Cutaneous Malignant Neoplasms in Hematopoietic Cell Transplant Recipients: A Systematic Review.

IMPORTANCE: Hematopoietic cell transplantation has increased the survival of patients with several types of malignant hematologic disease and hematologic disorders; however, these patients have an increased risk of posttransplant cutaneous malignant neoplasms. Physicians should be aware of associated risk factors to provide appropriate patient screening and long-term care. OBJECTIVE: To identify the incidence and risk factors for cutaneous malignant neoplasms following hematopoietic cell transplantation. EVIDENCE REVIEW: A systematic review was conducted using Medline and Cochrane databases

Allogeneic hematopoietic cell transplantation in adult patients with myelodysplastic/myeloproliferative neoplasms.

BACKGROUND: In adults, the 2 main types of myelodysplastic/myeloproliferative neoplasms (MDS/MPN) are chronic myelomonocytic leukemia (CMML) and atypical chronic myeloid leukemia (aCML). Both are associated with a poor prognosis. Allogeneic hematopoietic cell transplantation (HCT) is the only known curative treatment modality for these diseases, but data on outcomes following such treatment are limited. We analyzed the outcomes of patients with MDS/MPN after allogeneic HCT. METHODS: This retrospective study included 10 patients with MDS/MPN who received allogeneic HCT at Asan Medical Center f

Hematopoietic stem cell transplantation for natural killer-cell lineage neoplasms.

Neoplasms of natural killer (NK)-lineage are rare. Their prognosis is generally poor except for cases of solitary nasal NK-cell lymphoma. The NK-cell Tumor Study Group performed a survey in Japan on patients diagnosed between 1994 and 1998. Of 228 patients selected for analysis, 40 underwent HSCT (15 allografts and 25 autografts). The underlying diseases were myeloid/NK cell precursor acute leukemia (n = 4), blastic NK-cell lymphoma (n = 11), aggressive NK-cell leukemia (n = 3), and nasal-type extranodal NK-cell lymphoma (n = 22). At the time of HSCT, 22 patients were in complete remission (CR

Prolonged, transplant-free remission with tagraxofusp in combination with venetoclax for transplant-ineligible blastic plasmacytoid dendritic cell neoplasm.

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare, aggressive hematologic malignancy predominantly affecting older adults. While the CD123-directed agent tagraxofusp has improved outcomes, durable long-term survival is typically limited to patients eligible for transplant, leaving effective transplant-free treatment strategies as an urgent unmet need. Here, we report the case of an 84-year-old woman with concurrent Rai stage 0 chronic lymphocytic leukemia (CLL) who presented with skin-limited BPDCN and was treated with tagraxofusp and venetoclax combination therapy. The patient ac

Subsequent Neoplasms After Umbilical Cord Blood Transplantation in the Japanese and European Populations.

BACKGROUND: Subsequent neoplasms (SN) are serious late complications after umbilical cord blood transplantation (UCBT). However, population-based comparisons of the incidence and outcomes of SN between Japan and Europe are lacking. OBJECTIVES: To compare the incidence, types, and outcomes of subsequent neoplasms after unrelated UCBT between Japan and Europe. METHODS: We conducted a retrospective registry-based study using data from the Japanese Society for Transplantation and Cellular Therapy/Japanese Data Center for Hematopoietic Cell Transplantation and Eurocord/European Society for Blood

Risk Stratification Models for Cutaneous Malignant Neoplasms After Blood or Marrow Transplant.

IMPORTANCE: Blood or marrow transplant (BMT) recipients are at increased risk of developing subsequent cutaneous malignant neoplasms. There is a need to develop risk stratification tools to identify those at highest risk to inform dermatologic surveillance and evaluate risk mitigation strategies. OBJECTIVE: To develop and validate a risk stratification tool for subsequent cutaneous malignant neoplasms among BMT recipients that incorporates readily available patient and treatment information. DESIGN, SETTING, AND PARTICIPANTS: This prognostic study included participants in the multi-instituti

Clinicopathologic analysis of malignant or premalignant cutaneous neoplasms in Japanese kidney transplant recipients.

It is well known that recipients of kidney transplants are at an increased risk of developing malignant or premalignant cutaneous neoplasms (MPCNs) after transplantation. However, the pathogenesis of MPCNs after kidney transplant has not been well-studied in Asian populations. This study aimed to describe the clinicopathologiccharacteristics of MPCNs in an Asian population. We retrospectively reviewed the medical records of 1956 patients who received kidney transplants at two hospitals in Japan, between 2003 and 2019. Among these patients, 24 developed 50 MPCN lesions, including 14 squamous ce

Secondary Neoplasms of the Female Lower Genital Tract After Hematopoietic Cell Transplantation.

