The Role of Nephrologists in the Management of Methanol Poisoning.

Acute methanol poisoning is a rare but serious condition that can lead to significant morbidity and mortality. Toxic metabolites produced by methanol, primarily formaldehyde, can cause high anion gap metabolic acidosis, with the severity of clinical presentation ranging from mild symptoms to multi-organ failure. Nine people died and four patients needed treatment at our university hospital following a collective intoxication caused by the consumption of homemade alcoholic beverages in the central region of Morocco. The four patients presented to the emergency department with varying clinical s

[A brief update on FGF23 for the clinical nephrologist].

Fibroblast growth factor-23 (FGF23) is a hormone that controls homeostasis of calcium and phosphate metabolism in health and disease. Unlike most other hormones, FGF23 rises exponentially with declining kidney function, reaching extreme elevations in many end-stage renal disease patients. Clinical and experimental data further suggest that FGF23 is a very early biomarker of kidney injury, and its predictive value of adverse clinical outcomes has been confirmed across the entire spectrum of chronic kidney disease, including pre-dialysis and dialysis populations as well as renal transplant recip

Rheumatological diseases and kidneys: a nephrologist's perspective.

Renal involvement is a common occurrence in subjects with rheumatological diseases and can develop either due to the disease itself or secondary to drugs used in the treatment. The prevalence of renal involvement and its severity depends on the underlying disease as well as aggressiveness of the therapy. For most rheumatological diseases, renal involvement heralds a poor prognosis and warrants aggressive immunosuppressive treatment. Thus, it is important to diagnose and manage them at an early stage. On the other hand, patients with primary kidney disease can also develop rheumatological manif

Central vein stenosis: a nephrologist's perspective.

Central vein stenosis is commonly associated with placement of central venous catheters and devices. Central vein stenosis can jeopardize the future of arteriovenous fistula and arteriovenous graft in the ipsilateral extremity. Occurrence of central vein stenosis in association with indwelling intravascular devices including short-term, small-diameter catheters such as peripherally inserted central catheters, long-term hemodialysis catheters, as well as pacemaker wires, has been recognized for over two decades. Placement of multiple catheters, longer duration, location in subclavian vein, and

[Nephrotoxicity and fluoride from the viewpoint of the nephrologist].

Fluoride released from methoxyflurane (MOF) during its hepatic and extrahepatic metabolism has been regarded as the major culprit responsible for MOF-induced nephrotoxicity. In the isolated, perfused rat kidney model, admixture of 1500 mumol/l fluoride to the perfusate resulted in tubular and glomerular damage with concomitant anuria. Fluoride administration in Fischer 344 rats in vivo elicited a renal diabetes insipidus-like syndrome that had also been observed in patients after MOF anaesthesia. The renal concentrating defect is most probably due to both dissipation of the corticomedullary os

Permanent vascular access: a nephrologist's view.

Vascular access complications are the greatest cause of morbidity in hemodialysis patients in the United States. Although arteriovenous fistulas have been recommended as the preferred mode of vascular access, recent data indicate that the majority of patients on hemodialysis in the United States have prosthetic graft fistulas. The most frequent complications of prosthetic graft fistulas are thrombosis and stenosis. Hospitalization rates for fistula complications are higher in patients with diabetes mellitus and of black race. Pathogenesis of intimal hyperplasia may include elaboration of plate

[Hypertension from the nephrologist's point of view].

Secondary hypertension represents about 5-10% of all forms of hypertension, renal and renovascular being the commonest forms. Renal artery stenosis is the principal cause of renovascular hypertension due to atheromatous disease or fibromuscular dysplasia. Rapid sequence intravenous pyelogram, isotope renogram, captopril test and digital subtraction angiography or conventional arteriography, are the diagnostic procedures in the diagnosis of renal artery atenosis. Hypertension is also very common in parenchymal renal disease, mainly in chronic renal insufficiency. In this condition, the mechanis

Monoclonal gammopathy of clinical significance: a guide for nephrologists.

