'The obesity paradox': a reconsideration of obesity and the risk of preterm birth.

OBJECTIVE: The association between obesity and spontaneous preterm births (sPTBs) has been shown to be influenced by obesity-attendant comorbidities. Our objective was to better understand the complex relationship of obesity and its attendant comorbidities with sPTBs. STUDY DESIGN: A retrospective analysis utilizing maternally linked hospital and birth certificate records of 2 049 196 singleton California deliveries from 2007 to 2011. Adjusted relative risks (aRRs) for sPTBs were estimated using multivariate Poisson regression modeling. RESULTS: Obese women had higher aRRs for sPTBs than the

Rethinking the obesity paradox in sepsis: Metabolic pharmacotherapy explains survival benefits of obesity.

BACKGROUND & AIMS: Sepsis remains a leading global health burden. While the "obesity paradox" suggests improved outcomes in overweight and obese individuals, the impact of metabolic abnormalities (diabetes, hypertension, hyperlipidemia) and their associated pharmacotherapies on sepsis prognosis remains unclear. This study aimed to investigate the independent and synergistic effects of overweight status and metabolic abnormalities on sepsis mortality. METHODS: This retrospective cohort study utilized data from 16,288 sepsis patients in the Medical Information Mart for Intensive Care-IV databas

The obesity paradox: A further consideration in dialysis patients.

Several epidemiological cohorts have demonstrated that higher body mass index (BMI) is associated with lower mortality risk among patients receiving hemodialysis. However, BMI may be an inaccurate indicator of nutritional status among dialysis patients because it does not differentiate between muscle and fat mass or provide information about body fat distribution. More sophisticated methods of body composition analysis are therefore required to address the question of which component is associated with greater survival. Recent evidence has also shown that changes in body weight and body compos

Obesity and bone health: reconciling the density-fragility paradox.

The relationship between obesity and bone health is increasingly recognized as complex and paradoxical. Despite higher bone mineral density (BMD) in individuals with obesity, fracture risk is not uniformly reduced. Instead, fractures tend to occur more frequently at peripheral sites such as the ankle, humerus, and tibia, indicating a mismatch between BMD and true bone strength. In the early stages of weight gain, mechanical loading and adipose-derived hormonal factors may promote bone formation. However, these effects are progressively counterbalanced by chronic inflammation, oxidative stress,

The obesity paradox and hypoglycemia in critically ill patients.

BACKGROUND: A high body mass index (BMI) has been associated with decreased mortality in critically ill patients. This association may, in part, relate to the impact of BMI on glycemia. We aimed to study the relationship between BMI, glycemia and hospital mortality. METHODS: We included all patients with a recorded BMI from four large international clinical databases (n = 259,177). We investigated the unadjusted association of BMI with average glucose levels, mortality and hypoglycemia rate. We applied multivariate analysis to investigate the impact of BMI on hypoglycemia rate, after adjustin

No Obesity Paradox in Pediatric Patients With Dilated Cardiomyopathy.

OBJECTIVES: This study aimed to examine the role of nutrition in pediatric dilated cardiomyopathy (DCM). BACKGROUND: In adults with DCM, malnutrition is associated with mortality, whereas obesity is associated with survival. METHODS: The National Heart, Lung, and Blood Institute-funded Pediatric Cardiomyopathy Registry was used to identify patients with DCM and categorized by anthropometric measurements: malnourished (MN) (body mass index [BMI] <5% for age ≥2 years or weight-for-length <5% for <2 years), obesity (BMI >95% for age ≥2 years or weight-for-length >95% for <2 years), or normal bo

The paradox of obesity in patients with heart failure.

PURPOSE: Heart failure (HF) patients often have comorbid conditions that confound management and adversely affect prognosis. The purpose of this study was to determine whether the obesity paradox is also present in hospitalized HF patients in an integrated healthcare system. DATA SOURCES: A cohort of 2707 patients with a primary diagnosis of HF was identified within an integrated, 20-hospital healthcare system. Patients were identified by ICD-9 codes or a left ventricular ejection fraction < or =40% dating back to 1995. Body mass index (BMI) was calculated using the first measured height and

Do Obesity Classifications Create the Obesity Paradox? A Scoping Review of Obesity Definitions Applied in Sepsis Research.

Obesity appears to be associated with improved health outcomes in patients with sepsis, a phenomenon termed the obesity paradox. However, the potential influence of varying operational definitions of obesity on clinical outcomes within this paradox remains inadequately characterised. This scoping review aimed to identify, analyse, and synthesise the methodological approaches to obesity definition employed in sepsis research. A systematic literature search was conducted in August 2023 across MEDLINE, Embase, CINAHL, and CENTRAL databases. This review included original articles, systematic revie

Obesity Paradox in Critically Ill Patients: Do Patients with Critical Diseases Benefit from Obesity?

