The deltopectoral flap v the pectoralis major myocutaneous flap. Which one?

During the past 2 1/2 years, we have used 21 deltopectoral (DP) and 25 pectoralis major myocutaneous (PM) flaps of which 25 were applied to oral and pharyngeal defects. The concurrent use of both flaps has enabled elucidation of some specific and relative advantages and disadvantages. The following five situations have been identified in which we believe the PM flap is clearly superior: (1) lateral mandibular composite resection defects; (2) large anterior segmental defects; (3) reconstitution of tongue and base of tongue; (4) radionecrosis; and (5) through-and-through defects of skin and oral

[Sternohyoid myocutaneous flap in pediatric laryngotracheal stenosis treatment].

Objective:To assess the indications and outcomes of laryngotracheal reconstruction (LTR) using the sternohyoid myocutaneous flap (SMF) in pediatric patients with laryngotracheal stenosis. Method:Twenty patients (15 males, 5 females; mean age, 9 years old, range 3 to 14) with laryngotracheal stenosis who underwent SMF procedures at Tangdu Hospital between August 1991 and October 2014 were analyzed. Stenosis was classified according to the Myer-Cotton grading system. Four patients were grade Ⅱ, ten were grade Ⅲ, and six were grade Ⅳ. The mean length of stenosis was 1.8 cm (rang, 0.3-4.0 cm). Septal-myocutaneous flap technique for lower lid blepharoplasty.

Lower lid blepharoplasty can present a significant challenge to the facial plastic surgeon. Routine findings of periorbital fat herniation and dermatochalasia of the lower lid are often associated with the presence of more occult findings, e.g., tarsoligamentous laxity and ectropion. Traditional surgical approaches to the aging lower eyelid utilize the skin flap, the skin-muscle flap, and the transconjunctival technique. The limitation of any one of these procedures alone is that of not addressing the multiple problems of the aging eyelid; this may lead to common postoperative problems of lowe

Infrahyoid myocutaneous flap in head and neck reconstruction.

Infrahyoid myocutaneous flap is one of the alternatives to be considered for the reconstruction of moderate defects following resection of the oral cavity, oropharynx, or hypopharynx cancers. The flap is based on the uni- or bilateral superior thyroid pedicle; its major limitations are due to small flap volume and arc of rotation. The authors reviewed a series of 15 consecutive patients with carcinomas of the oral cavity or pharynx who underwent radical surgical resections followed by immediate reconstruction using an infrahyoid myocutaneous flap. Four of five cases with prior irradiation pres

Platysma myocutaneous flap for intraoral and surface reconstruction.

Different procedures have been described for facial and oral reconstruction after malignant tumor removal, such as skin grafts and local and distal staged flaps. Myocutaneous island or free flaps are the method of choice for these reconstructions. Our experience concerns the use of the platysma myocutaneous flap in 12 patients after intraoral and extraoral malignant tumor resection. Six patients had squamous cell carcinomas, i.e., 2 malignant melanomas of the skin and 4 squamous cell carcinomas of the oral mucosa. The complete tumor resection was controlled by frozen-section biopsy in all pati

Rectus abdominis myocutaneous flap for primary vaginal reconstruction.

Reconstructive procedures are being performed with increasing frequency in conjunction with pelvic exenterations and other radical gynecologic surgeries. The most common reconstructive procedures include continent urinary diversion, rectosigmoid anastomosis, and vaginal reconstruction. Historically, the gracilis myocutaneous flap has been the procedure of choice for vaginal reconstruction. However, the gracilis myocutaneous flap has a history of partial to severe necrosis, a propensity to prolapse, and leaves ipsilateral donor scars on the thigh. In contrast, neovaginal reconstruction using a

Pectoralis myocutaneous flap for replacement of cervical esophagus.

The authors report experience with 10 cases of pharyngoesophageal replacement with tubed pectoralis major myocutaneous (PM) flap. Six patients had primary reconstruction following total pharyngolaryngectomy for cancer; the other 4 had severe pharyngoesophageal stenosis requiring resection and replacement following previous laryngectomy. Seven of the patients were octogenarians, 6 had been irradiated previously, and all were severely debilitated. Two patients died postoperatively of cardiac disease. The remaining eight regained satisfactory with lasting deglutition. Four fistulae healed spontan

Two modifications of pectoralis major myocutaneous flap (PMMF).

Pectoralis major myocutaneous flap is the most commonly used versatile flap in head and neck reconstructive surgery. The use of entirely tubed pectoralis major myocutaneous flap for reconstruction of the hypopharynx following total laryngectomy and total pharyngectomy has a disadvantage of bulkiness of the flap and poor postoperative deglutition. One-stage reconstruction of the entire hypopharynx utilizing a combination of pectoralis major myocutaneous flap and dermal graft minimizes bulkiness, thus achieving satisfactory to excellent functional results. The operation has been performed on fou

Orbitomaxillomandibular reconstruction with free osteomyocutaneous fibular flap and lower trapezius myocutaneous flap.

