Epigastric Ventral Hernia Repair Through Vaginal Natural Orifice Transluminal Endoscopic Surgery.

OBJECTIVE: Ventral hernia repair is a commonly performed operation and can be executed by open or laparoscopic approach. The search for even less invasive techniques continues. Natural orifice transluminal endoscopic surgery (NOTES) is a known method of minimally invasive surgery. METHODS: We performed an epigastric ventral hernia repair through vaginal NOTES during a concurrent hysterectomy and bilateral salpingectomy. We used the access to do a synchronous hernia repair with mesh augmentation. The technique of repair was identical to the laparoscopic intraperitoneal onlay mesh repair (Lap.

Repeat Vaginal Natural Orifice Transluminal Endoscopic Surgery: A First Feasibility Study.

OBJECTIVES: Vaginal natural orifice transluminal endoscopic surgery (vNOTES) is an emerging minimally invasive technique in benign gynaecologic surgery whereby surgical access to the peritoneal cavity is achieved through natural orifices, namely through a vaginal colpotomy. Experience in repeat vNOTES cases is limited and so far, repeat vNOTES cases have not been described in the literature. The purpose of this study was to demonstrate the feasibility of vNOTES hysterectomy in women with a history of previous vNOTES adnexal surgery. DESIGN: We performed a retrospective cohort study of the fir

[A new way of performing hysterectomy: natural orifice transluminal endoscopic surgery].

Natural orifice transluminal endoscopic surgery (NOTES) is a minimally invasive surgical technique that uses the natural orifices of the human body such as the vagina (vNOTES) in order to access the abdominal cavity. Currently, the vNOTES hysterectomy technique is carried out on benign indication such as dysfunctional blood loss, uterine myomatosis or adenomyosis. During surgery, the surgeon and the assistant are able to operate in a better ergonomic position than at a total laparoscopic or vaginal hysterectomy. In addition, they have a better view of the anatomical structures in the operative

Natural orifice transluminal endoscopic surgery for patients with perforated peptic ulcer.

BACKGROUND: Perforation accounts for 70% of deaths attributed to peptic ulcers. Laparoscopic repair is effective but infrequently used. Our aim was to assess how many patients with perforated peptic ulcer could be candidates for a transluminal endoscopic omental patch closure. METHODS: This retrospective study reviewed patients with perforated peptic ulcer from 2005 to 2010. Demographics, ulcer characteristics, operative procedure, and outcomes were recorded. Candidates for endoscopic transluminal repair were defined as those having undergone omental patch closure of an ulcer of appropriate s

Transgastric cecectomy in canine models: natural orifice transluminal endoscopic surgery (NOTES).

BACKGROUND: Natural orifice transluminal endoscopic surgery (NOTES) for bowel resection is a challenging procedure. We studied the feasibility and safety of NOTES by performing cecectomy in dogs, which is equivalent to simple bowel resection in man. METHODS: Three dogs underwent transgastric endoscopic cecectomy with laparoscopic assistance. Antibiotics and irrigation were administered as preoperative preparation. A single-channel endoscope was introduced through the stomach into the peritoneal cavity, and the cecum was dissected and ligated with an L-knife and endoloops, respectively. Labora

The future of bariatrics: endoscopy, endoluminal surgery, and natural orifice transluminal endoscopic surgery.

This review considers the evolving role of endoscopic techniques in the treatment of obesity, and developments in minimally invasive procedures in endoscopy, endoluminal surgery and natural orifice transluminal endoscopic surgery (NOTES). Endoscopic approaches that are similar to surgery but less invasive, relying on gastric restriction and/or malabsorption, show some promise. Intragastric injection of botulinum toxin is also considered, as is the role of minimally invasive bariatric endoscopic interventions. Gastrointestinal stimulation with implanted electrodes may have potential in the trea

[Natural orifice trans-luminal endoscopic surgery (notes)--a new era in general surgery].

Four years ago, a new surgical technique was presented, the natural orifice trans-luminal endoscopic surgery (NOTES). This technique provides an incisionless operation. The surgical devices are inserted into the peritoneal cavity through the gastrointestinal or the urogenital tracts. Today, a cholecystectomy can be performed using an advanced endoscope inserted through the stomach or the vagina. The advantages of NOTES are: reduced post operative pain, no hernias, no surgical wounds infections and better cosmetic results. The disadvantages are: difficulties in achieving safe enterotomy closure

Virtual-reality endoscopic navigation system in mediastinal natural orifice transluminal endoscopic surgery (with video).

The approaches to mediastinal surgery are open thoracic, thoracoscopic, and mediastinoscopy. However, using natural orifice transluminal endoscopic surgery (NOTES) would be minimally invasive if the mediastinum is reached via the esophagus. One disadvantage of NOTES was no information outside the wall. We focused on the electromagnetic tracking solution. The sensor position in the space created by the magnetic field generator can be determined. We performed a computed tomography (CT) of a pig before constructing the target mediastinal organs in 3D. The pig was placed in the magnetic field spac

Complications during natural orifice translumenal endoscopic surgery: endoscopic management of splenic laceration and hemorrhage.

BACKGROUND: Accidental splenic laceration and hemorrhage during natural orifice translumenal endoscopic surgery (NOTES) can lead to life-threatening consequences. The NOTES approach may need to be aborted in these circumstances for a standard laparoscopy or laparotomy. AIM: To determine the feasibility of endoscopically managing intraoperative splenic laceration and hemorrhage during NOTES using standard endoscopic tools. METHODS: Nine pigs underwent transcolonic endoscopic surgery, and 18 intentional splenic lacerations were made. Animals were treated as follows: (1) control group with no t

Application of natural orifice transluminal endoscopic surgery (NOTES) instrumentation to the endolarynx.

OBJECTIVES: RealHand instruments are high-dexterity instruments that have been designed for natural orifice transluminal endoscopic surgery applications. They provide dexterity by offering a full range of motion to endoscopic instruments. We hypothesize that RealHand instruments will resolve some of the limitations encountered in traditional endolaryngeal surgery. They have the potential to do so in the following ways: (1) they negate the limitation of mobility of traditional laryngoscopy instrumentation, which is rigid and fixed; (2) they maintain the ability of direct visualization through a

Peroral dual scope for natural orifice transluminal endoscopic surgery (NOTES) gastrotomy closure.

BACKGROUND: Although transgastric intraperitoneal surgery is feasible both in experimental models and humans, secure gastrotomy closure remains challenging. As there is still no method that is simple, reliable, inexpensive, and effective, we aimed to evaluate the feasibility, efficacy, and safety of a novel endoscopic approach to this issue that intends to ensure secure healing by obtaining full thickness gastric wall apposition without requiring specialized instrumentation. METHODS: Six pigs underwent general anesthesia followed by peritoneoscopy through a 12-mm gastrotomy by a double-channe

Natural orifice surgery for rescue of stent misplacement in endoscopic gallbladder drainage.

BACKGROUND AND AIMS: Lumen-apposing metal stent (LAMS) misdeployment in endoscopic ultrasound-guided gallbladder drainage is one of the primary adverse events, but few studies, to our knowledge, have reported the misplacement of the LAMS into the abdominal cavity and the following endoscopic rescue treatment. METHODS: We reported a case of the proximal flange misplaced into the abdominal cavity in a fully expanded condition, whereas the distal flange was attached to the gallbladder, resulting in a perforation between the gallbladder and abdominal cavity, and only the puncture site was identif
