Estimated Burden of Serious Fungal Infections in Mozambique.

Mozambique is a sub-Saharan African country with limited information on the burden of fungal disease. We estimate the burden of serious fungal infections for the general healthy population and for those at risk, including those infected with HIV, patients with asthma, as well as those under intensive care. We consult the Mozambican National Institute of Statistics Population and Housing Census report to obtain denominators for different age groups. We use modelling and HIV data to estimate the burdens of Pneumocystis jirovecii pneumonia (PCP), Cryptococcal meningitis (CM) and candidiasis. Asth

Epidemiology of Disease and Mortality From a PICU in Mozambique.

OBJECTIVES: Delivery of pediatric critical care in low-income countries is limited by a lack of infrastructure, resources, and providers. Few studies have analyzed the epidemiology of disease associated with a PICU in a low-income country. The aim of this study was to document the primary diagnoses and the associated mortality rates of patients presenting to a tertiary PICU in Mozambique in order to formulate quality improvement projects through an international academic partnership. We hypothesized that the PICU mortality rate would be high and that sepsis would be a common cause of death. D

Determinants of gastric CDX2 expression: a study in Mozambique.

As CDX2 expression precedes the occurrence of gastric preneoplastic lesions in the intestinal differentiation pathway, study of these steps of gastric carcinogenesis may contribute toward understanding the early effects of gastric cancer determinants. Our aim was to quantify the association between Helicobacter pylori infection and other environmental factors and the gastric expression of CDX2. Dyspeptic patients undergoing an upper digestive endoscopy (Gastroenterology Department, Maputo Central Hospital) were consecutively invited to participate in this study and classified as having normal

Men's migration and women's fertility in rural Mozambique.

Labor migration profoundly affects households throughout rural Africa. This study looks at how men's labor migration influences marital fertility in a context where such migration has been massive while its economic returns are increasingly uncertain. Using data from a survey of married women in southern Mozambique, we start with an event-history analysis of birth rates among women married to migrants and those married to nonmigrants. The model detects a lower birth rate among migrants' wives, which tends to be partially compensated for by an increased birth rate upon cessation of migration. A

Chronic microsporidial enteritis in a missionary from Mozambique.

Microsporidiosis often occurs in immunocompromised persons but may also occur in those who are immunocompetent. Infection by Microsporidia involves a variety of organs and systems, most notably, intestine, lung, kidney, brain, sinuses, muscle, and eyes. Enterocytozoon bieneusi and Encephalitozoon intestinalis are associated with gastroenteritis, and Enterocytozoon hellem and Encephalitozoon cuniculi are associated with keratoconjunctivitis. We report a case of chronic microsporidiosis in a 28-year-old woman missionary from Mozambique who came to our diagnostic laboratory with nausea, lower abd

Private demand for cholera vaccines in Beira, Mozambique.

In the summer of 2005, we interviewed 996 randomly selected respondents in Beira, Mozambique concerning their willingness and ability to pay for cholera vaccine for themselves and for other household members. Respondents were told that two doses of the vaccine would be required 2 weeks apart, and that the cholera vaccine would offer excellent protection against infection for the first year following vaccination, and some protection during the second and third year after a person is vaccinated. This research was carried out in order to learn more about private demand for vaccines in a cholera-e

Acute outbreak of porcine parvovirus infection in Mozambique.

Investigations were made to determine the causal agent of an acute outbreak of abortions recorded in a swine herd in Mozambique. Isolation of porcine parvovirus and demonstration of its specific antibodies accomplished by using enzyme-linked immunosorbent assay, haemagglutination inhibition and immunofluorescent tests, indicated that porcine parvovirus was the causal agent of the abortions. Other pathogenic agents causing reproductive failure, e.g. pseudorabies virus, Leptospira or Brucella species, were ruled out because investigations of these agents proved to be negative.

Abortion Legal Reform and Neonatal Mortality in Mozambique.

INTRODUCTION: Abortion law reforms have been hypothesized to influence reproductive, maternal, and neonatal health services and health outcomes, as well as social inequalities in health. In 2014, Mozambique legalized abortion in specific circumstances. However, due to challenges implementing the law, there is concern that it may have negatively influenced neonatal outcomes. METHODS: Using a difference-in-differences design, we used birth history data collected via the Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys (MICS) between 2004 and 2018 to assemble a panel o

Incidence of porcine cysticercosis in Angónia District, Mozambique.

A total of 108 pigs from an endemic area in Mozambique were selected and followed for 8 months to estimate the prevalence and incidence of Taenia solium cysticercosis as indicators of ongoing transmission of the disease. The pigs were sampled and tested repeatedly for cysticercosis by Ag-ELISA at 4, 9 and 12 months of age. Porcine cysticercosis was diagnosed in 5.6% (95% CI: 2.1-11.7%), 33.3% (95% CI: 23.7-44.1%) and 66.7% (95% CI: 55.5-76.9%) of the animals, for the first, second and third sampling rounds, respectively, and varied by village. The mean incidence rate of the disease increased s

Acceptability of cervical cancer screening in rural Mozambique.

In Zambézia province, Mozambique, cervical cancer (CC) screening was introduced to rural communities in 2010. Our study sought to determine whether women would accept screening via pelvic examination and visual inspection with acetic acid (VIA) at two clinical sites near the onset of a new CC screening program. A cross-sectional descriptive study of 101 women was undertaken in two rural communities in north-central Mozambique. We assessed a woman's willingness to be screened, knowledge about CC symptoms and treatment, and her recommendations for best methods to deliver information to other wom

Sociocultural and epidemiological aspects of HIV/AIDS in Mozambique.

BACKGROUND: A legacy of colonial rule coupled with a devastating 16-year civil war through 1992 left Mozambique economically impoverished just as the human immunodeficiency virus (HIV) epidemic swept over southern Africa in the late 1980s. The crumbling Mozambican health care system was wholly inadequate to support the need for new chronic disease services for people with the acquired immunodeficiency syndrome (AIDS). METHODS: To review the unique challenges faced by Mozambique as they have attempted to stem the HIV epidemic, we undertook a systematic literature review through multiple search

Cholera epidemiology in Mozambique using national surveillance data.

BACKGROUND: Mozambique has experienced cholera for several decades. This study was undertaken to evaluate epidemiologic patterns to assist in guiding public health interventions. METHODS: We evaluated district-level Ministry of Health data for 123 consecutive weeks starting 1 January 2009. Cholera cases reported to the national level were based on clinical suspicion rather than microbiological confirmation. Time and space analyses with mapping and spatial statistics were undertaken. RESULTS: During 2009-2011, Mozambique identified 220 deaths among the 25 431 reported suspected cholera cases

Lessons learned from bednet distribution in Central Mozambique.

INTRODUCTION: Malaria is an important cause of mortality and morbidity in sub-Saharan Africa. Use of insecticide-treated bednets (ITNs) is an important preventive intervention. Selection of the best mechanisms for distribution and promotion of ITNs to vulnerable populations is an important strategic issue. METHODS: Commercial shopkeepers and groups of community leaders were trained to promote and sell ITNs in 19 sites in central Mozambique between 2000 and 2004. Pregnant women and children under 5 years of age comprised the target population. Sales records, household survey results and projec
