OBJECTIVE: The decision-making in neonatal brachial plexus palsy (NBPP) treatment continues to have many areas in need of clarification. Graft repair was the gold standard until the introduction of nerve transfer strategies. Currently, there is conflicting evidence regarding outcomes in patients with nerve grafts versus nerve transfers in relation to shoulder function. The objective of this study was to further define the outcomes for reconstruction strategies in NBPP with a specific focus on the shoulder.
METHODS: A cohort of patients with NBPP and surgical repairs from a single center were

Postoperative femoral nerve palsy with nerve division is a potentially devastating complication with a poor prognosis, especially when there is a delay in diagnosis. We present a novel 'belt and braces' approach for managing this unusual injury, using a modification of a nerve transfer, which has only been previously reported in the literature three times to our knowledge.

BACKGROUND: Nerve transfers are routinely performed in patients with brachial plexus injuries because these patients have limited alternative solutions secondary to their severe injury with substantial functional limitations5,6. Nerve transfers offer distinct advantages over other surgical options, as they are able not only to bypass the zone of injury but also to decrease regeneration time because of the proximity of the motor end plate to the repair site1,2. It is for this latter reason that a nerve transfer should be considered for an isolated axillary nerve injury, in which a full recovery

Brachial plexus palsy due to traction injury, especially spinal nerve-root avulsion, represents a severe handicap for the patient. Despite recent progress in diagnosis and microsurgical repair, the prognosis in such cases remains unfavorable. Nerve transfer is the only possibility for repair in cases of spinal nerve-root avulsion. This technique was analyzed in 37 patients with 64 reinnervation procedures of the musculocutaneous and/or axillary nerve using upper intercostal, spinal accessory, and regional nerves as donors. The most favorable results, with an 83.8% overall rate of useful functi

Palliative tendon transfer is an integral part of radial nerve palsy treatment. It can be considered in the first weeks when the possibility of nerve repair by direct suture or nerve grafting is not feasible or reasonable. Mostly, it is discussed secondarily when it is too late for nerve surgery and motor recovery cannot be expected, or after failure or incomplete recovery after nerve repair. The goal of tendon transfers is to restore wrist, finger and thumb extension. For wrist extension, the use of pronator teres is well accepted. The best tendon transfer for finger extension is debated. Thi

Abductive ability was restored by means of a modified nerve-muscle pedicle technique, utilizing nerve-muscle multipedicle and multipoint implantation in seventeen dogs. The results showed that the procedure was superior to traditional Tucker's technique in functional recovery, electrophysiological activity and muscle strength which was also demonstrated by histochemical studies. There was little difference between double and three nerve-muscle pedicle transfer. The procedure has been used in three patients with bilateral vocal cord paralysis. Return of spontaneous abduction in the operated sid

The influence of vascularized intercostal nerve detachment on axonal regeneration and skeletal muscle recovery was examined in two experiments using two mongrel dogs. Experiment 1: The influence of detachment of the vascularized intercostal nerve on axonal regeneration and interfascicular blood flow was examined. Detachment of the intercostal nerve without accompanying blood vessels resulted in a marked decrease in blood flow and axonal degeneration at the distal 60% of the total length of the intercostal nerve. In contrast, no significant axonal degeneration was observed in the intercostal ne

BACKGROUND: Injuries to the ulnar nerve at or above proximal forearm level result in poor recovery despite early microsurgical repair, especially concerning the intrinsic motor function of the hand. To augment the numbers of regenerating axons into the targeted muscles, a nerve transfer of the distal branch of the median nerve, the anterior interosseous nerve, to the ulnar motor branch has been described.
METHODS: Two patients with severe atrophy of the intrinsic hand muscles following an initial proximal ulnar nerve repair had surgery with an end-to-side transfer of the anterior interosseous

INTRODUCTION: Advances in the care of soft-tissue tumors, including imaging capabilities and adjuvant radiation therapy, have broadened the indications and opportunities to pursue surgical limb salvage. However, peripheral nerve involvement and femoral nerve resection can still result in devastating functional outcomes. Nerve transfers offer a versatile solution to restore nerve function following tumor resection.
METHODS: Two cases were identified by retrospective review. Patient and disease characteristics were gathered. Preoperative and postoperative motor function were assessed using the

Recovery after an upper extremity peripheral nerve injury varies depending on multiple factors. In patients with poor functional recovery, tendon transfers may be indicated. The decision to perform an early tendon transfer at the time of nerve repair or before expected reinnervation is considered on a case-by-case basis. There are a multitude of potential tendon transfer options, the choices of which depend on remaining function, specific deficits, and surgeon experience and preferences. A thoughtful approach to reconstruction can lead to a substantial functional improvement with minimal donor

This article provides an update of the current strategies of motor and sensory nerve transfers for peripheral nerve lesions of the upper extremity. Indications, techniques, and outcomes are summarized for both well-established transfers used in the management of proximal and brachial plexus injuries as well as those more recently developed for more distal and isolated nerve injuries in the forearm and hand.

The primary objective of this systematic review is to describe the effectiveness of nerve transfers for restoring quadriceps motor function in patients with femoral nerve palsy. MEDLINE, Embase, and CENTRAL were searched from their inception to June 2023 for any English language, primary literature investigating nerve transfers for femoral nerve palsy. Data were extracted for study and intervention characteristics, as well as clinical outcomes, including preoperative and postoperative knee extension strength, electrodiagnostic studies, functional outcomes, adverse events, and donor site morbid

Distal nerve transfer is a refined surgical technique involving the redirection of healthy sacrificable nerves from one part of the body to reinstate function in another area afflicted by paralysis or injury. This approach is particularly valuable when the original nerves are extensively damaged and standard repair methods, such as direct suturing or grafting, may be insufficient. As the nerve coaptation is close to the recipient muscles or skin, distal nerve transfers reduce the time to reinnervation. The harvesting of nerves for transfer should usually result in minimal or no donor morbidity

Paralysis of the suprascapular nerve, in partial injuries of the brachial plexus, most often warrants a nerve transfer. Transfer of the spinal accessory nerve to the suprascapular nerve is performed most often. We propose to directly transfer the nerve of the rhomboid muscles (branch of the dorsal scapular nerve) to the suprascapular nerve in the supraspinatus fossa. This anatomical study included 10 shoulders. Dissection of the suprascapular nerve and the branch of dorsal scapular nerve to rhomboid muscles (rhomboid nerve) was performed through a posterior approach. Once the nerves were freed
