Severe early lower respiratory tract infection is associated with subsequent respiratory morbidity in preschool Inuit children in Nunavut, Canada.

BACKGROUND: Inuit children in Nunavut, Canada, have high rates of lower respiratory tract infection (LRTI) early in life. Whether this commonly results in chronic respiratory symptoms later in life is unknown. METHODS: A cross-sectional survey of 3- to 5-years-old Inuit children was conducted in all three regions of Nunavut, as part of the "Qanuippitali, what about us, how are we?" survey. RESULTS: Reported chronic cough and wheezing were common in preschool Inuit children, although reported asthma diagnosed by a healthcare professional was uncommon. The presence of smokers in the home tende

Inuit parent perspectives on sexual health communication with adolescent children in Nunavut: "it's kinda hard for me to try to find the words".

BACKGROUND: For Inuit, the family unit has always played a central role in life and in survival. Social changes in Inuit communities have resulted in significant transformations to economic, political and cultural aspects of Inuit society. Where the family unit was once the setting for dialogue on family relations and sexuality, this has largely been replaced by teachings from the medical community and/or the school system. OBJECTIVE: The purpose of this study was to explore Inuit parent perspectives on sharing knowledge with teenage children about sexual health and relationships. METHOD: A

Analysis of Neisseria gonorrhoeae and Mycoplasma genitalium from nucleic acid amplification test specimens, Nunavut region of Inuit Nunangat, Canada, 2020-2023.

This study analyzed Neisseria gonorrhoeae (GC) and Mycoplasma genitalium (Mgen) from remnant nucleic acid amplification test (NAAT) specimens in Nunavut, Canada (2020-2023) to assess strain distribution, antimicrobial resistance (AMR), and co-infection rates. Among 2,221 GC-positive specimens, no cephalosporin resistance was detected, but intermediate-to-decreased susceptibility increased in 2023. Predicted ciprofloxacin resistance was 45%, and azithromycin resistance remained stable at 9.6%. Of 778 tested specimens, 31.4% were co-infected with Mgen, with 49.2% associated with azithromycin res

Drivers of Mercury Cycling in the Rapidly Changing Glacierized Watershed of the High Arctic's Largest Lake by Volume (Lake Hazen, Nunavut, Canada).

Across the Arctic, glaciers are melting and permafrost is thawing at unprecedented rates, releasing not only water to downstream aquatic systems, but also contaminants like mercury, archived in ice over centuries. Using concentrations from samples collected over 4 years and calibrated modeled hydrology, we calculated methylmercury (MeHg) and total mercury (THg) mass balances for Lake Hazen, the world's largest High Arctic lake by volume, for 2015 and 2016. Glacial rivers were the most important source of MeHg and THg to Lake Hazen, accounting for up to 53% and 94% of the inputs, respectively.

TRANSMISSION DYNAMICS OF TOXOPLASMA GONDII IN ARCTIC FOXES (VULPES LAGOPUS): A LONG-TERM MARK-RECAPTURE SEROLOGIC STUDY AT KARRAK LAKE, NUNAVUT, CANADA.

Transmission dynamics of Toxoplasma gondii, a parasite of importance for wildlife and human health, are enigmatic in the Arctic tundra, where free-ranging wild and domestic felid definitive hosts are absent and rarely observed, respectively. Through a multiyear mark-recapture study (2011-17), serosurveillance was conducted to investigate transmission of T. gondii in Arctic foxes (Vulpes lagopus) in the Karrak Lake region, Nunavut, Canada. Sera from adult foxes and fox pups were tested for antibodies to T. gondii by using serologic methods, including the indirect fluorescent antibody test, dire

Achievements and challenges on policies for allied health professionals who use telehealth in the Canadian Arctic.

We formulated policies and procedures for allied health professionals (AHPs) who provide services using telehealth in Nunavut, Canada's newest Arctic territory. These are a supplement to the clinical policies and procedures already established for Nunavut physicians and nurses. The services were in the areas of audiology, dietetics/nutrition, midwifery, occupational therapy, ophthalmic services, pharmacy, physiotherapy, psychology, respiratory therapy, social work and speech therapy. Documents specific to each of the services were developed, drawing on information from Government of Nunavut da

The feasibility of the interferon gamma release assay and predictors of discordance with the tuberculin skin test for the diagnosis of latent tuberculosis infection in a remote Aboriginal community.

BACKGROUND: The tuberculin skin test (TST) is the standard test used to screen for latent TB infection (LTBI) in the northern Canadian territory of Nunavut. Interferon gamma release assays (IGRA) are T cell blood-based assays to diagnose LTBI. The Bacillus Calmette-Guerin (BCG) vaccine is part of the routine immunization schedule in Nunavut. The objective of this study was to test the feasibility, and predictors of discordance between the Tuberculin Skin Test (TST) and the IGRA assay in a medically under-serviced remote arctic Aboriginal population. METHODS: Both the TST and QuantiFERON-TB Go

Evidence of Arctic Fox (Vulpes lagopus) Survival Following Exposure to Rabies Virus.

The arctic fox variant of the rabies virus (RABV) is enzootic in the circumpolar north. Reports of abortive RABV exposures motivated a retrospective analysis of sera from 41 arctic foxes (Vulpes lagopus) captured at Karrak Lake in Nunavut, Canada, during 2011-15. Estimated RABV antibody prevalence among foxes was 15% (95% confidence interval, 7-28%).

Erysipelothrix rhusiopathiae clone reemergence in association with a multi-year mass mortality event in high Arctic muskoxen (Ovibos moschatus).

