Psychometric properties of the Spanish adherence to exercise for musculoskeletal pain tool (ATEMPT) in people with musculoskeletal chronic pain.

OBJECTIVE: To translate, culturally adapt, and validate the Spanish version of the Adherence to Exercise for Musculoskeletal Pain Tool (ATEMPT) for use in Spanish-speaking individuals with musculoskeletal pain. METHODS: A cross-sectional psychometric validation study was conducted in accordance with COSMIN and international guidelines for cross-cultural adaptation. The translation process followed a forward-backward method and included expert review and pilot testing. Psychometric analyses included internal consistency (Cronbach's alpha), test-retest reliability (intraclass correlation coeffi

Chronic pelvic pain: comorbidity between chronic musculoskeletal pain and vulvodynia.

Chronic pelvic pain (CPP) is a common condition that has a major impact on the quality of life of both men and women. Male CPP is usually attributable to well-defined urogenital conditions (most frequently infectious/non infectious prostatic diseases) or musculoskeletal or bowel diseases, whereas the features of female CPP are much more complex and are of particular clinical and epidemiological importance. It is a multifactorial syndrome that can be due to diseases of the urogenital, gastrointestinal, or musculoskeletal systems, or to neurological or neuropsychiatric disorders. It is not alway

A sailor's pain: Veterans' musculoskeletal disorders, chronic pain, and disability.

A few years after leaving the navy, a 50-year-old Veteran* presents to a new family physician with chronic knee and back pain. He is seeking a new physician for opioid and benzodiazepine refills, referrals for ongoing acupuncture and massage therapy, and completion of Veteran Affairs Canada (VAC) disability claim forms for his back. He was medically released at the rank of Petty Officer owing to knee impairment secondary to a fracture sustained aboard ship. He twice strained his back on deployments, but did not develop chronic low back pain until after leaving the Canadian Forces (CF). On rele

Musculoskeletal Pain in Surgeons on Operating Days.

UNLABELLED: Work-related musculoskeletal (MSK) pain is frequent in surgeons, but factors associated with it are not well studied. METHODS: Repeat surveys were sent to general and subspecialty surgeons in the evening of their operating room (OR) days in one month. Age, sex, BMI, weekly exercise, NASA Task Load Index (NASA-TLX: a composite measure of workload), and pain scores were collected. Operating room case length and approach were collected from a registry. All variables were analyzed for their independent association with mean pain score changes in 12 body regions (with 95% CI). RESULTS

Role of proprioceptors in chronic musculoskeletal pain.

Proprioceptors are non-nociceptive low-threshold mechanoreceptors. However, recent studies have shown that proprioceptors are acid-sensitive and express a variety of proton-sensing ion channels and receptors. Accordingly, although proprioceptors are commonly known as mechanosensing neurons that monitor muscle contraction status and body position, they may have a role in the development of pain associated with tissue acidosis. In clinical practice, proprioception training is beneficial for pain relief. Here we summarize the current evidence to sketch a different role of proprioceptors in 'non-n

Pathophysiology of musculoskeletal pain: a narrative review.

Musculoskeletal pain (excluding bone cancer pain) affects more than 30% of the global population and imposes an enormous burden on patients, families, and caregivers related to functional limitation, emotional distress, effects on mood, loss of independence, and reduced quality of life. The pathogenic mechanisms of musculoskeletal pain relate to the differential sensory innervation of bones, joints, and muscles as opposed to skin and involve a number of peripheral and central nervous system cells and mediators. The interplay of neurons and non-neural cells (e.g. glial, mesenchymal, and immune

Tapentadol for chronic musculoskeletal pain in adults.

BACKGROUND: Chronic musculoskeletal pain is a prevalent condition and a major cause of disability and absence from the workplace worldwide. Opioids are frequently used to treat chronic pain, although adverse effects often restrict their long-term benefits. Tapentadol is an opioid and norepinephrine re-uptake inhibitor, which may cause a lower incidence (and severity) of adverse effects compared to other strong opioids. OBJECTIVES: To determine the efficacy, safety and tolerability of tapentadol extended release for moderate-to-severe pain for at least three months for any musculoskeletal caus

Use of botulinum toxin in musculoskeletal pain.

