Childhood parasomnias and psychotic experiences at age 12 years in a United Kingdom birth cohort.

STUDY OBJECTIVES: To examine associations between specific parasomnias and psychotic experiences in childhood. DESIGN: Birth cohort study. Information on the presence of frequent nightmares in children was obtained prospectively from mothers during multiple assessments conducted when children were aged between 2.5 and 9 y. Children were interviewed at age 12 y about nightmares, night terrors, sleepwalking, and psychotic experiences (delusions, hallucinations, and thought interference) occurring in the previous 6 mo. SETTING: Assessments were completed in participants' homes or a University c

Nightmares.

Although sporadic nightmares may be normal responses to stress, chronic nightmares can be troublesome. Nightmare syndrome involves dreams during REM sleep and is associated with abnormal psychologic tests and psychiatric disorders. Night terrors are most common in childhood, occur during deep sleep and rarely reflect underlying pathology. Post-traumatic stress disorder produces recurrent and repetitive nightmares. The onset of nightmares in an adult is often associated with medication or illness.

The parasomnias of childhood.

Parents are rarely questioned in detail about the sleep characteristics of their children except during the infancy period. Somnambulism, commonly known as sleepwalking, affects approximately 15% of young children. Safety is the biggest concern for a family whose child sleepwalks. Other parasomnias include night terrors, nightmares, and sleeptalking. This article explores the incidence and characteristics of parasomnias, reviews the physiology of sleep, and offers pediatric nurse practitioners information for history taking and anticipatory guidance.

Autosomal dominant sleep-related hypermotor epilepsy caused by a previously unreported CHRNA4 variant.

Autosomal dominant sleep-related hypermotor epilepsy (ADSHE) is a rare heritable form of epilepsy. It is characterized by hypermotor seizures occurring mainly during sleep. Seizures are typically abrupt in onset and offset and tend to increase in complexity and duration during the night. ADSHE is inherited in an autosomal dominant manner, and penetrance is estimated to be 70%. We describe two brothers with ADSHE with a previously unreported variant in CHRNA4, and the effect of medical treatment with carbamazepine. We highlight the relevance of genetic testing in patients with atypical and clus

Nocturnal psychophysiological correlates of somatic conditions and sleep disorders.

Modern sleep research studies have provided the practicing physician with considerable new information concerning the basic psychophysiology of sleep, the effects of medical conditions on sleep and the role of maturational and emotional factors in producing certain sleep disorders. Medical and psychiatric disorders, sleep disorders and drug-induced sleep stage alterations are studied in the sleep laboratory using the same techniques developed to analyze sleep patterns in normal subjects. After initial sleep laboratory adaptation, a profile of the sleep characteristics of various clinical condi

Autosomal dominant frontal epilepsy misdiagnosed as sleep disorder.

We describe a distinctive epilepsy syndrome in six families, which is the first partial epilepsy syndrome to follow single gene inheritance. The predominant seizure pattern had frontal lobe seizure semiology with clusters of brief motor attacks occurring in sleep. Onset was usually in childhood, often persisting through adult life. Misdiagnosis as night terrors, nightmares, hysteria, or paroxysmal nocturnal dystonia was common, and the inheritance pattern was often not appreciated. This autosomal dominant epilepsy syndrome is ideal for identification of partial epilepsy genes.

A 7-Year-Old Boy With Sudden Onset of Loud Snoring.

A 7-year-old boy was referred for evaluation of loud nightly snoring. Snoring started suddenly 2 weeks prior to presentation and grew progressively worse. Currently, the parents witnessed breathing pauses and gasping at night. The parents moved the child to a recliner to be able to breathe better, but the snoring and apneas persisted. There was no sleepwalking, night terrors, or nocturnal enuresis. During the day, the boy did not endorse symptoms of excessive sleepiness. The review of systems was negative for fever, weight loss, night sweats, sore throat, dysphagia, nasal congestion, ear pain,

The lasting legacy of Charles Fisher (1908-1988), pioneering sleep laboratory scientist and sleep medicine psychiatrist.

Charles Fisher is a pioneering historical figure in sleep laboratory research and sleep medicine who distinguished himself in nine areas: (1) he first documented nocturnal sleep-onset rapid eye movement (REM) sleep periods in narcoleptic patients; (2) he published the first case of polysomnography (PSG) documented acute REM sleep behavior disorder (RBD) that was triggered by sudden withdrawal from a monoamine oxidase inhibitor in 1978, 8 years before the formal identification of RBD; (3) he worked with Roffwarg and Dement on the early delineation of the ontogeny of the human sleep cycle; (4) h

Nightmares related to fluoxetine treatment.

