[Behcet's disease with neurologic manifestations].

Behcet's disease (BD) is a type of systematic chronic inflammatory disease of unknown etiology that is characterized by the combination of recurrent oral aphthous, genital ulcers and uveitis. A damage of the nervous system is observed in 3-9% of patients, in which two variants of the course of the disease are possible: parenchymal in the form of immune-mediated meningoencephalitis, and non-parenchymal, which can be represented by venous sinus thrombosis, intracranial arterial aneurysms and intracranial hypertension. The article presents a clinical case of BD with a dominant nervous system dama

Neurologic manifestations of Lyme Borreliosis.

In the first part of this review, a summarized description of the typical clinical manifestation as well as less frequent symptoms of early (acute) Lyme Neuroborreliosis (LNB) are presented. In the second part, insights into the controversial topic of "Post-Treatment Lyme Disease Syndrome" are given and the quite rare, but clearly defined picture of late (chronic) LNB is described.

Neurologic Manifestations of Blood Dyscrasias.

Neurologic manifestations are common in blood diseases, and they can be caused by the hematologic disorder or its treatment. This article discusses hematologic diseases in adult patients, and categorizes them into benign and malignant conditions. The more common benign hematologic diseases associated with neurologic manifestations include anemias, particularly caused by B12 deficiency and sickle cell disease, and a variety of disorders of hemostasis causing bleeding or thrombosis, including thrombotic microangiopathy. Malignant conditions like multiple myeloma, leukemias, and lymphomas can hav

Porphyria and its neurologic manifestations.

Porphyrias are rare disorders resulting from a defect in the heme biosynthetic pathway. They can produce significant disease of both the peripheral and central nervous systems, in addition to other organ systems, with acute intermittent porphyria, hereditary coproporphyria, and variegate porphyria as the subtypes associated with neurologic manifestations. The presence of a motor-predominant peripheral neuropathy (axonal predominant), accompanied by gastrointestinal distress and neuropsychiatric manifestations, should be a strong clue to the diagnosis of porphyria. Clinical confirmation can be

[Neurologic manifestations in rheumatic diseases].

Among varying organ involvements, neurologic as well as musculoskeletal involvement are important toward the quality of life and prognosis in the patients of rheumatic diseases. In general, these occur as the increment of disease activity and influence the choice of therapeutic regime. In this review, neurologic and musculoskeletal involvements, especially the former, found in systemic lupus erythematosus, dermatomyositis/polymyositis, Sjögren's syndrome, Behçet's disease and vasculitis syndrome are discussed.

Cardiac manifestations of neurologic disorders.

The cardiac complications of certain neurologic diseases have been well recognized for over 50 years and are mostly evident for cerebrovascular accidents. Although these complications are frequent and in most circumstances benign, detrimental cardiac side effects, such as serious arrhythmias and myocardial infarctions, may occur. The link to most of these cardiac derangements is a transient or chronic autonomic dysfunction, depending on the specific neurologic disease. Myocardial infarcts, left ventricular dysfunction, and arrhythmias are well-recognized complications of subarachnoid hemorrhag

Neurologic manifestations of Chagas disease.

Chagas disease is endemic in Latin America and has become an emerging problem in developed countries because of international migrations. The protozoan Trypanosoma cruzi is the etiologic agent and the disease is divided into two phases. The acute phase is mostly asymptomatic or presents with unspecific symptoms. Rarely, a severe and often fatal form occurs in immunosuppressed patients or infants, characterized by meningoencephalitis (sometimes including brain tumor-like lesions) and myocarditis. The chronic phase consists of an indeterminate, asymptomatic form followed by digestive, cardiac, o

Remote neurologic manifestations of cancer.

Paraneoplastic disorders may affect any part of the central or peripheral nervous systems. Although relatively uncommon, these disorders are a significant cause of neurologic morbidity for cancer patients. At least some paraneoplastic syndromes are believed to be caused by an autoimmune reaction against shared tumor-neural antigens. This article summarizes the clinical features of paraneoplastic disorders, the current evidence for autoimmunity, and guidelines for diagnosis and treatment.

[Aids neurologic manifestations in childhood].

