The Frame-In Approach: A Neurobiological and Attachment-Based Model for Managing the Therapeutic Frame in Complex Trauma Treatment.

This article proposes an updated conceptualization of the psychotherapeutic frame, emphasizing its adaptive functions, particularly in the treatment of patients with complex trauma histories or disrupted early attachment relationships. Although conventional views emphasizing the frame's static elements need to be considered, they are insufficient for managing complex relational dynamics, frame departures, impasses, and enactments that commonly arise in treatment with patients who have experienced significant trauma. Integrating insights from attachment theory and the neuroscience of memory sys

Attachment at the end of life: A systematic review.

BACKGROUND: The diagnosis of an advanced life-threatening illness brings with it existential challenges that activate the attachment system and different attachment styles influence coping with advanced illness. OBJECTIVES: The objective of this work were (a) to analyze the influence of attachment styles of patients with advanced disease and their relatives on emotional distress and other psychological and existential aspects, and (b) to identify the most used assessment instruments to measure it, highlighting those with better psychometric properties in palliative care contexts. METHODS: Ar

Hostile-helpless states of mind: A scoping review of risk factors, correlates, and consequences.

Chronic relational trauma can lead to the formation of pervasively unintegrated attachment representations in adulthood, referred to as Hostile-Helpless (HH) states of mind. Individuals with this type of attachment disorganization evidence conflicting evaluations of caregivers and have difficulty reflecting on their traumatic childhood experiences. This scoping review is the first to systematically integrate the results of all empirical studies on HH states of mind in an effort to highlight the scientific and clinical contributions of the concept and guide future research. Does secure base script knowledge mediate associations between observed parental caregiving during childhood and adult romantic relationship quality and health?

Increasingly, attachment representations are being assessed via secure base script knowledge - the degree to which individuals show awareness of the temporal-causal schema that summarizes the basic features of seeking and receiving effective support from caregivers during times of need. Limited research has assessed the links between secure base script knowledge and aspects of adult functioning and the role that secure base script knowledge may play in accounting for associations between early caregiving quality and adulthood functioning. We used follow-up assessments of the NICHD Study of Ear

The relationships between attachment styles and well-being in older adults: a three-level meta-analysis and systematic review.

BACKGROUND AND OBJECTIVES: Well-being is essential for older adults in an aging society. Although numerous studies have examined the relationship between attachment styles and well-being in older adults, the primary aim of this meta-analysis was to clarify the distinct associations of secure and insecure attachment with both well-being and ill-being in this population. RESEARCH DESIGN AND METHODS: Following the guidelines of the Preferred Reporting Items for Systematic Review and Meta-Analyses statement, a three-level meta-analytic model was applied to quantitatively synthesize relevant findi

Attachment Theory: Novel Clinical and Molecular Insights.

Early-life adversity (ELA) disrupts brain development and is linked to poor health outcomes across species, including humans and rodents. A growing body of work suggests that impaired attachment to a caregiver-arising from erratic, neglectful, or abusive parenting-mediates a substantial portion of ELA's long-term effects. Despite the conceptual and clinical appeal of this idea, the neural mechanisms by which ELA disrupts attachment and how altered attachment in turn produces diverse psychiatric and medical sequelae remain incompletely understood. In this review, we synthesize recent randomized

Representations are constructed: where are the questions about the dynamics of the first interactions and the security of the attachment bond?

This commentary proposes a dynamic approach to the construction of relationship representations and mentalization. We argue that these processes result from early caregiver-infant interactions, particularly through mechanisms of synchrony, parental sensitivity, and attachment. This extends the author's model beyond cognitive and innate perspectives by emphasizing the role of early interactions in constructing representations that are embodied, affective, and dynamic.

When safety fails: A narrative review on attachment in patients with obsessive-compulsive disorder.

Obsessive-compulsive disorder (OCD) has traditionally been conceptualized according to cognitive-behavioral and neurobiological domains. However, increasing attention has been directed toward its relational and developmental dimensions, particularly the potential role of attachment-related processes. This narrative review explored the relationship between attachment patterns and OCD, with the aim at synthesizing and commenting on the current empirical and clinical evidence, and on how disruptions in early attachment may contribute to the onset and maintenance of OCD. Increased attention has be

A systematic review and meta-analysis of clinician behaviors and characteristics related to alliance building with youth clients.

The literature on developing alliance with youth clients has severely lagged behind that of adults. To date, there has been no meta-analytic review and limited qualitative synthesis of clinician variables which may contribute to the alliance with youth clients. Systematic literature search found a total of 49 relevant citations. Of the identified articles, 13 (27.08%) reported quantitative methods. Results of the qualitative review suggested that the majority of this research has focused on developing an emotional bond and a sense of collaboration. Meta-analysis found significant relations bet

Insecure attachment and personality pathology: Concurrent assessment and longitudinal modeling.

