Chronic unilateral neglect from focal meningoencephalitis lesions in an immune-compromised hemodialysis patient.

Unilateral neglect (UN) is a disorder associated primarily with right-brain damage; it causes individuals to behave as if the contralesional half of their world has become unimportant or has simply ceased to exist. This is the first known case study to describe and measure UN caused by an infectious process, meningoencephalitis. The patient was immune compromised as a result of antirejection drugs following a kidney and pancreas transplant, as well as from a baseline vulnerability common to patients with hemodialysis. She was reassessed serially during hemodialysis treatments over 12 months an

Neuroleptic malignant syndrome: a dangerous complication of neuroleptic therapy.

Neuroleptic malignant syndrome is a potentially fatal syndrome that can occur in patients taking neuroleptics or other psychotropic drugs. It is characterized by muscle rigidity, hyperthermia, altered mentation, autonomic dysfunction, increased CPK and leukocytosis. A primary factor in NMS may be a decrease in functioning of dopaminergic neurons. Treatment usually consists of discontinuation of the neuroleptic drug, drug therapy with bromocriptine and dantrolene and supportive measures. The key to successful medical and nursing management is aggressive supportive care with a focus on preventin

Cranial Nerve Wheel of Competencies.

BACKGROUND: The neuroscience intensive care unit (ICU) staff development committee explored ways to test cranial nerve competencies of staff. With the assistance of the nurse manager, clinical nurse specialist, and education specialist, a game called Cranial Nerve Wheel of Competencies was developed. METHOD: The game tested learners' competency knowledge of cranial nerves. Game participants had the opportunity to attend classes on cranial nerve function and to review written materials prior to the day of testing. RESULTS: Staff evaluated the testing methodology as excellent and preferable to

Using Esophageal Temperature Management to Treat Severe Heat Stroke: A Case Report.

BACKGROUND: Exertional heat stroke (EHS) is defined by a core body temperature that exceeds 40°C with associated central nervous system dysfunction, skeletal muscle injury, and multiple organ damage. The most important initial focus of treatment involves reduction of patient temperature. First approaches to achieve temperature reduction often include ice packs, water blankets, and cold intravenous fluid administration. When these measures fail, more advanced temperature management methods may be deployed but often require surgical expertise. Esophageal temperature management (ETM) has recently

Toward a Common Language for Measuring Patient Mobility in the Hospital: Reliability and Construct Validity of Interprofessional Mobility Measures.

BACKGROUND: The lack of common language among interprofessional inpatient clinical teams is an important barrier to achieving inpatient mobilization. In The Johns Hopkins Hospital, the Activity Measure for Post-Acute Care (AM-PAC) Inpatient Mobility Short Form (IMSF), also called "6-Clicks," and the Johns Hopkins Highest Level of Mobility (JH-HLM) are part of routine clinical practice. The measurement characteristics of these tools when used by both nurses and physical therapists for interprofessional communication or assessment are unknown. OBJECTIVE: The purposes of this study were to evalu

Critical Care Nursing in Moyamoya Disease: Complexities and Considerations.

Moyamoya disease is a rare progressive cerebrovascular disorder involving stenosis of major intracranial arteries and the formation of fragile collateral vessels. Patients face lifelong risks of ischemic and hemorrhagic stroke. Surgical revascularization is the definitive treatment, aimed at restoring cerebral perfusion and reducing neurologic injury. Postoperative care is among the most complex in neuroscience nursing. Critical care nurses play a vital role in managing cerebral perfusion, conducting frequent neuro assessments, regulating blood pressure, and preventing seizures. Success requir

From stigma to support: the mediating role of sympathy between nurses' perceived stigma and helping behavior tendency for alcohol use disorder.

BACKGROUND: The detrimental effect of stigma on healthcare for individuals with alcohol use disorders (AUDs) is well-established, often resulting in social distance and diminished helping behavior tendencies. However, contemporary neuroscience reconceptualizes addiction as a brain disease, potentially altering emotional responses to stigma. This study examines a seemingly paradoxical possibility: that under specific conditions, perceived stigma is primarily associated with sympathy (rather than anger or fear), which in turn is linked to helping behavior tendencies among nurses. METHODS: A cro

Central pontine myelinolysis: a multidisciplinary approach to care.

Central pontine myelinolysis (CPM) is a neurological disorder with a variety of manifestations which may be fatal or result in permanent disabilities. Various factors, most commonly too rapid correction of serum sodium, have been implicated in the development of this osmotic demyelinating syndrome. The patient's survival depends on accurate assessment and prompt intervention by the neuroscience nurse. Optimal outcome depends on collaboration by all members of the health care team.

Lumbar spine: common pathology and interventions.