Sleep disturbances are found in most depressive patients. Serotonin reuptake inhibitors, such as fluoxetine hydrochloride, seem to improve sleep by changing the depressive affect and the underlying biological mechanisms. Insomnia is an occasional adverse effect of the medication, but it was shown that only 2-3% of the patients with fluoxetine-induced insomnia discontinued the drug for this reason. We could not find any report of nightmares or night terrors under fluoxetine treatment. We report on four patients who experienced nightmares on fluoxetine monotherapy.

[Parasomnia].

The parasomnias comprises a group of disorders that intrude into the sleep process and are not primarily disorders of sleep and wake states per se. These disorders are manifestations of central nervous system activation usually transmitted through skeletal muscle or autonomic nervous system channels (ICSD 1990). The parasomnias are divided into four groups: Arousal disorders (sleepwalking, night terrors, and confusionnal arousal), sleep-wake transition disorders (rhythmic movement disorder, sleep starts), REM sleep parasomnias (nightmares, sleep paralysis, hallucinations, and REM sleep behavio

Sleep and temporal lobe epilepsy: a case study with depth electrodes.

We studied a patient with hippocampal epilepsy who had frequent nocturnal and diurnal seizures. Depth electrode recording showed that focal seizure discharges in the right hippocampus were of shorter duration in REM and non-REM sleep. However, awakening, especially from REM sleep or shortly after a REM period, facilitated the occurrence of a generalized seizure. There was no ultradian fluctuation in frequency or duration of seizure during diurnal recording. Night terrors (but not nightmares) disappeared after right temporal lobectomy, suggesting that pavor nocturnus was an ictal manifestation

Type A-B behavior and nightmare types among college students.

To consider the relationships among Type A-B behavior, gender, and specific types of nightmares, 780 university undergraduates were tested with Glass' version of the Jenkins Activity Survey and the Spadafora and Hunt Dream Types Survey which included the critical nightmares (fantastic nightmares, posttraumatic nightmares, and night terrors). Relative to students classified as Type B, those classified as Type A were significantly more likely to report experiencing certain types of nightmares, i.e., fantastic and posttraumatic nightmares. We also observed that women reported significantly greate

"Mater puerorum'. A medieval naming for an enigmatic children's disease.

The pediatric clinical entity called "Mater Puerorum' appears first in the latin translation of Rhazes' "Practica Puerorum' and in his "Continens'. His descriptions of the disease could give some likeness either to a diagnosis of night terrors, or of hyperpyretic convulsions, or of a slight form of epilepsy. Mater Puerorum is afterwards described by most pediatric authors till the Renaissance period without much originality. Rhazes being one of their main sources anyway. Mater Puerorum has been considered by Still and Radbill as a synonym for hysterical fits in children. Going back to the Arab

Effect of exposure to terrorism on sleep-related experiences in Israeli young adults.

General sleep-related experiences (Watson, 2001), including special dream states such as elevated dream recall, nightmares, hypnagogic hallucinations, flying dreams, and waking dreams, have been shown to be associated with life stress and psychopathological distress (Soffer-Dudek & Shahar, 2009). In this study, we attempted to examine these associations within the context of terror-related stress. In the week following operation Oferet Yetzuka in Israel, ninety-one participants filled out questionnaires concerning sleep experiences, psychopathological distress, and degree of exposure to terror

Behavioral and psychological treatments for NREM parasomnias: A systematic review.

BACKGROUND: Non-rapid eye movement (NREM) parasomnias are often benign and transient, requiring no formal treatment. However, parasomnias can also be chronic, disrupt sleep quality, and pose a significant risk of harm to the patient or others. Numerous behavioral strategies have been described for the management of NREM parasomnias, but there have been no published comprehensive reviews. This systematic review was conducted to summarize the range of behavioral and psychological interventions and their efficacy. METHODS: We conducted a systematic search of the literature to identify all report

A case report of sleep terrors exacerbated by cetirizine.

Sleep terrors are a type of sleep disorder that is classified as parasomnias and is more common in children than in adults. Cetirizine is a histamine H1 antagonist that is US Food and Drug Administration approved for the treatment of allergic rhinitis and urticaria and has common adverse effects of drowsiness and headaches. We present a case of an adult man with a history of chronic sleep terror disorder and allergic rhinitis who developed worsening of his sleep terrors after initiation of cetirizine that subsequently resolved after discontinuing cetirizine and starting paroxetine.

Sleep disturbance: The overlooked side after open heart surgery in adults.

BACKGROUND: Sleep is essential for full mental and physical renewal. Cardiac surgery improves the life expectancy and quality. But unfortunately, some patients lacked that merit as they suffered sleep disturbance. We used validated questionnaire applied to 153 cardiac surgery patients to assess the sleep disturbances after surgery. RESULTS: About 70 (45.8%) had valve procedure, 55 (36%) had revascularization, 19 (12.4%) had both procedures and 9 (5.8%) had other procedures. The majority had no post-operative bleeding nor infection. About 78% had sleeping difficulty for one month. About 50% us