Several types of flaps and techniques have been used in the reconstruction of maxillomandibular defects. Myocutaneous flaps of the fibula, the scapula, the rib, the iliac bone, the radial forearm, the rectus abdominis, the anterolateral thigh, the latissimus dorsi, and the pectoralis major have been used either alone or in combination for this purpose. The aim of the current study was to discuss a 17-year-old patient with a gunshot injury who underwent orbitomaxillomandibular bone reconstruction using free fibular graft shaped as 3 pieces and soft tissue reconstruction using lower trapezius myo

A new compound osteo-myocutaneous free flap: the posterior iliac artery flap.

A hitherto undescribed osteo-myocutaneous free flap is presented. It is based on a branch of the superficial branch of the superior gluteal artery and includes the posterior iliac crest, the posterior part of the gluteus maximus and medius muscles and the overlying skin. This type of flap has allowed us to reconstruct a bony defect of up to 8 cm in the tibia.

Trapezius Myocutaneous Flap Reconstruction for the Head and Neck.

INTRODUCTION: The trapezius myocutaneous flap is a valuable option for reconstructing defects in the head and neck region requiring a sizeable skin paddle. This study assessed surgical outcomes of the trapezius myocutaneous flap in head and neck reconstruction, specifically focusing on defects of the lateral temporal bone, lateral neck, and scalp or posterior midline neck. METHODS: This retrospective study analyzed patients grouped by site of defect within the head and neck. Subgroup analysis was performed. RESULTS: Overall rates of complication for the lateral temporal bone and lateral neck

Reconstruction for cervical irradiation ulcers with myocutaneous flaps.

Radiation-induced skin changes are commonly seen in patients who have been treated for head and neck malignancies. Some of these skin changes can progress into chronic postradiation ulcers, which despite aggressive medical wound management will not resolve spontaneously. Skin grafts and local cutaneous flaps located within the radiation field are unreliable and rarely provide adequate stable coverage. In this article, the authors report a combined experience of 52 patients whose postradiation cervical ulcers were successfully and reliably treated with myocutaneous flaps.

[Value of the skin and muscle pedicle in myocutaneous flaps].

The value of a combined skin and muscle pedicle as opposed to the purely muscular pedicle is discussed using the example of the myocutaneous island skin flap of the M. latissimus dorsi in the neck-, shoulder-, and upper arm region. There is an indication scheme for the choice of flap pedicle depending on the tension and kinking of the flap or possible previous damage of the muscle pedicle.

Cervicofacial-facial artery myocutaneous flap (C-FAM flap): A novel flap in oral cavity reconstruction.

BACKGROUND: Reconstruction is an integral part of the surgical management of oral cancer. Local flaps are one of the options in reconstruction of small to moderate sized defects of oral cavity. Reconstruction is an integral part of the surgical management of oral cancer. Every defect needs to be assessed individually to tailor made the reconstruction because of its multidimensionality and complexity. Varieties of autologous tissues are being used in reconstruction like local, regional, or free flaps which can be simple or complex tissues. There is always a trend to identify the newer flaps/tec

Tongue reconstruction with the gracilis myocutaneous free flap.

We describe our experience in tongue reconstruction using the transverse gracilis myocutaneous (TMG) free flap after major demolitive surgery for advanced cancer. This technique was used in 10 patients: seven underwent total glossectomy and three partial glossectomy. In eight patients we performed motor reinnervation attempting to maintain muscular trophism and gain long-term volumetric stability. The follow-up period ranged from 6 to 28 months. The overall flap survival was 100%. Nine out of 10 patients resumed oral intake. Our preliminary experience shows that this flap is a good reconstruct

Sternocleidomastoid muscle myocutaneous flap for corrosive pharyngoesophageal strictures.

BACKGROUND: Strictures at the pharyngoesophageal junction represent a subgroup of corrosive esophageal strictures requiring a specialized management approach. Non-dilatable cricopharyngeal strictures need surgical intervention. We report the use of the sternocleidomastoid muscle myocutaneous inlay flap (SCMMIF) for reconstruction of the cervical esophagus in patients with corrosive strictures. METHODS: A SCMMIF was used in four patients with cricopharyngeal strictures. The surgical technique is described. All patients had complete dilatation of the stenosed cricopharyngeal segment as seen on

Primary thinning of the myocutaneous vastus lateralis flap.

To expand the indicational spectrum of the myocutaneous vastus lateralis flap, which is often too voluminous for intraoral application, primary thinning of the fat and muscle component of this microsurgical transplant was performed in 14 patients. The surgical technique includes subfascial localization of at least one myocutaneous perforating vessel of the lateral circumflex femoral artery and its dissection through the fascia, muscles and fatty tissue up to the skin. The epifascial fatty tissue is completely removed except for a 1-2 cm wide cuff around the perforating vessel. The thinning tec

Platysma myocutaneous flap for repair of hypopharyngeal strictures.

Hypopharyngeal strictures, either isolated or in conjunction with laryngeal and esophageal strictures, can occur following lye ingestion. Extensive stricture formation requires reconstruction to create a functional funnel system that empties below the cricoid. Esophageal replacement is not a substitute for adequate hypopharyngeal reconstruction. The pectoralis major muscle is often inadequate, because it yields too much bulk and often leads to continued aspiration. The platysma myocutaneous flap for hypopharyngeal reconstruction has not been previously reported. The inferiorly based platysma m
