[Constraint-induced movement therapy in neurological rehabilitation: which modality to choose? Our experience in rehabilitation in Geneva].

OBJECTIVE: The purpose of constraint-induced movement therapy (CIMT) is to stimulate the use of the affected upper arm following stroke and to improve the integration of the arm in activities of daily living. CIMT consists of restraining the unaffected arm to limit its use, combined with a training program based on the shaping principle. We aimed to explore how this method can be used in practice by studying 4 patients after stroke. METHOD: We investigated 4 patients, 2 with chronic disability (10 and 17 months after the stroke), and 2 with subacute disability (1 month after the stroke). Pati

Fatigue in Neurological Rehabilitation: Why Brain Diversity and Population Diversity Matter.

BACKGROUND: Fatigue affects 50% to 90% of patients with multiple sclerosis, Parkinson's disease, stroke, ME/CFS, and long COVID and represents a primary barrier to rehabilitation participation yet remains poorly understood despite decades of research. THE PROBLEM: Systematic examination of recent comprehensive reviews reveals a dual diversity failure. Brain diversity is underexamined: fatigue, fundamentally a central nervous system symptom, is assessed predominantly through questionnaires (95%-100% of studies) and peripheral biomarkers, while functional neuroimaging appears in fewer than 15%

Clinical applications and consideration of interventions of electrotherapy for orthopedic and neurological rehabilitation.

Electrotherapy or electrical stimulation (ES) is a part of clinical intervention in the rehabilitation field. With rehabilitation intervention, electrotherapy may be provided as a treatment for pain relief, strengthening, muscle education, wound recovery, or functional training. Although these interventions may not be considered as the primary therapy for patients, the advantages of the ease of operation, lower costs, and lower risks render ES to be applied frequently in clinics. There have also been emerging ES tools for brain modulation in the past decade. ES interventions are not only consi

Management and rehabilitation of neurologic patients with lower urinary tract dysfunction.

Diverse lower urinary tract problems arise in neurologic disease, caused by dysfunctions of the bladder and outlet, both during urine storage and voiding. Most neurologic diseases cause some lower urinary tract dysfunction (LUTD), and the type of dysfunction is related to the location of the nervous system lesion. Clinical evaluation requires identification of risk factors for major morbidity, particularly renal dysfunction, and mechanisms underlying symptoms. A holistic approach is needed to cover influential aspects (e.g., cognitive function, mobility, and urinary tract infections) and relat

Adaptability and flexibility of the human motor system: implications for neurological rehabilitation.

This article stresses the plasticity of the adult sensorimotor cortex in response to various injuries or environmental changes. The dominant role of sensory input is discussed. A number of studies are presented that show how input may lead to learning and change. Learning is discussed in relation to recovery. It is shown how concepts from the field of motor control and learning may be used for improving neurological rehabilitation. Specific attention is given to the variability of input, the meaningfulness of input, and the role of the learning context. The learning context and the application

[Stepwise motor rehabilitation of children with neurologic complications of tuberculous spondylitis].

The proposed protocol for postoperative motor rehabilitation of children with tuberculous spondylitis complicated by severe neurological disorders is based on the procedures developed by the authors to induce spinal locomotor activity. The developed algorithm of motor rehabilitation of spinal patients involves separate restoration of gait components (rhythmic stereotypic walking (spinal locomotion), maintenance of a standing posture and voluntary control over the movements ensuring gait. The stages of motor rehabilitation were defined by taking into account these factors and a child's neurolog

MITEx: A Portable Hand Exoskeleton for Assessment and Treatment in Neurological Rehabilitation.

This work describes the design and preliminary characterization of a novel portable hand exoskeleton for poststroke rehabilitation. The platform actively mobilizes the index-metacarpophalangeal (I-MCP) joint, and it additionally offers individual rigid support to distal degrees of freedom (DoFs) of the index and thumb. The test-bench characterization proves the capability of the device to render torques at the I-MCP level with high fidelity within frequencies of interest for the application (up to 3 Hz). The introduction of a feed-forward friction compensation at the actuator level lowers the

Enhanced Gait Variability Index in Neurological and Geriatric Rehabilitation: A Scoping Review.

BackgroundGait variability is a sensitive marker of neuromotor function and a clinically relevant indicator of fall risk and mobility impairment in neurological and geriatric populations. The Enhanced Gait Variability Index (eGVI) has emerged as a promising composite measure that captures multidimensional gait irregularity in a single interpretable score, offering a practical advantage over single-parameter approaches. Despite growing clinical interest, the evidence base supporting its standardized implementation has not been mapped.ObjectivesTo characterize populations and clinical contexts w

Intensity over duration in neurological rehabilitation: exploring evidence for optimised recovery paradigms.

BACKGROUND: Contemporary stroke rehabilitation protocols traditionally emphasise session frequency and treatment duration over intervention intensity-yet emerging evidence suggests we may be preparing patients for therapeutic marathons when their brains demand neuroplastic sprints. Across neuroscientific, behavioural, and clinical domains, convergent data indicate that repetition density, metabolic load, engagement, and temporal compression-not cumulative minutes-constitute the biologically meaningful drivers of neuroplastic and myoplastic adaptation. OBJECTIVE: This Perspective re-examines c