Lumbar herniated nucleus pulposus (HNP) and lumbar spinal stenosis (LSS) are common spine pathologies with different clinical presentations and interventions. HNP generally has an acute onset often without a precipitating event. Unless there is a significant or emergent neurologic deficit, nonsurgical medical management is warranted for 6 or more weeks after the onset of symptoms. If there is no improvement in 6 weeks, surgical intervention may be indicated. Microdiscectomy is the gold standard treatment for uncomplicated HNP. LSS has an insidious onset. Often, clinical presentation is a long

Posterior reversible encephalopathy syndrome: a case study.

A young woman 4 days postpartum was admitted after experiencing two seizures. Her mentation waxed and waned until, after several hours, staff were unable to arouse her with voice or touch. A computed tomography scan demonstrated considerable white-matter edema. The patient's condition declined to a coma. She remained comatose despite therapeutic interventions to control increased intracranial pressure. To her family, her condition was a source of anguish. To the physicians and nurses, she was a puzzle. The final diagnosis was posterior reversible encephalopathy syndrome, which was related to a

Hyperthermia treatment for malignant brain tumors: nursing management during therapy.

Interstitial hyperthermia therapy, the application of heat to control or destroy tumor tissue, is under investigation as an adjuvant treatment for recurrent intracranial malignancies. A brief review of the biologic effects and detailed information of one technique used to treat brain tumors are presented. Management of patients receiving hyperthermia is both challenging and complex. Neuroscience nurses must become knowledgeable about this form of therapy as it is more widely used in brain tumor management.

Using measures of agreement to develop a taxonomy of passivity in dementia.

A comprehensive taxonomy categorizing passive behaviors in people with dementia was developed and revised through the use of expert raters. The taxonomy was first derived from the synthesis of 15 empirical studies that addressed this phenomenon, then was rated by an expert panel of six nurse-scientists with expertise in dementia and neuroscience research. This article describes the application of two measures of agreement, multiple rater kappa and proportion of agreement for multiple raters, calculated using Stata and SPSS, to evaluate and revise the taxonomy. The method proved useful for esti

Neuronal pathway finding: from neurons to initial neural networks.

Neuronal pathway finding is crucial for structured cellular organization and development of neural circuits within the nervous system. Neuronal pathway finding within the visual system has been extensively studied and therefore is used as a model to review existing knowledge regarding concepts of this developmental process. General principles of neuron pathway finding throughout the nervous system exist. Comprehension of these concepts guides neuroscience nurses in gaining an understanding of the developmental course of action, the implications of different anomalies, as well as the theoretica

[The unconscious in psychoanalysis and psychiatry today].

The psychoanalytic unconscious is a concept with multiple meanings. Its first, positivist interpretation opens the door to a psychological debate with the neurosciences on the nature of the psyche, provided that the differences in methods and languages are respected. The second, hermeneutic interpretation views the unconscious as meaning to be interpreted or a narrative to be coconstructed. The third, which is emphasized here, offers a concrete definition of the unconscious within and through the pragmatics of psychoanalytic communication, in the here and now of the encounter. It is from this

Clinical case study: a 4-year-old boy with posterior fossa syndrome after resection of a medulloblastoma.

Posterior fossa syndrome can occur after neurosurgical resections of childhood posterior fossa tumors, such as medulloblastomas. Posterior fossa syndrome is characterized by transient mutism, emotional lability, cognitive deficits, weakness, ataxia, and cranial nerve dysfunction. Symptoms generally appear 1 to 3 days after surgery and persist for months to years. Neuroscience nurses play an integral role in helping affected children and their families through the diagnosis, treatment, and sequelae of this frightening syndrome.

Acute Flaccid Myelitis.

Acute flaccid myelitis is a poliolike illness affecting mainly children, which seems to occur every 2 years in the late summer and early fall. The 2018 outbreak was alarming to parents and healthcare providers because the etiology is still under investigation and long-term outcomes are unclear. Becoming familiar with what is known about the epidemiology and clinical characteristics of acute flaccid myelitis enables neuroscience nurses to disseminate accurate information and recognize and report suspicious symptoms.

Primary tethered cord syndrome: diagnosis and treatment of an insidious defect.

Failure to recognize the signs and symptoms of tethered cord syndrome in patients with spina bifida occulta can result in tragic consequences. Of patients with tethered spinal cord, 35% have bowel and/or bladder dysfunction. Scoliosis, foot or leg length discrepancies, pes cavus, and varus or valgus deformities also can occur. Early assessment and intervention by the neuroscience nurse who is familiar with these clinical signs can help prevent significant deformities and irreversible neurological deficits.

Essential blepharospasm: nursing update.

Essential blepharospasm is a chronic, potentially disabling disorder for which there is no known cause or cure. The term blepharospasm derives from the Greek word 'blepharon' meaning eyelid and the word 'essential' implies unknown cause. In neurological literature blepharospasm is classed as a focal dystonia. Lack of recognition of the disease may cause patients to consult numerous physicians as well as acupuncturists, chiropractors, faith healers and others in an effort to find a cure to what they are sure is not just a psychological problem.
