[Nutritional therapy. Indication and approaches].

Appropriate nutritional therapy of surgical patients intends to supply calories for the maintenance of essential body functions. Beyond this goal, nutritional support may also significantly reduce nosocomial morbidity if applied properly and to the right patients. In surgical patients, nutritional therapy should start preoperatively by identifying and treating malnutrition and be continued postoperatively as a patient-tailored supportive measure. Oral/enteral nutrition is feasible in the majority of patients. Rare exceptions are patients with intestinal leakage, overt ileus, and circulatory sh

Access routes for nutritional therapy.

Enteral nutrition (EN) and total parenteral nutrition (TPN) may provide life-sustaining therapy for surgical patients. The duration of nutritional therapy (enteral or parenteral) implies distinct access routes. We review the main aspects related to access routes for nutrient delivery. The enteral route, whenever feasible, is preferred. For EN lasting less than 6 weeks, nasoenteric tubes are the route of choice. Conversely, enterostomy tubes should be used for longer-term enteral feeding and can be placed surgically or with fluoroscopic and endoscopic assistance. The first choice for patients w

Nutritional therapy in liver disease.

Nutritional inadequacies are frequent in patients with liver damage, specially in those with advanced chronic liver disease, and this fact, as in other pathological circumstances, may influence their clinical evolution and outcome. Therefore, nutritional therapy may play an important role in the management of these patients. The present paper will deal with some aspects of the nutritional management in patients with liver disease. Special emphasis will be paid to the role of artificial nutritional support in the treatment of acute alcoholic hepatitis, the current status of artificial nutrition

Nutritional therapy and liver disease.

In terms of a general approach to these patients, the initial and most important step for the clinician is to have recognition and concern for the magnitude of malnutrition in patients with chronic liver disease. It is best to assume an inadequate diet and to have trained personnel review the individual's nutritional needs and design specific dietary regimens that supply sufficient energy and protein, perhaps in the form of frequent interval feedings. The currently available data support the use of specialized formulations only in selected patients with hepatic encephalopathy who are intoleran

Nutritional therapy in children with cancer.

The special biology and behavior of the child make nutrition an even more important adjunct to cancer therapy than is true for the adult. The time has come to add nutritional therapy routinely to our other modes of therapy: surgery, radiotherapy, chemotherapy, and immunotherapy. But it should be done in the same way other modalities are added, i.e., with continued prospective and retrospective review of the data to optimize the approach to the child.

Preoperative nutrition therapy in people undergoing gastrointestinal surgery.

BACKGROUND: Poor preoperative nutritional status has been consistently linked to an increase in postoperative complications and worse surgical outcomes. We updated a review first published in 2012. OBJECTIVES: To assess the effects of preoperative nutritional therapy compared to usual care in people undergoing gastrointestinal surgery. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials, MEDLINE, Embase, three other databases and two trial registries on 28 March 2023. We searched reference lists of included studies. SELECTION CRITERIA: We included randomised contr

[Small intestinal bacterial overgrowth (SIBO) - Therapy, nutrition, microbiome].

SIBO (small intestinal bacterial overgrowth) is defined by bacterial overgrowth or colonization of the small intestine in combination with gastrointestinal symptoms such as bloating, nausea, pain, diarrhoea, malabsorption and food intolerance. SIBO can be caused by various mechanisms such as reduced intestinal motility, altered gastrointestinal anatomy, reduced gastric acid or pancreatic enzyme production, altered bile acid metabolism, or immune defects. Accordingly, SIBO often develops secondary to different underlying diseases.Diet has a fundamental influence on the composition of the intest

Nutrition Delivery During Pediatric Extracorporeal Membrane Oxygenation Therapy.

BACKGROUND: Macronutrient delivery during pediatric ECMO therapy can be challenging. We examined predictors of nutrient delivery in the first 2 weeks of extracorporeal membrane oxygenation (ECMO) therapy in the pediatric intensive care unit (ICU). METHODS: Details of macronutrient delivery were recorded in children (newborn-18 years of age) who survived 24 hours after cannulation to ECMO over a 3-year period (2012-2015). RESULTS: We analyzed data from 54 consecutive eligible patients, 43% female, with median (interquartile range) ECMO duration of 8.5 (6-24) days, age 0.1 (0, 16) months, ICU

Medicare medical nutrition therapy: legislative process and product.

The new Medicare benefit, medical nutrition therapy (MNT), came into effect January 2002-the product of a lengthy legislative process. Over several years, evidence-based advocacy by groups such as the American Diabetic Association and the National Kidney Foundation led to a legislative product that was introduced and passed by Congress. More recently, the legislation entered an implementation process, including the most recent Coverage Determination phase. The definition of MNT and the scientific evidence supporting it are presented. Evidence-based nutrition now enters a new phase of implement

Biotin in man's nutrition and therapy -- a review.

