The cases of six patients with pronounced, persistent, isolated gaze-evoked orbicularis oculi myokymia (GEOM) discovered fortuitously are described. None had brain-stem lesions, and two had associated, unusual cranial synkinesis (trigeminofacial, hypoglossal-facial, and spinal accessory-facial). Additional features included asymmetric blinking and pseudoblepharoclonus in a third patient and buccopharyngeal synkinesis and post-deglutional tremors in a fourth. Ptosis and pupillary changes were absent. With the possible exception of one case, GEOM seems to be a congenital benign oculofacial synki…

INTRODUCTION: The objective of this study was to describe facial myokymia in experimental animals accompanying kainic acid affects on facial motor neurons.
MATERIAL & METHODS: Anesthetized cats were injected with kainic acid into the pons adjacent to the facial nucleus. Facial movements appeared shortly after the injections and facial electromyographic potentials were recorded. Cats were killed up to 4 weeks later, the brainstems were processed histologically, and the number of neurons in the facial nucleus counted.
RESULTS: Cats receiving injection of kainic acid adjacent to facial nucleus …

Orbicularis myokymia frequently occurs in young, otherwise health individuals. The intermittent muscle fasciculations are transient and generally disappear with time. If the myokymia is persistent or progressive, neurologic assessment and investigation may be necessary. Muscle relaxants, botulinum-A toxin, and surgical myectomy are methods of treatment that only occasionally need to be considered. We present limited orbicularis myectomy and botulinum-A toxin injections as efficacious treatments in five selected intractable cases of orbicularis myokymia.

Patients with snakebites have highly variable presentations, and delayed diagnosis may lead to unfavorable outcomes. Here, we describe the case of a snakebite in a 23-year-old male who presented with myokymias. On management with mechanical ventilation and anti-snake venom, the patient improved and was discharged. The presence of myokymias may be an early clue to diagnosis and the need for mechanical ventilation in a patient with a snakebite.

Myokymia results from complex bursts of repetitive discharges of a motor unit typically attributable to a demyelinating condition. We report a 12-year-old girl with unilateral trigeminal myokymia who presented with involuntary jaw movements. Electromyography demonstrated unilateral rhythmic myokymic discharges in the left masseter and temporalis muscles at a rate of 3.5 Hz. Abnormal jaw movements spontaneously resolved over 8 months without treatment or residual deficits. The pathophysiology of myokymic discharges is discussed.

In five patients with Guillain-Barré syndrome, clinical myokymia increased and myokymic burst amplification occurred when ionized Ca++ was lowered by hyperventilation. Myokymia decreased when ionized Ca++ was increased after IV infusion of CaCl2. These responses were absent or diminished in the four patients with myokymia due to brainstem lesions, suggesting that the blood-brain barrier impedes the effects of altered serum ionized Ca++ on axonal excitability. Altering serum Ca++ can distinguish peripheral and central myokymia.

BACKGROUND: Gabapentin is a commonly used medication for neuropathic pain and epilepsy that is prescribed by a wide range of medical specialties. Adverse effects including asterixis and myoclonus have been described in patients with chronic kidney disease, but myokymia has not been previously reported.
CASE PRESENTATION: A 69-year-old man with a history of traumatic brain injury, peripheral neuropathy, amnesia, and posttraumatic stress disorder presented to the hospital after multiple falls attributed to acute onset muscle spasms. He reported taking a total daily dose of 9600 mg of gabapentin…

Facial myokymia is a clinical sign that can occur as a manifestation of demyelinating lesions. As seen in our patient with multiple sclerosis, acute-onset continuous facial myokymia can be indicative of an active lesion and can have localizing value.

Myokymia is an uncommon neuromuscular disorder that rarely affects the human larynx. No previous reports of isolated laryngeal myokymia are present in the literature, and as such, established treatment protocols are lacking. We report the first case of isolated laryngeal myokymia in a 48-year-old woman with no other neurological findings, and our successful results in initial treatment and maintenance therapy with focal intralaryngeal injections of botulinum toxin A.
