Laboratory diagnosis of mumps in a partially immunized population: The Nova Scotia experience.

BACKGROUND: In 2007, Atlantic Canada experienced a large outbreak of mumps predominately in university students who had received a single dose of measles, mumps and rubella vaccine. The present study describes the performance characteristics of reverse transcriptase polymerase chain reaction (RT-PCR) on buccal and urine specimens and immunoglobulin M (IgM) serology in this partially immune population. METHODS: Patients presenting with symptoms suspicious for mumps had a serum, urine and a buccal swab collected for diagnostic testing. Persons were classified as a 'confirmed' case according to

Injury severity in pediatric all-terrain vehicle-related trauma in Nova Scotia.

BACKGROUND/PURPOSE: In 2004-2005, legislation restricting all-terrain vehicle (ATV) use by children and an extensive social marketing campaign intended to reduce pediatric ATV-related morbidity. The frequency, nature, and severity of pediatric ATV-associated trauma were compared before and after such interventions. METHODS: A retrospective cohort study was performed for all pediatric ATV-related injuries that presented to the provincial level 1 pediatric trauma center from 1998 to 2014. National databases were queried for ATV-related injury hospitalizations (n=258), trauma center emergency de

Childhood exposure to ionizing radiation from computed tomography imaging in Nova Scotia.

BACKGROUND: Examining radiation dose in the paediatric population is particularly important due to the vulnerability of paediatric patients (increased radiosensitive tissues and postexposure life-years) and risk for future radiogenic malignancy. OBJECTIVES: To evaluate trends in paediatric computed tomography (CT) use and ionizing radiation exposure using population-based data from Nova Scotia. METHODS: A retrospective, population-based cohort study of CT use in patients <20 years of age, from January 1, 2004 to December 31, 2011, was performed in Nova Scotia. Nova Scotia Niemann-Pick disease (type D): clinical study of 20 cases.

Patients with Niemann-Pick type D have been traced to a single Acadian ancestor in Nova Scotia. The objective of this study was to describe the clinical course. A cohort of children with Niemann-Pick type D was identified by chart review. Some children were seen and a telephone interview with the remaining parents was conducted. Twenty children with Niemann-Pick type D were identified. The female to male ratio was 2:1. Five children had severe neonatal jaundice. Early milestones were normal in the majority. Neurologic symptoms generally developed between 5 and 10 years of age with a mean age o

Incidence of Q fever: pilot studies in two areas in Nova Scotia.

We carried out two pilot studies to determine the incidence of Q fever in Nova Scotia. The first study involved 199 people from the Annapolis Valley region. The first set of blood samples was collected between May and September and the second three to five months later. Two subjects had titers greater than or equal to 1:256 (by the indirect immunofluorescence test), suggesting recent infection. The second group studied consisted of 95 residents of Baddeck. Nova Scotia. Their first serum samples were collected between March and October 1987 and their second samples 18 months later. Two (2%) had

Human trichinosis: studies on eleven cases affecting two families in Nova Scotia.

A two-family outbreak of 11 cases of trichinosis with one fatality has been described. Efforts were made to obtain an early assessment of the severity of parasitization on the basis of initial clinical appearance, serial electrocardiograms, changes in eosinophil count, serum protein analyses and SGOT levels. The diagnoses were confirmed by muscle biopsy and by Suessenguth-Kline flocculation tests on serum. Factors portending a severe illness included: (a) rapid decline in total eosinophil count, (b) very low levels of serum albumin, (c) elevation of alpha(1)- and alpha(2)-fractions of serum pr

The origins and outcomes of triplet and quadruplet pregnancies in Nova Scotia: 1980 to 2001.

The objective of this study was to determine the rate, origin, maternal and perinatal outcomes, and the associated hospital costs of higher order multiple births in one Canadian province. All higher order multiple pregnancies (triplets and above) in Nova Scotia over a 22-year period (1980 to 2001) were reviewed, and the maternal and perinatal outcomes and hospital costs were compared with singletons and twins in the same hospital population. During the 22-year period, 116,785 infants were delivered, including 3448 twins, 99 triplets, and 16 quadruplets. Of the higher order multiple gestations,

A clinical audit of glottic cancer in Nova Scotia: a paradigm for effectiveness research.

We conducted a clinical audit of the recurrence-free rates and absolute survival of 146 patients who presented with T1-T3 glottic cancer and received primary radiotherapy treatment at the Nova Scotia Cancer Centre between 1984 and 1990. The outcomes are compared with a review of published results from other centres. We used stage T3 glottic cancer as an example to illustrate concepts of effectiveness research which are used to determine at what level interventions operate in the day to day practice of medicine. The actuarial recurrence-free rates at 5 years are: T1 91%, T2 73%, and T3 44%. The

Lyme disease chemoprophylaxis prescribing before and after pharmacist prescriptive authority in Nova Scotia.

BACKGROUND: Lyme disease is a bacterial infection transmitted to humans through bites from infected Ixodes species of ticks. Prophylaxis with a single dose of oral doxycycline following a bite from an infected tick reduces the risk of developing Lyme disease. Pharmacists in Nova Scotia (NS) were among the first in Canada to prescribe for this indication. The primary objective of this study is to describe experiences with pharmacist prescribing of prophylaxis after tick bites in NS. METHODS: A retrospective cohort study was performed using health administrative data 16 months before and after

Protothecosis and Toxoplasma gondii co-infection in a dog from Nova Scotia, Canada.

A 2-year-old, neutered male mixed-breed dog from Nova Scotia, Canada was evaluated for panuveitis, polyuria, polydipsia, and hind limb ataxia. Toxoplasmosis was diagnosed ante-mortem based on markedly increased Toxoplasma gondii titers. The post-mortem examination confirmed systemic toxoplasmosis and demonstrated disseminated protothecosis. This article documents the first reported case of canine protothecosis in Atlantic Canada. Key clinical message: This case report demonstrates that protothecosis should be a clinical consideration for dogs in Canada. Co-infection with other organisms may oc

Barriers and Facilitators for Accessing Oral Health Care for Ukrainian Newcomers to Nova Scotia.

INTRODUCTION: The war in Ukraine has resulted in a mass exodus of Ukrainians fleeing their country and seeking resettlement in many countries, including Canada. There is a lack of literature, particularly qualitative, that explores past experiences with oral health care in the country of origin for newcomers and the experience of access and utilization of oral health care once in Canada. The increase in Ukrainian newcomers to Canada requires an exploration of barriers and facilitators to oral health care access to inform policy. OBJECTIVE: To explore the barriers and facilitators to oral heal

Prevalence and prenatal diagnosis of neural tube defects in Nova Scotia in 1980-84.

A survey of the records of all hospitals with obstetric services in Nova Scotia revealed that during 1980-84 there were 122 pregnancies involving a neural tube defect. The mean rate was 2/1000 births. Of the affected fetuses or infants 54% had spina bifida, 35% had anencephaly and 11% had encephalocele. The records showed that in the early part of the period studied at least one prenatal ultrasonographic examination had been performed in 60% of the pregnancies; in 1984 the rate was 74%. When examinations done before 16 weeks' gestation were excluded, the overall detection rates at the first ul
