Upper ureteral calculi: extraction via percutaneous nephrostomy.

Symptomatic calculi in the upper ureter are usually removed using surgical and endoscopic techniques. Extraction via percutaneous nephrostomy was successful in 35 of 37 patients using sequentially the techniques of retrograde catheter push, basket sheath exchange, and steerable loop-snare. In 12 patients, the calculus was dislodged into the renal pelvis by the retrograde ureteral catheter; extraction was then easily done. Stone basket retrieval was used for an additional 17 patients. A steerable loop-snare was necessary in six other patients when basket retrieval failed.

Renal arterial injury associated with percutaneous nephrostomy.

A case of pseudoaneurysm and subcapsular hematoma as a complication of a percutaneous nephrostomy with unusual clinical findings is presented. Bleeding following a closed nephrostomy must be kept in mind as a potential hazard. Computerized tomography (CT) examination should be performed in such cases using intravenous radiographic contrast material whenever possible to identify the potential bleeding site rapidly.

CT-guided antegrade pyelography and percutaneous nephrostomy.

Percutaneous antegrade pyelography and percutaneous nephrostomy can be easily performed using CT guidance. CT provides cross-sectional images which facilitate accurate needle placement into the renal pelvis. CT is capable of detecting subtle density differences within the tissues so that even renal pelvises which do not contain contrast material and are not dilated can be accurately punctured. In addition to these procedures, CT accurately displays the perinephric space so that assessment of complications is possible. Four of five attempted percutaneous nephrostomies and two antegrade pyelogra

Spondylodiscitis: a rare complication following percutaneous nephrostomy.

Spondylodiscitis is an inflammation of the intervertebral disc and the adjacent vertebral bodies. The spondylodiscitis can not only be a complication of medical interventions such as an operation near spinal column but also urogenital and vascular interventions and intravenous catheter use. A 71-year-old man was admitted to our emergency department with fever and severe abdominal pain. Antibiotic therapy had been performed with intravenous administration of 2 g of ceftriaxone and the patient underwent the placement of a percutaneous nephrostomy according to Seldinger technique. After 1 week, t

Percutaneous nephrostomy with real-time sonographic guidance.

PURPOSE: Percutaneous nephrostomy (PCN) is an effective method for achieving temporary drainage of the obstructed urinary system and is usually performed under fluoroscopic guidance alone or more commonly under combined sonographic and fluoroscopic guidance. We undertook a retrospective analysis of 273 PCNs performed solely under ultrasound (US) guidance with the aim of evaluating the technique and the safety and efficacy of the procedure. MATERIAL AND METHODS: A total of 273 PCN procedures in 267 patients were performed under real-time US guidance using the Seldinger technique. The indicatio

Direct percutaneous transrenal embolization for renal artery injury following percutaneous nephrostomy.

The use is reported of percutaneous transrenal embolization for renal artery injury during percutaneous nephrostomy. If a nephrostomy catheter is seen to transfix the renal pelvis, the possibility of injury to a large renal artery must be considered. The misplaced nephrostomy catheter should be withdrawn over a guidewire. Percutaneous renal artery embolization may enable rapid arrest of life threatening haemorrhage.

Emergent percutaneous nephrostomy for the diagnosis and management of pyonephrosis.

Emergent percutaneous nephrostomy is a potentially lifesaving procedure with a high technical success rate, minimal morbidity, and long safety record that is often used in the setting of an obstructed and infected renal collecting system (i.e., pyonephrosis). This article discusses all aspects of the emergent placement of nephrostomy catheters including indications, techniques, results, and complications. Differences between emergent and nonemergent placement of percutaneous nephrostomy catheters are also addressed.

Percutaneous nephrostomy with extensions of the technique: step by step.

Minimally invasive therapy in the urinary tract begins with renal access by means of percutaneous nephrostomy. Indications for percutaneous nephrostomy include urinary diversion, treatment of nephrolithiasis and complex urinary tract infections, ureteral intervention, and nephroscopy and ureteroscopy. Bleeding complications can be minimized by entering the kidney in a relatively avascular zone created by branching of the renal artery. The specific site of renal entry is dictated by the indication for access with consideration of the anatomic constraints. Successful percutaneous nephrostomy req

Variations of the position of the colon as applied to percutaneous nephrostomy.

The widespread and increasing usage of percutaneous nephrostomy procedures makes an awareness of the anatomical variations of the colon in relation to the kidney essential. Injury to the colon during this procedure is associated with significant morbidity. The aim of this study was to determine the relation of the colon to the insertion line used during a percutaneous nephrostomy and therefore determine whether the colon is at risk of injury. 333 CT-scans taken in a supine position were studied in order to determine the position of both the ascending and descending colon to the percutaneous ne

Use of percutaneous nephrostomy in malignant ureteric obstruction.

Seventeen patients with malignant disease underwent percutaneous nephrostomy to relieve renal failure secondary to ureteric obstruction. Renal function improved in 88%. The median survival time was 18 weeks and 58% left the primary hospital to return home or to a terminal care hospice. Minor complications occurred in 58%. The use of bilateral nephrostomy tubes conferred no greater benefit than a unilateral tube. Intervention in malignant ureteric obstruction may confer some benefit and should not necessarily be viewed with pessimism.

[Personal experience with the use of percutaneous nephrostomy in children].

Over a 3-year period 52 percutaneous nephrostomies (PCN) were performed in 37 children. The youngest patient was 2 months old, the oldest was 16 years. The primary indication was the obstruction of the upper urinary tract; infection in 25 patients and renal failure in 14 patients were evident. By posterolateral approach a sonographically guided antegrade pyelography was first performed with Chiba 22-gauge needle. Under fluoroscopic control a lower polar calix was punctured with 18 G sheathed needle; a guide wire was introduced through the sheet. After dilatation of the tract a pigtail catheter

Percutaneous nephrostomy for palliation of metastatic ureteral obstruction.

Urinary diversion for palliation of metastatic ureteral obstruction has been associated with high rates of morbidity and mortality, especially with open surgical nephrostomies. An evaluation of percutaneous nephrostomy drainage for the palliation of metastatic ureteral obstruction in 27 patients revealed an increase in survival with decreased morbidity, and a follow-up ranging from three to twenty-five months. The mean survival of all patients was 6.63 months (N = 19), with 8 patients still alive. The histology was the main determinant of length of survival, with prostate, rectal, and cervical

Ultrasonically-guided percutaneous nephrostomy. Report of 24 cases.

Ultrasonically-guided percutaneous nephrostomy using the Seldinger technique was attempted on 24 kidneys in 19 patients with obstructive hydronephrosis. Seventeen of the procedures were successful, 4 failed, and in 3 the catheter slipped out of the renal pelvis 1-48 hours after insertion. The procedure is simple and independent of renal function. Postrenal uremia can be relieved, infected hydronephrosis drained prior to elective surgery, and urinary parameters assessed. Injection of contrast material can define pathological anatomy in those cases where excretory urography and retrograde pyelog
