Long-term use of a neural prosthesis in progressive paralysis.

Brain-computer interfaces (BCIs) enable communication with others and allow machines or computers to be controlled in the absence of motor activity. Clinical studies evaluating neural prostheses in amyotrophic lateral sclerosis (ALS) patients have been performed; however, to date, no study has reported that ALS patients who progressed from locked-in syndrome (LIS), which has very limited voluntary movement, to a completely locked-in state (CLIS), characterized by complete loss of voluntary movements, were able to continue controlling neural prostheses. To clarify this, we used a BCI system to

Restoring visual perception using microsystem technologies: engineering and manufacturing perspectives.

Microsystem technologies offer significant advantages in the development of neural prostheses. In the last two decades, it has become feasible to develop intelligent prostheses that are fully implantable into the human body with respect to functionality, complexity, size, weight, and compactness. Design and development enforce collaboration of various disciplines including physicians, engineers, and scientists. The retina implant system can be taken as one sophisticated example of a prosthesis which bypasses neural defects and enables direct electrical stimulation of nerve cells. This micro im

Retinal prostheses in degenerative retinal diseases.

Degenerative retinal diseases may lead to significant loss of vision. Age-related macular degeneration (AMD) and retinitis pigmentosa (RP), which eventually affect the photoreceptors, are the two most common retinal degenerative diseases. Once the photoreceptorcells are lost, there are no known effective therapies for AMD or RP. The concept of retinal prosthesis is to elicit neural activity in the remaining retinal neurons by detecting light and converting it into electrical stimuli using artificial devices. Subretinal, epiretinal, and other retinal prostheses implants are currently designed t

AI-Based Methods and Technologies to Develop Wearable Devices for Prosthetics and Predictions of Degenerative Diseases.

Neurodegenerative diseases, mainly amyotrophic lateral sclerosis, Parkinson, Alzheimer, and rarer diseases, have gained the attention of healthcare service providers due to their impact on the economy of countries where healthcare is a public service. These diseases increase with aging and affect the neuromotor cells and cognitive areas in the brain, causing serious disabilities in people affected by them.Early prediction of these syndromes is the first strategy to be implemented, then the developing of prostheses that rehabilitate motion and the primary cognitive functions. Evaluation of a Portable Bionic Ankle Prosthesis Under Direct Continuous Electromyography Control for Quiet Standing Tasks.

INTRODUCTION: Postural control and balance are necessary for activities of daily living. Passive prostheses that reduce ankle dorsiflexion/plantarflexion control can require different balance strategies compared with able-bodied individuals. Powered prostheses may restore ankle joint control and improve balance compared with passive prostheses. Muscle activity as the prosthetic control input leverages the human neural control system to directly modulate prosthetic dynamics. This study evaluates continuous myoelectric control during quiet standing tasks with an untethered electromechanically ac

Simulating the impact of photoreceptor loss and inner retinal network changes on electrical activity of the retina.

Objective.A major reason for poor visual outcomes provided by existing retinal prostheses is the limited knowledge of the impact of photoreceptor loss on retinal remodelling and its subsequent impact on neural responses to electrical stimulation. Computational network models of the neural retina assist in the understanding of normal retinal function but can be also useful for investigating diseased retinal responses to electrical stimulation.Approach.We developed and validated a biophysically detailed discrete neuronal network model of the retina in the software package NEURON. The model inclu

Positioning the Nerve Cuff Distally on the Sciatic Nerve Improves the Classification of Ankle-Movement Proprioceptive ENG Signals.

Recording of neural signals from intact peripheral nerves in patients with spinal cord injury or stroke survivors offers the possibility for the development of closed-loop sensorimotor prostheses. However, questions remain over the positioning of neural interfaces such that the separability of neural data recorded from the peripheral nerves is improved. Afferent electroneurographic signals were recorded with nerve cuffs placed on the sciatic nerve of rats in response to various mechanical stimuli to the hindpaw. The mean absolute value of the signal was extracted and fed into classifiers. The

Bionic vision technologies: progress and perspectives on retinal prostheses and optogenetics for the treatment of advanced retinal degeneration.

Advanced degenerative retinal diseases [e.g., retinitis pigmentosa (RP) and age-related macular degeneration (AMD)] are characterized by irreversible photoreceptor loss, and conventional interventions rarely prevent progression to profound vision loss at advanced stages. Retinal prostheses and optogenetic therapies are two leading bionic strategies to bypass absent photoreceptors, yet both are limited by a shared "encoding-biointerface" bottleneck: converting external scenes into interpretable neural activity in surviving retinal circuits with adequate selectivity, safety margins, and long-ter

Extra-neural signals from severed nerves enable intrinsic hand movements in transhumeral amputations.

Robotic prostheses controlled by myoelectric signals can restore limited but important hand function in individuals with upper limb amputation. The lack of individual finger control highlights the yet insurmountable gap to fully replacing a biological hand. Implanted electrodes around severed nerves have been used to elicit sensations perceived as arising from the missing limb, but using such extra-neural electrodes to record motor signals that allow for the decoding of phantom movements has remained elusive. Here, we showed the feasibility of using signals from non-penetrating neural electrod

The rubber hand illusion is a fallible method to study ownership of prosthetic limbs.

