, total knee arthroplasty has become the treatment of choice for haemophilia patients suffering from end-stage haemophilic knee arthropathy. The success of this surgery is also dependent on close collaborations among the orthopaedic surgeon, the haematologist and the physiotherapist. Although haemophilic patients undergoing this surgery would likely benefit from a targeted rehabilitation programme, its specificities, modalities and limitatio

[Early functional treatment of congenital clubfoot].

The concept of conservative clubfoot treatment beginning in the neonatal period has changed considerably in the last 2 decades. The rate of clubfeet requiring surgery (20%) and the extent of surgical interventions have dropped significantly. The early functional Montpellier method has proved to be highly effective for all degrees of deformity. It is used for patients from birth until the age of 1 year. The treatment during the first 3 months of life is decisive and consists of daily physical therapy, use of a continuous passive motion machine, and taping. This method represents an efficient al

The features of Gait Exercise Assist Robot: Precise assist control and enriched feedback.

BACKGROUND: In a patient with severe hemiplegia, the risk of the knee giving way is high during the early stage of gait exercise with an ankle-foot orthosis. However, use of a knee-ankle-foot orthosis has many problems such as large amount of assistance and compensatory motions. To resolve these problems, we have engaged in the development of the Gait Exercise Assist Robot (GEAR). OBJECTIVE: To evaluate the improvement efficiency of walk with GEAR in a stroke patient. METHODS: The subject was a 70-year-old man presented with left thalamus hemorrhage and right hemiplegia. The patient underwen

Kinesthetic sense of the shoulder in patients with impingement syndrome.

A proprioceptive deficit is an important determinant of disability in various shoulder disorders, such as instability and osteoarthrosis. In 15 patients with impingement syndrome stage II (Neer 1983), who were treated by arthroscopic subacromial decompression, we measured movement sense by determining threshold levels for the perception of motion of the shoulder. The patients were placed in a specially designed chair allowing continuous passive motion of the shoulder joint, while avoiding cutaneous, auditory and visual stimuli. To assess movement detection thresholds, passive abduction movemen

[Experimental studies of flexor tendon repair in "no man's land"].

The FDP of the middle toes of 51 chickens were divided and repaired. All of the chickens were divided into three groups, each had a procedure of protected passive mobilization beginning at 1, 5, 21 postoperative days respectively. Through studies by micrography, light-microscopy, transmission and scanning electronmicroscopy, we found that passive motion beginning at 1 day and 5 days after operation has similar effects on the healing tendons by preventing adhesions, stimulating proliferation of epitenon and remoulding tendon callus. On the basis of the above results, another 32 chickens were us

Open surgery versus nonoperative treatments for paediatric trigger thumb: a systematic review.

The purpose of this systematic review was to determine the outcome of interphalangeal (IP) joint motion in children undergoing open surgical release, splinting, and passive exercising therapy for the treatment of paediatric trigger thumb. We conducted an online literature search of seven major databases. Only studies with a mean follow-up of at least 12 months were considered for inclusion. Seventeen retrospective studies and one prospective study met all the inclusion criteria. They reported on the results of surgery (634 children, 759 thumbs), splinting (115 children, 138 thumbs), and passiv

A Novel Clinical Test for Setting Intermittent Pneumatic Compression Parameters Based on Edema Fluid Hydromechanics in the Lymphedematous Calf.

BACKGROUND: Long-term observations confirm lasting effects and lack of complications of intermittent pneumatic compression (IPC) therapy. So far, no test has been designed that would provide data necessary for setting pressure and time parameters of the IPC device to obtain optimum decrease in limb volume. AIM: To design a test providing data on decrease of circumference under the inflated chamber in time depending on the applied compression pressure. METHODS: One chamber was placed above the ankle joint and inflated to 120 mmHg in order to occlude tissue fluid backflow during inflation of t

The use of a passive motion machine in the postoperative rehabilitation of Dupuytren's disease.

The purpose of this study was to prospectively compare the postoperative use of passive motion (PM) and standard hand therapy after surgical treatment of Dupuytren's disease for extent and rate of recovery of joint motion. Our data indicate that metacarpophalangeal joint contractures improved completely to a mean of 0 degrees, regardless of the postoperative protocol. Contractures at the proximal interphalangeal joint showed incomplete recovery in both study groups, with a mean residual contracture of 28 degrees for PM patients and 38 degrees for control patients. Contractures of the metacarpo

Intrasynovial flexor tendon repair. An experimental study comparing low and high levels of in vivo force during rehabilitation in canines.

BACKGROUND: Rehabilitation methods that generate increased tendon force and motion have been advocated to improve results following intrasynovial flexor tendon repair. However, the effects of rehabilitation force and motion on tendon-healing may be masked by the high stiffness produced by newer suture methods. Our objective was to determine whether the biomechanical properties of tendons repaired by one of two multistrand suture methods were sensitive to an increased level of applied rehabilitation force. METHODS: Two hundred and fourteen flexor digitorum profundus tendons from 107 adult dogs

The voluntary driven exoskeleton Hybrid Assistive Limb (HAL) for postoperative training of thoracic ossification of the posterior longitudinal ligament: a case report.

CONTEXT: The hybrid assistive limb (HAL) is a wearable robot suit that assists in voluntary control of knee and hip joint motion by detecting bioelectric signals on the surface of the skin with high sensitivity. HAL has been reported to be effective for functional recovery in motor impairments. However, few reports have revealed the utility of HAL for patients who have undergone surgery for thoracic ossification of the posterior longitudinal ligament (thoracic OPLL). Herein, we present a postoperative thoracic OPLL patient who showed remarkable functional recovery after training with HAL. FIN

Continuous passive motion for preventing venous thromboembolism after total knee arthroplasty.

BACKGROUND: Total knee arthroplasty (TKA) is a common form of orthopaedic surgery. Venous thromboembolism (VTE), which consists of deep venous thrombosis (DVT) and pulmonary embolism (PE), is a major and potentially fatal complication after TKA. The incidence of DVT after TKA is 40% to 80% and the incidence of PE is approximately 2%. It is generally agreed that thromboprophylaxis should be used in patients who undergo TKA. Both pharmacological and mechanical methods are used in the prevention of DVT. Pharmacological methods alter the blood coagulation profile and may increase the risk of bleed