<p>Background: Overweight and obesity are linked to an elevated risk of numerous serious diseases and health conditions. Despite the detrimental effects associated with these conditions, recent studies have indicated that critically ill patients may experience a survival advantage with higher body mass index. Initial research on the obesity paradox primarily concentrated on cardiovascular diseases (CVDs), such as heart failure (HF); however, this focus has now expanded to encompass other critical illnesses, particularly sepsis, Acute Respiratory Distress Syndrome (ARDS), trauma, and others. The obesity paradox: impact of obesity on the prevalence and prognosis of cardiovascular diseases.

Obesity has reached global epidemic proportions and is associated with numerous comorbidities such as hypertension (HTN), type 2 diabetes mellitus, dyslipidemia, certain cancers, and chronic kidney disease (CKD). Obesity, via its direct maladaptive effects on cardiac structure and through its impact on conventional risk factors, is strongly associated with cardiovascular (CV) diseases such as heart failure (HF) and coronary heart disease (CHD). Despite these adverse associations, numerous studies indicate an "obesity paradox" in that being overweight or obese is associated with a favorable pro

Nutritional Status and Obesity Paradox in Acute Coronary Syndrome Patients.

The worse nutritional status of patients without obesity after an acute coronary syndrome (ACS) could explain their worse prognosis and the so-called obesity paradox. We performed a retrospective study of all consecutive patients admitted to a hospital for ACS between 2009 and 2020. Nutritional status was analyzed using the CONUT scale, and values > 4 were categorized as moderate-severe malnutrition. We included 2789 patients with a mean BMI of 28.0 (4.6) kg/m2 and 26.4% with obesity. The mean CONUT index was 2.4 (2.2), and 24.3% had moderate-severe malnutrition. Obese patients had a lower pre

Beyond BMI: The diet-microbiome context of the obesity paradox.

In Nature, Desharnais et al. show that in diverse mouse models, anti-PD-1 sensitivity is more closely linked to the diet-microbiome-metabolite ecosystem than to obesity-associated metabolic dysfunction. Their findings reinforce body mass index (BMI) as an incomplete proxy for host biology and suggest strategies to improve anti-PD-1 efficacy without inducing obesity.

Obesity paradox role in the immunosuppressive treatment of hepatocellular carcinoma.

The "obesity paradox" in hepatocellular carcinoma (HCC) suggests patients with obesity may experience better treatment outcomes compared to patients without obesity. Wang et al highlighted this paradox in HCC immunotherapy, demonstrating superior progression-free survival and overall survival in patients with overweight and obesity treated with lenvatinib and camrelizumab, focusing on hepatitis B virus-related HCC. Mechanisms such as better nutritional reserves, leptin-mediated immune modulation, and reduced protein breakdown may explain these outcomes. Obesity's role in anti-programmed cell d

Obesity Paradox in Heart Failure Revisited: Etiology as Effect Modifier.

The prognostic value of overweight/obesity in heart failure (HF) may vary according to HF etiologies. We aim to determine whether body mass index has differential impacts on survival among hospitalized HF patients with varying etiologies. Consecutive hospitalized HF patients between December 2006 and December 2017 were included. Multivariable analyses, including Cox proportional hazard models and restricted cubic splines, were used to investigate the impact of body mass index on mortality by HF etiology. Among the 3,836 patients included (mean age 57.1 years, 28.4% women), 1,475 (38.5%) were i

Does the "obesity paradox" exist after transcatheter aortic valve implantation?

BACKGROUND: Transcatheter aortic valve implantation (TAVI) for symptomatic aortic stenosis is considered a minimally invasive procedure. Body mass index (BMI) has been rarely evaluated for pulmonary complications after TAVI. This study aimed to assess the influence of BMI on pulmonary complications and other related outcomes after TAVI. METHODS: The clinical data of 109 patients who underwent TAVI in our hospital from May 2018 to April 2021 were retrospectively analyzed. Patients were divided into three groups according to BMI: low weight (BMI < 21.9 kg/m2, n = 27), middle weight (BMI 21.9-27

The obesity paradox in acute coronary syndrome: a meta-analysis.

In the general population, the lowest mortality risk is considered to be for the body mass index (BMI) range of 20-24.9 kg/m(2). In chronic diseases (chronic kidney disease, chronic heart failure or chronic obstructive pulmonary disease) the best survival is observed in overweight or obese patients. Recently above-mentioned phenomenon, called obesity paradox, has been described in patients with coronary artery disease. Our aim was to analyze the relationship between BMI and total mortality in patients after acute coronary syndrome (ACS) in the context of obesity paradox. We searched scientific