Enabling sensory feedback in limb prostheses can reverse a damaged body image caused by amputation. The rubber hand illusion (RHI) is a popular paradigm to study ownership of artificial limbs and potentially useful to assess sensory feedback strategies. We investigated the RHI as means to induce ownership of a prosthetic hand by providing congruent visual and tactile stimuli. We elicited tactile sensations via electric stimulation of severed afferent nerve fibres in four participants with transhumeral amputation. Contrary to our expectations, they failed to experience the RHI. The sensations w

Mechanisms of neuro-robotic prosthesis operation in leg amputees.

Above-knee amputees suffer the lack of sensory information, even while using most advanced prostheses. Restoring intraneural sensory feedback results in functional and cognitive benefits. It is unknown how this artificial feedback, restored through a neuro-robotic leg, influences users' sensorimotor strategies and its implications for future wearable robotics. To unveil these mechanisms, we measured gait markers of a sensorized neuroprosthesis in two leg amputees during motor tasks of different difficulty. Novel sensorimotor strategies were intuitively promoted, allowing for a higher walking s

Brain-machine interfaces for real-time speech synthesis.

This paper reports on studies involving brain-machine interfaces (BMIs) that provide near-instantaneous audio feedback from a speech synthesizer to the BMI user. In one study, neural signals recorded by an intracranial electrode implanted in a speech-related region of the left precentral gyrus of a human volunteer suffering from locked-in syndrome were transmitted wirelessly across the scalp and used to drive a formant synthesizer, allowing the user to produce vowels. In a second, pilot study, a neurologically normal user was able to drive the formant synthesizer with imagined movements detect

Activity-dependent recruitment of inhibition and excitation in the awake mammalian cortex during electrical stimulation.

Electrical stimulation is an effective tool for mapping and altering brain connectivity, with applications ranging from treating pharmacology-resistant neurological disorders to providing sensory feedback for neural prostheses. Paramount to the success of these applications is the ability to manipulate electrical currents to precisely control evoked neural activity patterns. However, little is known about stimulation-evoked responses in inhibitory neurons nor how stimulation-evoked activity patterns depend on ongoing neural activity. In this study, we used 2-photon imaging and cell-type specif

Restoring the Sense of Touch: From "Sci-Fi Dream" to Reality.

A range of remarkable prostheses are now available to give function back to people who have had hands, arms, feet, or legs amputated, but for all their capabilities, these devices are missing a critical feature: a real sense of touch. Without it, a patient has no tactile sensory feedback on whether they have stepped off a curb or onto a misplaced child's toy, or are gripping a Styrofoam coffee cup or a toddler's hand too tightly or too loosely. Research projects today, however, are coming closer to restoring the sense of touch, which will help prostheses evolve from amazing tools to true repla

A high-performance keyboard neural prosthesis enabled by task optimization.

Communication neural prostheses are an emerging class of medical devices that aim to restore efficient communication to people suffering from paralysis. These systems rely on an interface with the user, either via the use of a continuously moving cursor (e.g., mouse) or the discrete selection of symbols (e.g., keyboard). In developing these interfaces, many design choices have a significant impact on the performance of the system. The objective of this study was to explore the design choices of a continuously moving cursor neural prosthesis and optimize the interface to maximize information th

Biocompatibility study of three distinct carbon pastes for application as electrode material in neural stimulations and recordings.

Neural interfaces hold great promise for research and treatment of a wide variety of neurological diseases. Medical electrodes are designed to interface with the nervous system and provide control signals for neural prostheses. We fabricated previously a hook-up neural electrode. Here we investigate the in vitro cytotoxicity of three commercial carbon pastes used for printing the conductor tracks of this electrode. At first, the carbon pastes were characterized with respect to their microstructure and chemical composition. SEM images showed a grainy texture that is associated to the carbon/gra

Myoelectric control of a computer animated hand: a new concept based on the combined use of a tree-structured artificial neural network and a data glove.

This paper proposes a new learning set-up in the field of control systems for multifunctional hand prostheses. Two male subjects with a traumatic one-hand amputation performed simultaneous symmetric movements with the healthy and the phantom hand. A data glove on the healthy hand was used as a reference to train the system to perform natural movements. Instead of a physical prosthesis with limited degrees of freedom, a virtual (computer-animated) hand was used as the target tool. Both subjects successfully performed seven different motoric actions with the fingers and wrist. To reduce the trai

An Efficient Framework for Personalizing EMG-Driven Musculoskeletal Models Based on Reinforcement Learning.

This study aimed to develop a novel framework to quickly personalize electromyography (EMG)-driven musculoskeletal models (MMs) as efferent neural interfaces for upper limb prostheses. Our framework adopts a generic upper-limb MM as a baseline and uses an artificial neural network-based policy to fine-tune the model parameters for MM personalization. The policy was trained by reinforcement learning (RL) to heuristically adjust the MM parameters to maximize the accuracy of estimated hand and wrist motions from EMG inputs. Our present framework was compared to the baseline MM and a widely used M

Continuous A-Mode Ultrasound-Based Prediction of Transfemoral Amputee Prosthesis Kinematics Across Different Ambulation Tasks.

OBJECTIVE: Volitional control systems for powered prostheses require the detection of user intent to operate in real life scenarios. Ambulation mode classification has been proposed to address this issue. However, these approaches introduce discrete labels to the otherwise continuous task that is ambulation. An alternative approach is to provide users with direct, voluntary control of the powered prosthesis motion. Surface electromyography (EMG) sensors have been proposed for this task, but poor signal-to-noise ratios and crosstalk from neighboring muscles limit performance. B-mode ultrasound
