Focal contracture following injection of succinylcholine in patients with peripheral nerve injury.

Focal muscle contracture in the limb following sytemic administration of depolarizing neuromuscular blocking agents have been demonstrated experimentally with transection or crush injury of the nerve, but has rarely been observed clinically in patients with partial peripheral nerve injury. Three cases of spastic response in the hand and wrist are described in patients with subclinical chronic, subacute, and acute nerve injury, to document the occurrence of this phenomenon under various circumstances.

Applied pharmacology of common neuromuscular blocking agents in critical care.

While all neuromuscular blocking agents (NMBAs) effectively interrupt neuromuscular transmission, it must be emphasized that these drugs are completely devoid of analgesic, sedative, or amnestic properties. The increasing use of NMBAs in the ICU requires familiarity with their basic pharmacologic properties, as well as an appreciation of potential problems associated with chronic (> 24 hrs) neuromuscular blockade. Although NMBAs possess an impressive safety record, the majority of recommendations for neuromuscular blocker use in the ICU are extrapolated from short-term perioperative studies in

[Mivacurium].

Mivacurium is a short-acting nondepolarizing muscle relaxant (NDPMR) with a benzyl-isoquinoline structure and rapid, spontaneous reversal. It is hydrolyzed by cholinesterase in plasma and its chemical structure favors histamine release, leading to cutaneous or cardiovascular symptoms, particularly when the dose is increased or when the drug is injected rapidly. Both duration of effect and reversal of mivacurium are less dose dependent than they are with intermediate-duration NDPMRs. In adults the recommended dose for intubation (2 to 3 times the ED95) induces clinically effective blockade last

Use and Outcomes of Sugammadex for Neurological Examination after Neuromuscular Blockade in the Emergency Department.

INTRODUCTION: Non-depolarizing agents such as rocuronium and vecuronium are frequently used in the emergency department (ED) to facilitate intubation but may lead to delay in neurologic examination and intervention. Sugammadex is used for reversal of neuromuscular blockade by non-depolarizing agents but its role in the reversal of neuromuscular blockade for neurologic examination in the ED is poorly defined. METHODS: This was a multicenter cohort study using retrospective chart review. We reviewed all ED encounters from June 21, 2016-February 9, 2024 of the electronic health record of Mass Ge

Skeletal Muscle Relaxants and Their Impact on Intracranial Pressure in Neurosurgery.

Skeletal muscle relaxants have their place in everyday use in numerous anesthesiological procedures, such as preparing a patient for surgery, supporting mechanical ventilation, and performing effective intubation. These drugs can be divided, based on their mechanism of action, into depolarizing skeletal relaxants, such as succinylcholine, and non-depolarizing skeletal muscle relaxants. Non-depolarizing agents are further categorized, based on their structure, into steroidal (eg, rocuronium) and benzylisoquinoline (eg, atracurium) compounds. To gain better control over neuromuscular blockade an

Neuromuscular Blockade Monitoring: Having It but Knowing When Not to Trust It.

Butyrylcholinesterase (BChE) is an enzyme involved in the degradation of depolarizing and non-depolarizing neuromuscular blocking agents (NMBA), such as succinylcholine and mivacurium, respectively. Its deficiency is inherited or acquired, and results in paralysis of skeletal muscles after NMBA administration. We report a case of a 32-year-old pregnant woman proposed for cesarean section. General anesthesia (GA) was induced using propofol and succinylcholine. The surgical procedure was uneventful but after 40 minutes, there was no reversal of neuromuscular block (NMB). Other differential diagn

Prolonged Postoperative Paralysis Following Elective Tonsillectomy: A Case of Pseudocholinesterase Deficiency.

Pseudocholinesterase deficiency is a rare disorder of the enzyme butyrylcholinesterase that can lead to delayed emergence following exposure to depolarizing neuromuscular blocking agents. We report the case of a 19-year-old male who underwent elective tonsillectomy and experienced prolonged postoperative paralysis, suspected to be due to pseudocholinesterase deficiency. The patient had no significant past medical history or family history of the condition. This report highlights the clinical implications of pseudocholinesterase deficiency and discusses strategies for perioperative management.

