Invasive and Noninvasive Progression After Resection of Noninvasive Intraductal Papillary Mucinous Neoplasms.

OBJECTIVE: To define frequencies, pattern of progression (invasive vs noninvasive), and risk factors of progression of resected noninvasive intraductal papillary mucinous neoplasms (IPMNs). BACKGROUND: There is a risk of progression in the remnant pancreas after resection of IPMNs. METHODS: Four hundred forty-nine consecutive patients with resected IPMNs from 1995 to 2018 were included to the study. Patients with invasive carcinoma or with follow-up <6 months were excluded. Noninvasive progression was defined as a new IPMN, increased main pancreatic duct size, and increased size of an existi

Prediction of invasive carcinoma in branch type intraductal papillary mucinous neoplasms of the pancreas.

BACKGROUND: Patients with branch duct type intraductal papillary mucinous neoplasm (BD-IPMN) without invasion usually show favorable prognosis. However, the prognosis becomes poor when the IPMN lesions give rise to invasive carcinoma cells. In addition, recent studies have revealed that BD-IPMN is frequently complicated by common type pancreatic ductal carcinoma. Thus, the prognosis of BD-IPMN depends on the occurrence of these two types of invasive carcinoma. However, little is known about the risk factors for the development of these invasive carcinomas in BD-IPMN. This study aims to identif

Surgical outcomes of noninvasive and minimally invasive intraductal papillary-mucinous neoplasms of the pancreas.

BACKGROUND: Noninvasive and minimally invasive intraductal papillary-mucinous neoplasms (IPMNs) have a favorable surgical outcome. However, cases of recurrent noninvasive or minimally invasive IPMN are sometimes encountered, and the patterns of the recurrence of those tumors have not yet been fully clarified. In this study, we evaluated the surgical outcome of noninvasive and minimally invasive IPMNs, concentrating particularly on the pattern of recurrences. METHODS: Twenty patients with noninvasive and minimally invasive IPMNs were assessed. Resected specimens were evaluated histopathologica

Pancreatic parenchymal volume measurement for predicting invasive intraductal papillary mucinous neoplasm.

BACKGROUND: Surgical resection is strongly recommended for intraductal papillary mucinous neoplasm (IPMN) cases with high-risk stigmata (HRS). However, it is challenging to make this decision for elderly patients or those with multiple comorbidities. In such cases, it may be more appropriate to consider surgery for IPMN with higher malignancy potential, such as invasive IPMN. In this study, we investigated the significance of pancreatic parenchymal volume (PV) measurement in predicting invasive IPMN. METHODS: We retrospectively utilized a database comprising 162 consecutive resected IPMN case

Evaluation of the Significance of Lymphatic, Microvascular and Perineural Invasion in Patients With Pancreatic Neuroendocrine Neoplasms.

BACKGROUND: Some prognostic factors for pancreatic neuroendocrine neoplasms (PanNENs) have been reported; however, the significance of lymphatic, microvascular, and perineural invasion remains unclear. We aimed to clarify the role of these factors in PanNEN recurrence. PATIENTS AND METHODS: We analyzed 138 patients who underwent curative pancreatectomy and were pathologically diagnosed with PanNEN. We evaluated the association between clinicopathological factors and the recurrence of PanNENs. RESULTS: The numbers of patients with lymphatic, microvascular, and perineural invasion were 34 (25%

A case of serous cystic neoplasm with tumor growth acceleration leading to extrapancreatic invasion.

Serous cystic neoplasm (SCN) is a potentially malignant and invasive disease. However, there are no established guidelines regarding the surgical management of SCN. Here, we report a case of SCN with jejunal invasion that ultimately required a distal pancreatectomy with partial resection of the jejunum. The patient was a 65-year-old female who was referred to our department after a diagnosis of SCN in the pancreatic tail. CT and MRI showed a 75-mm multifocal cystic mass with calcifications; the splenic vein and left adrenal vein were entrapped within the tumor. Furthermore, the tumor was in co

Perineurial cells in granular cell tumors and neoplasms with perineural invasion: an immunohistochemical study.

Granular cell tumors (GCT) are nerve sheath neoplasms composed of Schwann cells with granular cytoplasm. Perineurial cells are the cellular component of the perineurium and of perineuriomas, neoplasms supposedly derived from perineurial cells. However, perineurial cells have also been found in other Schwann cell-derived tumors. These cells have not been well studied in GCTs. We studied the presence of perineurial cells in a series of 24 GCTs with EMA, claudin-1, and Glut-1, which are immunohistochemical markers for perineurial cells. Three cases lacked nerve fascicles. Three cases showed no pe

Microscopic benign and invasive malignant neoplasms and a cancer-prone phenotype in prophylactic oophorectomies.

BACKGROUND: The occurrence of approximately 5% of common epithelial malignant tumors of the ovary can be traced to inheritance of risk. One prophylactic strategy to decrease the probability of development of disease in individuals within families where this mendelian-dominant pattern of occurrence is apparent is to remove the ovaries of individuals at risk for ovarian cancer. The procedure, when done for this purpose, is recommended soon after completion of childbearing. PURPOSE: Our goal was to compare the histologic features of the ovaries of women at increased risk for ovarian cancer to th

Pancreatic intraductal papillary mucinous neoplasm with invasive carcinoma and uterine metastasis: a case report.