The literature on biotin in human nutrition and therapy has been reviewed. The influence of the diet on biotin excretion and the intestinal synthesis of biotin is discussed. Biotin deficiency symptoms are occasionally seen, especially in infants where the deficiency is manifested as seborrhoeic dermatitis. Nevertheless, low urinary excretion and low circulating biotin levels can be found in certain sections of the population.

Nutrition Guidance for Patients on Androgen Deprivation Therapy.

Dietary guidance for men with prostate cancer on androgen deprivation therapy (ADT) should focus on reducing the risk of prostate cancer-specific outcomes and other chronic diseases, including cardiovascular disease (CVD). National guidelines for both CVD prevention and cancer survivors recommend a diet rich in fruits and vegetables and low in red/processed meat, refined grains, and added fats and sugars. Additional observational studies and randomized controlled trials are needed to examine short- and long-term effects of diet on clinical, biological, and patient-reported outcomes for men tre

Medical nutrition therapy in Canadian federal correctional facilities.

BACKGROUND: Under- and over nutrition as well as nutrition risk factors such as communicable and non-communicable diseases are a common and major cause of morbidity and mortality in correctional facilities. Consequently, medical nutrition therapy (MNT), a spectrum of nutrition services aimed at optimizing individual well-being, is being recognized as integral to the health of people who experience incarceration. However, there is a paucity of research that explores the delivery of MNT in correctional facilities. METHODS: A scoping review combined with secondary analysis of qualitative data (f

Strategies for High-Quality Nutrition Therapy in Brazil.

Strategies for improving nutrition therapy (NT) are of utmost importance in any healthcare system. The identification and treatment of malnourished patients improves clinical outcome and reduces the length of hospital length of stay and the associated costs. In particular, federal policy regulating the use of NT in Brazil and reimbursement of enteral nutrition (EN) treatment were 2 powerful mechanisms that stimulated the development of nutrition care. Basic and clinical research have also contributed to best practices in NT, thereby highlighting the need for nutrition therapy teams, quality in

Micronutrient supplementation in adult nutrition therapy: practical considerations.

Preexisting micronutrient (vitamins and trace elements) deficiencies are often present in hospitalized patients. Deficiencies occur due to inadequate or inappropriate administration, increased or altered requirements, and increased losses, affecting various biochemical processes and resulting in organ dysfunction, poor wound healing, and altered immune status with deleterious sequelae. Guidelines for the 13 essential vitamins and 10 essential trace elements have been established. These recommendations, however, are applicable to healthy adults and not to critically ill patients, in whom decrea

[Supportive measures in palliation: pain therapy and nutrition].

Pain relief and nutritional support represent two main efforts of palliative medicine. A considerable proportion of surgical patients might not be treated with adequate analgetic medication. Those patients are often treated too late, too short or with an insufficient amount of drug. Particularly if the treatment goal is palliation problems of drug abuse are of less importance. However, randomized trials aiming at best pain relief have rarely been carried out in oncological patients. Psychological factors (suffering, affective aspects) have to be borne in mind when deciding upon pain treatment.

Alcohol craving in rehabilitation: assessment of nutrition therapy.

If untreated, alcohol abuse, which often results from alcohol craving, causes major metabolic abnormalities, altered life-styles, lost productivity, and eventually death. Biochemical mechanisms that may contribute to alcohol craving include the stress response of the hypothalamic-pituitary adrenal axis, the endogenous opiate beta-endorphin system, neurotransmitter synthesis and release, hypoglycemia, and nutrient deficiencies. The macronutrient ratio of meals, the resulting insulin response, and nutrient blood levels can affect amino acid and nutrient transport across the blood-brain barrier.

Improved total parenteral nutrition therapy management by a nutritional support team.

This study was performed to identify and document the benefits associated with a multidisciplinary consult-based nutritional support team. The adequacy of nutritional support and incidence of metabolic complications were prospectively studied in 28 adult patients receiving total parenteral nutrition in a large private tertiary care institution during a 2-week period. Estimated daily caloric and protein requirements were met significantly more often in patients followed by our NST than in control group patients. Chloride and bilirubin abnormalities occurred significantly less often in the NST g

Optimizing clinical nutrition practices: impact of a formulary prescription-based nutrition therapy protocol in surgical patients.

BACKGROUND: Parenteral nutrition (PN) is a crucial clinical therapy, particularly in surgical patients. However, due to the complex composition of PN formulations, the inappropriate use of PN remains widespread in clinical practice to date. This study established a formulary prescription-based nutrition therapy protocol and assessed the effectiveness of its implementation. METHOD: The formulary prescription-based nutrition therapy program included the development of formulary prescriptions for PN and the establishment of a perioperative nutrition medication pathway based on the formulary pres