[Guideline on the safe use of neuromuscular blocking agents].

Inappropriate use of muscle relaxants leads to postoperative residual neuromuscular block (PORNB), which is still a significant problem nowadays and is an independent risk factor for postoperative pulmonary complications. To decrease its incidence, the European Society of Anesthesiology and Intensive Care and the American Society of Anesthesiologists published their guidelines on the safe use of muscle relaxants in 2023. In this publication, the Edömér Tassonyi Neuromuscular Task Force of the University of Debrecen Clinical Center summarizes its recommendations. Based on international guidelin

[Pharmacological Treatment of Malignant Hyperthermia: Update 2019].

Malignant hyperthermia (MH) is a pharmacogenetic disorder of skeletal muscle presenting as a hypermetabolic response to potent volatile anesthetics such as sevoflurane, desflurane, isoflurane and the depolarizing muscle relaxant succinylcholine. Following introduction of the hydantoin derivative dantrolene, the previously high mortality of fulminant MH episodes could be reduced from > 80% to less than 10%. For treatment of MH an initial intravenous bolus of 2.5 mg/kg dantrolene based on the actual body weight should be applied. If symptoms are not improving after the initial dose, up to 10 mg/

Effects of some intravenous anaesthetics on the contractile responses produced in the chick biventer cervicis skeletal muscle.

The effects of some intravenous anaesthetics, in low and high concentrations, on the contractile responses produced in the chick biventer cervicis skeletal muscle were studied using electrophysiological and pharmacological techniques. The anaesthetics used were methohexitone sodium, thiopentone, ICI 35 868 (propofol, diprivan), althesin and etomidate. The results showed that all these anaesthetics had a common mode of action at the chick neuromuscular junction. In low concentrations, they increased the amplitude of the indirectly-elicited twitch and tetanic contractions, whereas in high concen

Prolonged paralysis associated with succinylcholine--a case report.

Prolonged paralysis is a rare complication of succinylcholine. We report a case of palmar hyperhidrosis who underwent transthoracic endoscopic sympathetic ganglionectomy under general anesthesia with a facemask developed prolonged paralysis following the operation. The prolonged paralysis supposedly caused by intraoperative use of succinylcholine was recognized and confirmed by laboratory examination which showed low pseudocholinesterase activity. In this report, we describe the course of the event and discuss the causes of delayed awakening associated with anesthesia and its management.

Postintubation Sedation After a Formulary Change From Succinylcholine to Rocuronium in a Critical Care Transport Organization.

OBJECTIVE: Rocuronium is increasingly used as a first-line neuromuscular blocker (NMB) in rapid sequence intubation by transport teams. Prior work has shown that rocuronium is associated with a delay in postintubation sedation compared with intubation with succinylcholine. METHODS: Boston MedFlight is a consortium-based transport organization. In 2017, the intubation protocol and formulary for Boston MedFlight was changed to replace succinylcholine with rocuronium. We performed a retrospective review of patients intubated by the critical care transport teams from January 2017 through December

Traumatic brain injury and RSI is rocuronium or succinylcholine preferred?

PURPOSE OF REVIEW: Traumatic brain injury is widespread and has significant morbidity and mortality. Patients with severe traumatic brain injury often necessitate intubation. The paralytic for rapid sequence induction and intubation for the patient with traumatic brain injury has not been standardized. RECENT FINDINGS: Rapid sequence induction is the standard of care for patients with traumatic brain injury. Historically, succinylcholine has been the agent of choice due to its fast onset and short duration of action, but it has numerous adverse effects such as increased intracranial pressure

Glycopyrrolate and Post-Operative Urinary Retention: A Narrative Review.

The use of non-depolarizing neuromuscular blockade (NDNMB) necessitates the use of reversal agents. Glycopyrrolate, an anticholinergic agent, is commonly used in combination with neostigmine, an anticholinesterase, for the reversal of neuromuscular blockade medications. Glycopyrrolate is known to effect on the genitourinary system adversely with an inhibitory effect on bladder contraction, bladder hypotonia, and increase in the frequency of urinary retention. Many studies analyzing the association between glycopyrrolate and urinary retention are outdated and published over a decade ago. The de
