Local Drug Delivery for the Treatment of Neurotology Disorders.

Neurotology disorders such as vertigo, tinnitus, and hearing loss affect a significant proportion of the population (estimated 39 million in the United States with moderate to severe symptoms). Yet no pharmacological treatments have been developed, in part due to limitations in effective drug delivery to the anatomically protected inner ear compartment. Intratympanic delivery, a minimally invasive injection performed in the office setting, offers a potential direct route of administration. Currently, off-label use of therapeutics approved to treat disorders via systemic administration are bein

Neurotologic manifestations of Chiari 1 malformation.

It is important for otolaryngologists to recognize Chiari malformations as part of the differential diagnosis of balance disorders because patients may initially be seen with symptoms referable to the vestibular system, including ataxia, nystagmus, or vertigo. The objective of this paper is to review the signs, symptoms, and vestibular test findings of a series of patients with Chiari 1 malformation. Six patients were identified by retrospective chart review with a diagnosis of Chiari malformation. Each patient had a complete otoneurologic examination and vestibular function testing. The resul

Vestibular neuronitis: neurotological findings and progress.

Neurotological follow-up observations were conducted on 38 of 43 cases of vestibular neuronitis treated between 1972 and 1983. The average observation period was 1 year and 3 months (range from 1 month to 9 years and 4 months) and 17 cases were followed for more than 1 year. The tests used for evaluation in this series were the following: spontaneous nystagmus, head positional nystagmus, a caloric test and a computed galvanic body sway test (GBST) conducted by Yamaguchi University. Changes in subjective symptoms are also noted. The main subjective complaint was vertigo (whirling; 93%). The rem

A case of cysticercosis cerebri. Neurotologic implications.

Cysticercosis is an unusual disease entity in the United States. A case is reviewed in which a young Mexican-American female presented to an otologist's office with a headache and vertigo. Her symptoms were a manifestation of a single larval cysticercus cyst which had become lodged in the fourth ventricle resulting in an obstructive hydrocephalus and a fatal course. The epidemiology of this disease traces the life cycle of the pork tapeworm from man, normally the definitive host, to the pig, the intermediate host. It is noteworthy that the disease cysticercosis in humans is a deviation from th

Current trends and applications in endoscopy for otology and neurotology.

There has been a rapid increase in endoscopic ear surgery for the management of middle ear and lateral skull base disease in children and adults over the last decade. In this review paper, we discuss the current trends and applications of the endoscope in the field of otology and neurotology. Advantages of the endoscope include excellent ergonomics, compatibility with pediatric anatomy, and improved access to the middle ear through the external auditory canal. Transcanal endoscopic ear surgery has demonstrated comparable outcomes in the management of cholesteatoma, tympanic membrane perforatio

Clinical applications of frameless stereotactic devices in neurotology: preliminary report.

Conventional stereotactic surgery has been utilized primarily for intracranial neuro-surgical procedures. Despite their utility, head-frame systems are restrictive and often inconvenient, thus they have not proved applicable for neurotologic surgery. Recently frameless stereotactic navigational systems have been developed that employ three-dimensional digitizers to transform the coordinates in surgical space to the corresponding image space, without the employment of head frames. This allows the determination of position of a surgical instrument in real time during surgery, and its display on

Neurotologic Findings of Idiopathic Acute Unilateral Audiovestibulopathy.

BACKGROUND AND PURPOSE: Unlike acute unilateral vestibulopathy or vestibular neuritis (AUVP/VN), of which the diagnosis is made based on well-established neurotologic findings, the diagnosis of idiopathic acute unilateral audiovestibulopathy (iAUAV, also known as labyrinthitis) still requires further elucidation. METHODS: We retrospectively reviewed the medical records of patients with first-onset acute audiovestibulopathy (AAVS) in a referral-based university hospital in South Korea between March 2018 and September 2024. The results of video-oculography, video head-impulse, and other neuroto

Neurotological Findings at a Health Unit for Adults with Cervicalgia.

Introduction The cervical spine is a flexible link between the sensory platform of the skull and torso. The fundamental principle of its operation is due to the balance between muscle strength and flexibility, and any dysfunction of this balance causes neck pain, known as cervicalgia. Objective The objective of this study is to analyze the most prevalent neurotological findings in adults with neck pain. Method A cross-sectional study in which 33 adults from 50 to 83 years of age with neck pain were evaluated and underwent the following procedures: anamnesis, as well as ENT, audiological, and v

Otologic neurotologic manifestations of HIV-related disease.

Human immunodeficiency virus (HIV), the causative agent of acquired immunodeficiency syndrome (AIDS), has reached worldwide epidemic proportions and is increasing. Otologists, neurotologists, and audiologists practicing in metropolitan centers in North America can expect to encounter patients with HIV-related illnesses, including patients with AIDS-related complex (ARC) and AIDS. Five representative cases are presented: chronic otitis media, facial palsy, Gradenigo's syndrome with facial paralysis, otosyphilis, and Kaposi sarcoma of the mastoid. The common link in all cases was HIV infection.

Neurotologic considerations in the treatment of advanced clival tumors.

Head and neck manifestations of advanced clival tumors result from subtle, yet progressive mass effect or direct involvement of multiple cranial nerves. Misinterpretation of clinical or radiographic findings in these patients may result in inappropriate treatment planning, increased patient morbidity, and probable tumor recurrence. Our combined experience in managing 21 patients with advanced clival tumors has shown that preoperative loss of vision, diplopia, or facial hypesthesia suggests superior parasellar disease. Facial twitching or neurotologic symptoms result from posterior central tumo

Neurotologic manifestations of von Hippel Lindau disease.

Von Hippel Lindau disease is a hereditary phakomatosis characterized by congenital angiomatosis of the retina and cerebellum. This autosomal dominant syndrome exhibits variable penetrance and expressivity. Because of the marked clinical variability and sporadic age of onset, members of affected families must be counseled and screening protocols established. It has been recommended that initial screening include appropriate laboratory work, ophthalmologic and neurologic examination, radiologic evaluation of the CNS, and when appropriate, abdominal ultrasound. Follow-up examination should be ann

Neurotological findings in Bell's palsy and Hunt's syndrome.

Neurotological findings were analysed in 23 patients with Bell's palsy and in 25 patients with Hunt's syndrome. The incidence and extent of the auditory and vestibular pathology was high in Hunt's syndrome. Although patients had no subjective symptoms related to the auditory and/or vestibular pathology in the Bell's palsy cases, about one-third of the patients showed abnormal findings upon neurotological examination. Differentiating these two diseases is therefore considered difficult by means of neurotological examination in the same way as by serological testing as has been reported by other