Insecure attachment and personality pathology are parallel frameworks for representing interpersonal dysfunction. Although research to date has revealed strong bivariate associations between these constructs, the current state of the science is nonspecific and leaves piecemeal guidance for clinical and empirical efforts. The goal of the present study was to determine the magnitude of the associations between attachment and personality pathology across two conceptualizations of maladaptivity and across three waves of time, thereby satisfying repeated calls for empirical innovation in this area.

Challenges for the Future of Psychoanalysis.

This paper summarizes the current crisis of psychoanalysis in its relations to the scientific and cultural environment. It proposes tasks to assure the survival and contributions of psychoanalysis as science, profession, and humanistic discipline. It proposes emphasis on empirical research in the boundaries with the neurosciences and social psychology, and the development of an infrastructure for research linked to its educational program. It proposes renovation in training, abolishment of the training analysis system, and systematic teaching and research on the psychoanalytic psychotherapies

Attachment Disorders.

The ability to form attachments with other human beings is an essential skill that typically begins early in life. The parent/caregiver-child relationship is the first crucial relationship that an infant forms, and the health of this relationship has a profound effect on the child's social and emotional development. Children who form secure relationships with their parent or other primary caregiver have fewer internalizing and externalizing behaviors, are more socially competent, and have better-quality friendships. Conversely, children with attachment disorders exhibit a varying capacity to f

Assessment and Treatment of Attachment-Related Aspects in Adolescents with Personality Disorders In recent years the relevance of attachment-related aspects in psychotherapy research has increased. This approach offers an opportunity to test the effectiveness of specific psychotherapeutic procedures for personality disorders in adulthood. Although there are a number of novel psychotherapeutic interventions for personality disorders that were adapted for adolescents, there is still a lack of research on studies that consider attachment-related characteristics. The present study demonstrates att

The neurobiological effects of childhood maltreatment on brain structure, function, and attachment.

Childhood maltreatment is a risk factor for psychopathologies, and influences brain development at specific periods, particularly during early childhood and adolescence. This narrative review addresses phenotypic alterations in sensory systems associated with specific types of childhood maltreatment exposure, periods of vulnerability to the neurobiological effects of maltreatment, and the relationships between childhood maltreatment and brain structure, function, connectivity, and network architecture; psychopathology; and resilience. It also addresses neurobiological alterations associated wi

Behavioral interactions and emotional engagement with loss-related objects in bereavement: associations with grief symptom severity.

BACKGROUND: Grief involves attachment- related regulatory processes through which individuals adapt to separation from the deceased. Loss-related objects (i.e., emotionally significant belongings of the deceased) may reflect may reflect externalized expressions of proximity-seeking, yet their association with grief symptoms remains understudied. AIMS: This study examines the role of loss-related objects in the grieving process by investigating their associations with attachment styles, difficulties in emotion regulation, and grief symptom severity. METHOD: The sample consisted of 250 adults

Changes in Attachment Representation and Personality Organization in Transference-Focused Psychotherapy.

OBJECTIVE: The aim of this article was to construct an empirical bridge between object relations theory and attachment theory by investigating how researchers in both traditions have contributed to understanding and assessing identity diffusion (a keystone of personality pathology) and object relations in patients with borderline personality disorder during 1 year of transference-focused psychotherapy (TFP). METHODS: The Adult Attachment Interview (AAI) and the Structured Interview of Personality Organization (STIPO) were administered to patients (N=104, all women) before and after 1 year of

The phallic child: its emergence and meaning in a clinical setting.

The nature of the drive toward forming and maintaining intense attachments with other human beings has been examined in the light of case material from the play therapy sessions of a ten-year-old boy. The primary question addressed concerns the relationship of the attachment drive to other drive systems. Freudian theory, object relations theory, and attachment theory present competing paradigms for understanding the relationship between the drives. The Freudian paradigm sees human attachment as derivative from other drives, especially the sexual drive. In developing his attachment theory, Bowl

Organizing adult attachment in alternative ways: A qualitative assessment of schemas antithetical to the secure base script.

How adults make meaning of their childhood experiences with caregivers plays a central role in how they anticipate support, manage distress, and interpret relationships. To understand how adults conceptualize these attachment relationships, the Adult Attachment Interview (AAI) probes for childhood memories of general relationships, day-to-day interactions, and instances of loss, threat, and separation. Across these narratives, the AAI captures expectations of whether caregivers are perceived as an available source of support in times of distress, and whether they serve as a secure base from wh

Toward a double bind theory of borderline personality disorder.

This article revisits the Bateson group's concept of the double bind, originally proposed as an etiological theory of schizophrenia, and reinterprets it as a central framework for understanding borderline personality disorder (BPD). We propose that paradoxical communication patterns in early development, characterized by contradictory and entrapping messages, become internalized and give rise to the structural and interpersonal paradoxes that define BPD. Drawing upon psychodynamic theory, empirical research, and contemporary object relations perspectives, we conceptualize BPD as a "disorder of