INTRODUCTION: Children with AIDS get infected mainly by vertical transmission. DEVELOPMENT: That was what happened in 90% of the cases in a series of 340 HIV+ children followed at Hospital de Clínicas de Porto Alegre, Brazil. Currently, after the use of prophylactic treatment during pregnancy and the six first weeks of life, our transmission rate is 3% to 4%. The incidence of neurologic complications in the vertical transmission group was 49% in our prospective series, and due either to immunosuppression or primary neurological disease. Neurologic changes secondary to HIV constitute a complex

[Neurologic manifestations of Whipple disease].

Pure neurologic Whipple's disease (WD) may be suspected by same clinical data (dementia-ophthalmoplegia-myoclonus triad, oculomasticatory myorhythmia) with support of MRI. Diagnosis is confirmed by intestinal and/or brain biopsy. Early recognition is critical in a disease that can lead to irreversible neurologic sequelae and that can potentially be cured. Despite therapy, relapses in patients with WD are common, being neurologic recurrence the most frequent and serious. Antibiotics that do not cross the blood-brain barrier are not adequate initial therapy for WD, because they predispose to neu

Neurologic manifestations of systemic vasculitides.

Involvement of the nervous system is frequently observed in patients with vasculitis. Peripheral neuropathy is often an early and dominating feature of the disease process. Cranial nerve palsy, central nervous system vasculitis, and inflammatory myopathy are other neurologic manifestations.

[Neurologic manifestations in Wolfram's syndrome].

Wolfram's syndrome, also known as DIDMOAD syndrome, includes juvenile diabetes mellitus and optic atrophy variously associated with diabetes insipidus and deafness. We describe the neurological findings in 5 patients with Wolfram's syndrome. All patients had a neurological examination and were subjected electrophysiological and brain imaging including CT scan and, in one patient, MRI. There were two pairs of brothers and a sporadic case with paternal consanguinity suggesting recessive inheritance. Neurological abnormalities were found in four patients including dysarthria, seizures, anosmia, n

Neurologic manifestations of hypothalamic disease.

The hypothalamus, in addition to regulating the anterior and posterior pituitary, controls water balance through thirst, regulates food ingestion and body temperature, influences consciousness, sleep, emotion and other behaviors. Much has been learned of these effects in human disease through the clinical manifestations that occur with hypothalamic lesions. This study reviews the clinical pathologic correlations that have been made in recent years showing that regions of the hypothalamus exert functions in humans that are similar to those identified in experimental animals. Clinical pathologic

[Neurologic manifestations of infectious endocarditis].

Thirty out of 287 patients (10.4%) admitted to hospital for infective endocarditis between December 1970 and January 1990 had neurological complications. Twenty-three patients had native valve infectious endocarditis and 7 had prosthetic valve endocarditis. The clinical features were characterized by the frequency of aortic valve involvement (23 out of 30) and other complications, especially cardiac failure (16 cases) and peripheral vascular manifestations (7 cases). The commonest organism was the staphylococcus (53% of identified organisms) but the number of negative blood cultures was high (

[Neurologic manifestations in Ollier's disease].

The authors describe a case of Ollier's syndrome which developed in a 31-year-old female patient. The patient manifested hypertrophy of osseous and soft tissues, deformity of the dorsum sellae and of the walls of the cavernous sinus and siphon of the internal carotid artery. In the clinical picture of the disease, one could see symptoms of the impairment of pairs III and VI of the craniocerebral nerves, pains due to the irritation of the first branch of nerve V.

Neurologic manifestations of child abuse.

During a four-year period, we saw 23 cases of child abuse with central nervous system involvement. Of these, five died and four were left with a substantial neurologic deficit as a result of injury. Of the 18 survivors, eight (44%) were removed from their home along with their siblings, one was placed in a sheltered environment with its mother, and nine (50%) were kept under surveillance. Six of the 12 patients with CT evidence of intracranial damage had no or minimal evidence of external cranial trauma. Although shaking as the major etiologic factor in neurologic impairment from child abuse h

Neurologic manifestations of pulmonary disease.

Respiratory insufficiency of any cause has significant effects on the nervous system. Headache, mental status changes, papilledema, and numerous motor abnormalities including asterixis are commonly seen. Abnormalities in ventilation and gas exchange result in hypoxia, hypercapnia, and respiratory acidosis, and these, in turn, interfere with cerebral metabolism, increase CBF, and may raise intracranial pressure. Chronic respiratory insufficiency can persist for many months with minimal neurologic symptoms, as numerous compensatory mechanisms, particularly renal, may take effect. Treatment inclu