Chronic musculoskeletal pain is a common cause of chronic pain, which is associated with a total cost of $635 billion per year in the U.S. Emerging evidence suggests an anti-nociceptive action of botulinum toxin, independent of its muscle paralyzing action. This review provides a summary of data from both non-randomized and randomized clinical studies of botulinum toxin in back pain and various osteoarticular conditions, including osteoarthritis, tennis elbow, low back pain and hand pain. Three randomized controlled trials (RCTs) of small sizes provide evidence of short-term efficacy of a sing

An overview of the management of persistent musculoskeletal pain.

Musculoskeletal pain is a common reason for patients to seek care from healthcare providers. These conditions are predominantly nontraumatic injuries with a broad differential diagnosis and often without a specific diagnosis despite thorough history and examination. The management of these conditions is often discouraging for patients with continued chronic symptoms despite numerous diagnostic workups and treatment options. Effective communication with respect to the patient's goals is the key to a good outcome. The combination of an accurate diagnosis, an informed, motivated patient and a com

The idiopathic musculoskeletal pain syndromes in childhood.

Idiopathic musculoskeletal pain syndromes in children have a variety of manifestations; they can be diffuse or well localized, constant or intermittent, with or without autonomic symptoms and signs, completely incapacitating or not limiting activities, and they can tax the physician's diagnostic skill. A careful history and examination is usually all that is needed to make a diagnosis, although the differential diagnosis is large and might require laboratory and radiographic investigation. Pain and functional assessment help track the progress with therapy. Intense exercise therapy is associat

Short-term variability of chronic musculoskeletal pain.

Chronic musculoskeletal (MSK) pain can be characterized by its temporal variability and evolution, affecting both pain management and treatment outcomes. While pain variability is traditionally studied over long timescales (e.g. days or weeks), few studies have explored short-term fluctuations (e.g. minutes to seconds) and their clinical relevance. This study investigated the short-term variability of chronic musculoskeletal pain across consecutive days, examining whether these fluctuations are stable, exhibit consistent temporal patterns, and relate to clinical severity. We also explored whet

[Chronic musculoskeletal pain syndrome in primary hyperparathyroidism].

Chronic somatic pain disorders with somatic and mental factors (ICD-10: F45.41) are common among psychosomatic patients. In the present case, due to the close temporal association with a trauma and the subsequent development of symptoms including depressive symptoms, a chronic pain disorder with a relevant somatoform component was suspected. However, after a period of several months without significant somatic findings, targeted diagnostic approaches resulted in the diagnoses of primary hyperparathyroidism and a papillary thyroid carcinoma. Surgical therapy resulted in an almost complete decli

Diffuse musculoskeletal pain syndromes in pediatric practice.

Diffuse musculoskeletal pains in children and adolescents are common. Females are affected relatively more often. Whereas growing pains and joint hypermobility as possible causes tend to occur in younger children, fibromyalgia syndrome (FMS) appears to be more frequent in adolescents. In growing pains, typically, a) the pains are localized to the thighs, shins, or calves, b) occur in the evening or at night; and c) are usually relieved by massage. Children are otherwise healthy and have normal growth and development. Children with joint hypermobility manifest pain mainly in the knees, ankles,

White matter involvement in chronic musculoskeletal pain.

UNLABELLED: There is emerging evidence that chronic musculoskeletal pain is associated with anatomic and functional abnormalities in gray matter. However, little research has investigated the relationship between chronic musculoskeletal pain and white matter. In this study, we used whole-brain tract-based spatial statistics and region-of-interest analyses of diffusion tensor imaging data to demonstrate that patients with chronic musculoskeletal pain exhibit several abnormal metrics of white matter integrity compared with healthy controls. Chronic musculoskeletal pain was associated with lower