Monoclonal gammopathy of undetermined significance (MGUS) is commonly encountered in older individuals. MGUS, by definition, is a benign finding where an expanded population of clonal plasma and B-cells produces a monoclonal antibody that is a biologically inactive. Even in patients with overt plasma cell neoplasms such as multiple myeloma, the monoclonal antibody is often a marker of disease burden but not pathogenic except in light chain cast nephropathy. However, there is a growing recognition that, in rare cases, clonal cells and/or monoclonal gammopathy not meeting criteria for hematologi

Attitudes of nephrologists towards assisted home dialysis in Germany.

BACKGROUND: Assisted home dialysis (AHD) is an option to combine the benefits of home dialysis therapy with the needs of dialysis patients who are unable to perform self-treatment at home. While this method is growing in many countries worldwide, no data so far are reported for Germany. METHODS: A survey was designed to identify the barriers to the implementation of AHD with the focus on attitudes and beliefs concerning AHD. The survey was sent to all 2060 members of the Germany Society of Nephrology. RESULTS: The response rate was 14% of nephrologists (n = 286), representing 24% of all Germ

Monoclonal gammopathy of undetermined significance: an update for nephrologists.

Screening for a monoclonal protein is a common part of the assessment of patients presenting with a renal injury. While in the settings of acute kidney injury, chronic kidney disease and proteinuria monoclonal proteins can be associated with significant pathologies such as cast nephropathy, amyloidosis, and light chain deposition disease, they can also be an unrelated finding. The purpose of this review is to provide the nephrologist with an update to the diagnostic assessment and risk stratification of monoclonal proteins to avoid unnecessary investigation and monitoring of those patients wit

The kidney and bone metabolism: Nephrologists' point of view.

The kidney plays an important role in the regulatory system for bone and mineral metabolism. In chronic kidney disease (CKD), various abnormalities, recently named CKD-mineral and bone disorder (CKD-MBD), may develop in this system. The optimal management of CKD-MBD should be achieved without increasing the risk of metastatic calcification, including that of blood vessels. Thus, it is quite important to identify severe cases of hyperparathyroidism refractory to medical therapy. The size of the parathyroid glands, serum levels of fibroblast growth factor (FGF)23, and, possibly, the overproducti

A survey of pediatric nephrologists on adolescent sexual health.

Chronic renal disease often has an adverse effect on adolescent physiologic and psychosocial development. The severity of the disease may necessitate that the pediatric nephrologist be the adolescent's main medical provider and the most available physician to screen for adolescent health risk behaviors. The purpose of this study was to determine pediatric nephrologists' practices of sexual history taking and diagnosis and treatment of sexually transmitted infections in their adolescent patients. A survey was performed on a convenience sample of 66 pediatric nephrologists attending an education

Treatment practices of FSGS among North American pediatric nephrologists.

A survey of North American pediatric nephrologists was conducted to assess the variability in the treatment of primary steroid-resistant focal segmental glomerulosclerosis (FSGS) of native kidneys. The most widely used immunosuppressive drug was cyclosporin A, with 73.9% using it often or sometimes. Only 44.3% used intravenous methylprednisolone combined with an alkylating agent at least sometimes; the use of methylprednisolone without cytotoxic drugs was slightly more common. Prolonged oral steroid therapy (>3 months) was used often or sometimes by 50.3%. The use of angiotensin converting enz

Sodium load of novel antibiotics: considerations for nephrologists and clinicians.

BACKGROUND: Sodium restriction is a cornerstone in managing patients with chronic kidney disease. Parenteral antibiotic therapy may constitute a hidden and iatrogenic source of sodium overload, particularly relevant in hospitalized and volume-sensitive patient populations. METHODS: We conducted a structured review of the sodium content of novel antibiotics in nephrology, based on approved Summary of Product Characteristics (SmPC). Standard dosing regimens were used to estimate daily sodium burden in adult patients. Clinical implications were contextualized for patients with chronic kidney dis

Nephrotoxicity associated with anticancer agents: perspective on onconephrology from nephrologists.

Nephrotoxicity is one of the most important complications in cancer patients. In particular, acute kidney injury (AKI) is known to be associated with discontinuing effective oncological treatments, longer hospitalizations, increased costs, and a higher risk of death. In addition to acute kidney injury, clinical signs associated with nephrotoxicity during treatment with anticancer agents include chronic kidney disease, proteinuria, hypertension, electrolyte abnormalities, and other characteristic manifestations. Many of these signs are caused both by cancer treatment as well as by cancer itself