Hematopoietic cell transplantation (HCT) results in long-term survival (≥10 years) in 85% of patients who survive transplant-related complications within the first 2 years posttransplant. Transplant survivors, however, are at an increased risk of chronic health conditions compared with the general population, including the emergence of secondary malignant neoplasms. In particular, female transplant survivors may face a greater risk of lower genital tract (cervical, vulvar, or vaginal) neoplasms due to chronic immune dysregulation in the peritransplant and posttransplant environment. Persistent

Myeloid Neoplasms Following Solid Organ Transplantation: Clinicopathologic Studies of 23 Cases.

OBJECTIVES: Myeloid neoplasms (MNs) after solid organ transplant are rare, and their clinicopathologic features have not been well characterized. METHODS: We retrospectively analyzed 23 such cases. RESULTS: The ages ranged from 2 to 76 years, with a median of 59 years at the diagnosis. The median interval between the transplant and diagnosis was 56 months (range, 8-384 months). The transplanted organs included liver in five, kidney in six, lung in five, heart in six, and heart/lung in one case(s). The types of MN included acute myeloid leukemia (AML) in 12, myelodysplastic syndrome (MDS) in

Generation of a novel, multi-stage, progressive, and transplantable model of plasma cell neoplasms.

Multiple myeloma is a plasma cell neoplasm with an extremely variable clinical course. Animal models are needed to better understand its pathophysiology and for preclinical testing of potential therapeutic agents. Hematopoietic cells expressing the hypermorphic Rad50(s) allele show hematopoietic failure, which can be mitigated by the lack of a transcription factor, Mef/Elf4. However, we find that 70% of Mef(-/-)Rad50(s/s) mice die from multiple myeloma or other plasma cell neoplasms. These mice initially show an abnormal plasma cell proliferation and monoclonal protein production, and then dev

Hair Transplantation on Free Flap in a Patient with Malignant Scalp Neoplasms: A Case Report.

This case report describes a pioneering approach to address severe alopecia following free flap reconstruction for malignant scalp tumor resection. A 41-year-old female with recurrent malignant fibrous histiocytoma underwent extensive scalp resection (10 cm × 21 cm defect) repaired with a free anterolateral thigh flap. While the flap healed successfully, the resulting baldness caused significant psychological distress, leading her to decline conventional scalp expansion due to its invasiveness. Instead, we performed staged autologous hair transplantation directly onto the flap-a technically ch

Outcomes of Allogeneic Hematopoietic Stem Cell Transplantation for Myelodysplastic/Myeloproliferative Overlap Neoplasms.

Myelodysplastic/myeloproliferative overlap neoplasms (MDS/MPN) are rare hematological malignancies. We analyzed the outcomes of 75 patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT) for MDS/MPN. Graft-versus-host disease (GvHD) prophylaxis included post-transplantation cyclophosphamide (PTCy) in 71% of patients, with 44 (59%) receiving a combination of anti-thymocyte globulin (ATG) and PTCy. The median follow-up was 44.4 months. Primary graft failure occurred in three patients (4%). The incidence of grade III-IV acute GvHD at day 100 was 13% (95% CI: 6-22). At 2 year

Progression of Incidental Intraductal Papillary Mucinous Neoplasms of the Pancreas in Liver Transplant Recipients.

OBJECTIVES: Intraductal papillary mucinous neoplasms (IPMNs) are premalignant pancreatic cysts commonly found incidentally. Immunosuppression accelerates carcinogenesis. Thus, we aimed to compare IPMN progression in liver transplant (LT) recipients on chronic immunosuppression to progression among an immunocompetent population. METHODS: We retrospectively assessed adult LT recipients between 2008 and 2014 for imaging evidence of IPMN. Diagnosis of IPMN was based on history, imaging, and cyst fluid analysis. The immunocompetent control group consisted of nontransplant patients from our pancreat

Increased risk for lymphoid and myeloid neoplasms in elderly solid-organ transplant recipients.

BackgroundBy assessing the spectrum of hematologic malignancies associated with solid-organ transplantation in the elderly, we provide information on the pathogenesis of lymphoid and myeloid neoplasms and the clinical manifestations of immunosuppression.MethodsUsing data from the U.S. Surveillance, Epidemiology, and End Results Medicare database, we identified 83,016 cases with a hematologic malignancy (age 66-99 years) and 166,057 population-based controls matched to cases by age, sex, and calendar year. Medicare claims were used to identify a history of solid-organ transplantation. We used p

Allogeneic Transplantation in the Rare Disease Myelodysplastic Syndrome/Myeloproliferative Neoplasm with Neutrophilia: Age and Disease Burden Determine Outcome.

Myelodysplastic syndrome/myeloproliferative neoplasm (MDS/MPN) with neutrophilia, as termed by the World Health Organization (WHO) in 2022 [1], formerly known as atypical chronic myeloid leukemia (aCML) [2], is a rare hematologic malignancy with a poor prognosis. Allogeneic hematopoietic stem cell transplantation (allo-HCT) is the only curative treatment, but prognostic factors for post-transplantation outcomes are poorly defined. This retrospective multicenter analysis included 85 patients receiving allografts over a 30-year period at 8 German centers, representing the largest cohort reported