We investigated a multi-year mass mortality event in muskoxen (Ovibos moschatus) on Ellesmere and Axel Heiberg Islands, Nunavut, Canada and linked the deaths with the "Arctic clone" of the bacterium Erysipelothrix rhusiopathiae (Er), making this the third reported independent mass mortality event in muskoxen associated with this bacterial strain. Between 2021 and 2024, we collected and analyzed samples from 139 unique muskox carcass sites using gross and microscopic pathology, bacterial culture, and molecular methods. Er Arctic clone was found in association with > 70% of sampled carcasses. Ou

Prevalence of Haemophilus influenzae in the nasopharynx of children from regions with varying incidence of invasive H. influenzae serotype a disease: Canadian Immunization Research Network (CIRN) study.

Haemophilus influenzae serotype a (Hia) has recently emerged as an important cause of invasive disease in the North American Arctic and Sub-Arctic regions, mainly affecting young Indigenous children. In this study, we addressed the question of whether the prevalence of Hia and all H. influenzae in the nasopharynx differed between paediatric populations from regions with high versus low incidence of invasive Hia disease. Nasopharyngeal specimens from children with acute respiratory tract infections (ARTI) collected for routine diagnostic detection of respiratory viruses were analysed with molec

Evaluating the Utility of a Psychoeducational Serious Game (SPARX) in Protecting Inuit Youth From Depression: Pilot Randomized Controlled Trial.

BACKGROUND: Inuit youth in Northern Canada show considerable resilience in the face of extreme adversities. However, they also experience significant mental health needs and some of the highest adolescent suicide rates in the world. Disproportionate rates of truancy, depression, and suicide among Inuit adolescents have captured the attention of all levels of government and the country. Inuit communities have expressed an urgent imperative to create, or adapt, and then evaluate prevention and intervention tools for mental health. These tools should build upon existing strengths, be culturally a

Histologic and Genotypic Characterization of Lung Cancer in the Inuit Population of the Eastern Canadian Arctic.

Inuit are the Indigenous Arctic peoples and residents of the Canadian territory of Nunavut who have the highest global rate of lung cancer. Given lung cancer's mortality, histological and genomic characterization was undertaken to better understand the disease biology. We retrospectively studied all Inuit cases from Nunavut's Qikiqtani (Baffin) region, referred to the Ottawa Hospital Cancer Center between 2001 and 2011. Demographics were compiled from medical records and tumor samples underwent pathologic/histologic confirmation. Tumors were analyzed by next generation sequencing (NGS) with a

A cluster of three cases of trichinellosis linked to bear meat consumption in the Arctic.

We report here three cases of trichinellosis due to polar bear meat consumption in East Greenland. In the past 20 years, 31 cases of trichinellosis have been reported in French travellers to the Arctic (North Quebec, Nunavut and Greenland) who consumed undercooked meat from black, brown, or polar bears. If local communities are increasingly becoming aware of the risk of trichinellosis, travellers visiting regions where bear meat is consumed should be informed of the risk of eating raw or non-heat-processed meats.

Anthropogenic Perchlorate Increases since 1980 in the Canadian High Arctic.

An ice core of 15.5 m retrieved from Agassiz Ice Cap (Nunavut, Canada) in April 2009 was analyzed for perchlorate to obtain a temporal trend in the recent decades and to better understand the factors affecting High Arctic deposition. The continuous record dated from 1936 to 2007, covers the periods prior to and during the major atmospheric releases of organic chlorine species that affected the stratospheric ozone levels. Concentrations and yearly fluxes of perchlorate and chloride showed a significant correlation for the 1940-1959 period, suggesting a predominant tropospheric formation by ligh

Comparative analysis of tuberculosis management in Indigenous North Canada and Alaska, USA from 1950s to 2019.

BACKGROUND: Both Alaska and Indigenous North Canada share similarities in geographic location, population, and a history of colonization. While both regions have seen a significant decline in tuberculosis (TB) prevalence over the last century, Nunavut, Canada, has reported a troubling resurgence of TB cases since the early 2000s. OBJECTIVE: To identify analogies and highlight dissimilarities between the two regions using a comparative health systems approach within the historical and sociopolitical contexts. We also aim to provide governments with insights on employing best practices and adop

Ecology of Arctic rabies: 60 years of disease surveillance in the warming climate of northern Canada.

Rabies occurs throughout the Arctic, representing an ongoing public health concern for residents of northern communities. The Arctic fox (Vulpes lagopus) is the main reservoir of the Arctic rabies virus variant, yet little is known about the epidemiology of Arctic rabies, such as the ecological mechanisms driving where and when epizootics in fox populations occur. In this study, we provide the first portrait of the spatio-temporal spread of rabies across northern Canada. We also explore the impact of seasonal and multiannual dynamics in Arctic fox populations and climatic factors on rabies tra

Oral aspiration, type 1 laryngeal cleft, and respiratory tract infections in canadian inuit children.

Canadian Inuit infants suffer the highest rate of lower respiratory tract infections (LRTI's) in the world. The causes of this are incompletely understood. The primary objective of this study was to determine whether there exists an association between respiratory morbidity and oral aspiration in Inuit children. A retrospective chart review was conducted including children from Nunavut who underwent Video Fluoroscopic Swallowing Study between the years of 2001 to 2015. The primary outcome was hospitalization for LRTI. We hypothesized that infants found to have aspiration would experience a hig

Urgent air transfers for acute respiratory infections among children from Northern Canada, 2005-2014.

BACKGROUND: The incidence of hospitalizations for acute respiratory infections (ARI) among young Indigenous children from Northern Canada is consistently high. ARIs requiring urgent air transfer can be life-threatening and costly. We aimed to describe their epidemiology, estimate age-specific incidences, and explore factors associated with level of care required. METHODS: We undertook a retrospective cohort study of children <5 years old from Northern Canada transferred by urgent air transport for ARI from 2005 through 2014 to 5 pediatric tertiary care centers in Vancouver, Edmonton, Winnipeg
