Nickel release from coins.

Nickel allergy is the most frequent contact allergy and is also one of the major background factors for hand eczema. The clinical significance of nickel release from coins was discussed when the composition of euro coins was decided. Current European coinage is dominated by cupro-nickel coins (Cu 75; Ni 25); other nickel-containing and non-nickel alloys are also used. Nickel release from used coinage from the UK, Sweden and France was determined. It was shown that nickel ions are readily available on the surface of used coins. After 2 min in artificial sweat, approximately 2 microg of nickel p

A neurological mystery from history: the case of Claudius Caesar.

The Roman emperor Claudius suffered from a wide range of physical tics and disabilities. Many scholars have explained these symptoms by hypothesizing that Claudius suffered from cerebral palsy. However, this hypothesis is dated and does not take into account all of the evidence available. This article re-examines ancient historical sources in the light of modern medical knowledge in an attempt to retrospectively diagnose the cause of Claudius's problems. In doing so, it suggests that Claudius may have suffered from Tourette's syndrome.

Zinc toxicity from massive and prolonged coin ingestion in an adult.

Acquired copper deficiency anemia is rare in humans. This report describes a 38-year-old schizophrenic man with metal pica, especially coins, who presented with symptomatic anemia. Two hundred seventy-five coins were surgically removed from the gastrointestinal tract of this patient during the course of his hospitalization. Some of the post-1981 pennies, which consist primarily of zinc, showed severe corrosion because of their prolonged contact with acidic gastric juice. The patient presented with clinical manifestations consistent with the local corrosive as well as the systemic effects of zi

Zinc-induced copper deficiency in a coin swallower.

In humans, acquired copper deficiency anemia is rare. This report describes a 58-yr-old man with metal pica, especially coins, who presented with symptomatic anemia. His workup led to the diagnosis of zinc-induced copper deficiency. We believe that, in this man, leaching of zinc from pennies explained the clinical and laboratory findings. This case demonstrates that health care workers should consider the possibility of zinc-induced copper deficiency when confronted with patients with unexplained anemia who have ingested coins or other zinc-containing metals.

The use of Foley's catheter in the removal of a coin in the oesophagus.

A coin in the oesophagus is a common paediatric emergency in Zambia. There are very few specialist surgeons and hospitals where endoscopic removal can be carried out in many resource-limited countries. The technique described here allows the removal of an ingested coin using an ordinary Foley's catheter. The use of this technique under fluoroscopic guidance has been described in literature. However, fluoroscopic facilities and trained radiologists are not commonly available in many parts of Africa. This technique is simple, safe and has a success rate close to that achieved by endoscopic remov

Which coin is easier to pass esophagus spontaneously?

OBJECTIVE: To determine the factors associated with the spontaneous passage (SP) of coins lodged in the esophagus in children. SUBJECTS AND METHODS: A total of 351 pediatric patients with coin ingestion admitted to our hospital from March 2016 to March 2019 were included in the study. The patients underwent a period of overnight watch with a repeated chest x-ray within 24 h after the commencement of hospitalization. The influencing factors (sex, age, types of coin, time in the esophagus, and location) were analyzed. RESULTS: The repeated chest x-ray showed coins that had passed into the stom

Sagittal orientation of ingested coins in the esophagus in children.

OBJECTIVE: The classic teaching has been that coins in the esophagus are oriented in the coronal plane projecting en face on frontal radiographs and tangentially on lateral views, whereas coins in the trachea are oriented sagittally and appear tangential on frontal radiographs and en face on lateral radiographs. We evaluated the clinical presentation and radiographic appearance in eight cases of esophageal coins in children with an atypical sagittal orientation on chest radiographs. MATERIALS AND METHODS: The clinical records and chest radiographs of eight children with sagittally oriented es

Gingival recession and pathologic migration due to an unusual habit.

Various etiologies are implicated as causes of gingival recession and pathological tooth migration, most prevalent among these being the tissue destruction caused by periodontal pathogens. One of the least documented causes is a parafunctional habit, such as digit-sucking or sucking on objects like pencils, pens or toothpicks, all of which can lead to adverse effects on the dentition and the periodontal tissues resulting in midline diastema, alveolar bone loss, gingival recession, mobility, and ultimately tooth loss. This case report describes the periodontal injury caused by an unusual habit

First detection of an OXA-48-producing Enterobacter cloacae isolate from currency coins in Algeria.

OBJECTIVES: The aim of this study was to screen for the presence of β-lactamase-producing Gram-negative bacteria (GNB) from Algerian currency collected from food vendors in Batna city, Algeria. METHODS: During two periods (May 2018 and March-April 2019), a total of 408 coins and currency notes of different denominations of Algerian Dinar were randomly recovered from several food vendors. Samples were subjected to selective isolation of extended-spectrum cephalosporin- and carbapenem-resistant GNB. Bacterial species identification was performed using matrix-assisted laser desorption/ionisation

Alexander Gordon, puerperal sepsis, and modern theories of infection control--Semmelweis in perspective.

Ignaz Semmelweis, a Hungarian doctor who practised in 19th century Vienna, is widely believed to be the father of modern infection control. He earned this accolade when he showed that puerperal sepsis was contagious and that it could be prevented with adequate hand hygiene. In fact, such ideas had circulated in the medical world for at least a century before Semmelweis' work. Moreover, it is well documented that Alexander Gordon, an obstetrician working in Aberdeen, UK, was the first to prove the contagious nature of puerperal sepsis. He also advocated the need for good hygiene for its prevent

Coin exposure may cause allergic nickel dermatitis: a review.

Nickel is used in coins because the metal has beneficial properties, including price, colour, weight, and corrosion resistance, and also because it is easy to stamp. It has often been claimed that the duration of skin contact with coins is too short to cause nickel release and dermatitis. However, it is well known by dermatologists specialized in occupational skin diseases, and by their nickel-allergic patients, that hand eczema in cashiers and other professionals who handle coins may be caused or aggravated by nickel release from coins. In this review, we present evidence from past studies sh

Lodged oesophageal button battery masquerading as a coin: an unusual cause of bilateral vocal cord paralysis.

An 11-month-old girl with an oesophageal foreign body was presented: from the radiographic appearance it was presumed to be a coin. Microlaryngoscopy 5 h after ingestion revealed a button battery impacted in the hypopharynx with severe damage to the oesophageal mucosa. The patient was intubated for 6 days in the intensive care unit because of stridor and respiratory distress. Repeat microlaryngoscopy demonstrated bilateral vocal cord palsy, which was presumed to be secondary to the involvement of the recurrent laryngeal nerves in the injury. We recommend that in the absence of a history of obs