Intraductal papillary mucinous neoplasm (IPMN) with invasive carcinoma is a rare type of pancreatic cancer that has a better prognosis than classic pancreatic infiltrating ductal carcinoma. Most distant metastases occur in the lymph nodes, lungs, liver, and bones at advanced stages. We report a rare case of an IPMN with invasive carcinoma that metastasized to the uterus, resulting in long-term survival after debulking surgery combined with hyperthermic intraperitoneal chemotherapy on a systemic treatment basis. This rare case highlights the need for oncologists and gynecologists to be vigilant

Non-invasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP) in pediatrics.

INTRODUCTION: Non-invasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP) has a low malignancy potential. First acknowledged by the WHO in 2017, this tumor is rare in the pediatric population, with less than 25 cases reported. Accurate diagnosis is key to avoid overtreatment and complications. CLINICAL OBSERVATION: Two cases of adolescents with solid thyroid nodules assessed by fine-needle aspiration biopsy are presented. Hemithyroidectomy was decided upon. Both surgical specimens were reported as NIFTP. Both cases were presented before a cross-disciplinary board, an

Preoperatively diagnosed intraductal oncocytic papillary neoplasm of the pancreas with prominent invasion: a case report.

Previously considered as one of the less-invasive subtypes of intraductal papillary mucinous neoplasm, intraductal oncocytic papillary neoplasm (IOPN) has recently been acknowledged as a new entity of pancreatic tumor. We herein present a case of preoperatively diagnosable IOPN invasion in the stomach and colon. A 78-year-old woman was referred to our hospital for evaluation of anorexia and gastroesophageal reflux. Upper gastrointestinal endoscopy revealed a gastric subepithelial lesion with ulcerated mucosa that required hemostasis. Computed tomography revealed a 96-mm-diameter solid tumor wi

Surgical approach to solid pseudopapillary neoplasms of the proximal pancreas: minimally invasive vs. open.

BACKGROUND: Solid pseudopapillary neoplasms (SPN) of the pancreas are rare pancreatic neoplasms where complete resection is the cornerstone in management. It has been demonstrated in previous studies that minimally invasive surgical approaches are effective management options in treating SPNs of the distal pancreas. The purpose of this study is to evaluate the feasibility of minimally invasive surgery in treating SPNs of the uncinate, head, and neck of the pancreas. METHODS: Data from 2005 to 2017 at Severance Hospital of the Yonsei University Health systems in Seoul, South Korea, were retros

Comparison of metastatic neuroendocrine neoplasms to the breast and primary invasive mammary carcinomas with neuroendocrine differentiation.

Metastatic neuroendocrine neoplasms to the breast may show considerable morphologic overlap with primary mammary carcinomas, particularly those showing evidence of neuroendocrine differentiation, and may be misdiagnosed as such. Accurate distinction between these two entities is crucial for determination of appropriate clinical management. The histologic and immunohistochemical features of metastatic neuroendocrine neoplasms to the breast were studied and compared with the features of primary invasive mammary carcinomas with neuroendocrine differentiation, which served as controls. Of the meta

Retrospective comparison of robot-assisted minimally invasive versus open pancreaticoduodenectomy for periampullary neoplasms.

BACKGROUND: As with other open procedures now routinely performed using laparoscopy, minimally invasive pancreaticoduodenectomy (MIPD) may result in decreased pain, fewer wound complications, and accelerated recovery. However, when used for periampullary cancers, it is also important to assess if MIPD offers comparable oncologic outcomes. METHODS: Technical and perioperative outcomes were compared between patients with a preoperative diagnosis of periampullary neoplasm offered MIPD or open pancreaticoduodenectomy (OPD) from November 2009 to July 2011. RESULTS: Fifty-six consecutive MIPD and

[Evaluation of different technics of uterine evacuation as a risk factor for invasive and metastatic trophoblastic neoplasms].

A study was conducted to determine the effect of the form of uterine evacuation (curettage or vacuum aspiration) and of the use of oxytocin on the incidence of invasive/metastatic gestational trophoblastic neoplasia among patients with a diagnosis of molar abortion. The study was conducted on 42 patients with a histopathological diagnosis of benign complete hydatidiform mole and with a uterine height of more than 12cm. Twenty-five patients were submitted to uterine evacuation by curettage and 17 to uterine evacuation by vacuum aspiration. Twenty-seven of the same 42 patients received oxytocin

Recent Advances in Non-Invasive Blood Markers for Intraductal Papillary Mucinous Neoplasm Grading.

Intraductal Papillary Mucinous Neoplasm (IPMN) is a major precancerous lesion of pancreatic ductal adenocarcinoma. Accurate risk stratification of IPMN is key to preventing and controlling pancreatic cancer. At present, clinicians mainly grade IPMN following the Kyoto consensus guidelines, together with imaging examinations and traditional serum biomarkers like CA19-9 and CEA. This review unveils the latest research progress of non-invasive blood biomarkers for the grading of IPMN, including the diagnostic performance, molecular mechanisms, and current clinical translation of several types of

Non-Invasive Stratification of Portal Hypertension in Patients With BCR::ABL1-Negative Myeloproliferative Neoplasms.

BACKGROUND & AIMS: The course of BCR::ABL1-negative myeloproliferative neoplasms (MPN) is frequently complicated by thromboembolic events in the splanchnic venous system, resulting in portal hypertension (PH). Therefore, the introduction of spleen stiffness measurement (SSM) might improve the diagnosis of PH. The aim of this study was to evaluate the clinical utility of SSM (performed by using the 100 Hz probe) in non-invasive stratification of PH in these patients. METHODS: We performed a retrospective, monocentric, cross-sectional analysis including consecutive patients with BCR::ABL1-negat
