Obturator nerve block for painful hip in adult cerebral palsy.

Nearly 25% of patients with severe spastic cerebral palsy (CP) develop hip dislocations/subluxations which may become painful. Three patients with spastic CP and painful hip dislocations/subluxations, who were poor surgical candidates and were unresponsive to conservative medical management, underwent obturator nerve block which resulted in temporary relief of pain and improved their activities of daily living. There were no known complications from this procedure.

Morphology and morphometry of the human obturator nerve in males and females.

Peripheral nerve injury and the nerves' subsequent repair and regeneration continues to be marked clinically by poor functional recovery. The analysis of nerve morphology is an aspect which may provide an impact on successful clinical outcomes through better prediction of donor and recipient matching. In this study, we evaluated the morphological aspects of the human obturator nerve for a better understanding of its potential in nerve transplantation. Morphological characteristics of donor obturator nerves were analysed, including nerve diameter and length, fascicle count and the ratio of neur

Obturator mononeuropathy caused by lipomatosis of the nerve: a case report.

We report a patient who presented with the clinical features of obturator mononeuropathy. Abdomino-pelvic computed tomography revealed a fusiform mass in the right perivesical space; magnetic resonance imaging (MRI) showed characteristic "coaxial-cable-like" appearance in cross-section and "spaghetti-like" appearance in longitudinal section, pathognomonic of lipomatosis of the nerve. Nerve lipomatosis as the cause of obturator neuropathy has not been previously reported. MRI provides definite and graphic proof of the diagnosis.

[Neuropathy of the obturator nerve as a source of pain in soccer players].

Chronic inguinal and lower abdominal pain in high-performance athletes is common and often disabling problem. Diagnose and treatment is often difficult due to many anatomic structures in the inguinal and groin region that have the potential to cause pain. We report 52 cases of a chronic groin pain in soccer players due to fascial entrapment of the obturator nerve. All patients presented clinical symptoms and signs of post exercise groin, lower abdominal or medial tight pain and adductor muscles weakness and paresthesia in cutaneous distribution of medial thigh. Except clinical signs in the dia

Repair of the femoral nerve by two motor branches of the obturator nerve: A case report.

Complete femoral nerve palsies are uncommon but devastating injuries when they are caused by large nerve defects. Direct repair is usually not possible and nerve grafting renders uncertain outcomes. Recent studies proposed different peripheral nerve transfers as treatment strategies for large femoral nerve defects. We report a clinical application of a nerve transfer to reinnervate the quadriceps muscle with two motor branches of the obturator nerve in a 48 years-old man that was diagnosed with a femoral nerve palsy after resection of a retroperitoneal schwannoma. The branches supplying the gr

[Vaginal revision of irritation of the obturator nerve within colposuspension].

Irritation of the obturator-nerve within colposuspension is a possible complication because of topographic proximity between obturator-nerve and operating-field. The main symptoms are weakness of the adductor muscles, sensory disturbance of thigh till paralysis and pain in the operating- field early after surgery. Too lateral fixing of the sutures in the pectineal ligament above the obturator-channel can cause compression of the obturator-nerve. Precocious intervention is a precondition for complete remission of symptoms, retropubic revise of surgery is evident. The method outlined here descri

An evaluation of the cutaneous distribution after obturator nerve block.

UNLABELLED: In 1973, Winnie et al. introduced the inguinal paravascular three-in-one block, which allegedly provides anesthesia of three nerves--the femoral, lateral cutaneous femoral, and obturator nerves--with a single injection. This concept was undisputed until the success of the obturator nerve block was reassessed by using evidence of adductor weakness rather than cutaneous sensory blockade, the latter being variable in its distribution and often absent. We performed this study, therefore, to evaluate the area of sensory loss produced by direct injection of local anesthetic around the ob

[Schwannoma of the obturator nerve with urologic manifestations: report of a case].

Peripheral nerve tumors, which are called schwannomas because they arise from nerve sheath Schwann cells, are rare tumors than can develop at any site in the body but involve the limbs in over 50% of cases. Only 13% of schwannomas arise in the trunk; about 3% of schwannomas are retroperitoneal, and about 4% of retroperitoneal tumors are schwannomas. Pelvic schwannomas are equally uncommon and can develop in a broad range of structures. The most common presenting manifestation is a very large space-occupying lesion responsible for compression of neighboring organs. Excision of the tumor is ofte

Obturator nerve entrapment. A cause of groin pain in athletes.

Chronic groin pain in athletes is often difficult to diagnose and treat. There are many anatomic structures in the inguinal and groin region that have the potential to cause pain. We report 32 cases of a previously undescribed condition in athletes of "obturator neuropathy," a fascial entrapment of the obturator nerve where it enters the thigh. This condition represents a type of groin pain in athletes that is treatable by surgical means. There is a characteristic clinical pattern of exercise-induced medial thigh pain commencing in the region of the adductor muscle origin and radiating distall

Anatomical investigation of a new vertical obturator nerve block technique.

BACKGROUND: We evaluated a vertical obturator nerve block (VOB) using a single morphological landmark and no additional distance measurement or obligatory changes of the needle's direction. MATERIALS AND METHOD: A total of 88 cadavers (176 lower limbs), prepared using Thiel's embalming method, were examined. The index finger was placed lateral to the palpable pubic tubercle and the needle inserted laterally to the distal part of the fingernail at the tubercle's level and advanced strictly perpendicular to the table's surface. If bone contact was made, the needle was slightly turned to pass th

Stimulation of the obturator nerve during extracorporeal shock wave lithotripsy.

This paper presents eigth patients with lithiasis of the lower third of the ureter that were treated by extracorporeal shock wave lithotripsy (ESWL). All the procedures were performed with a Dornier HM4 lithotriptor in the sitting position. In all cases clinical signs of stimulation of the obturator nerve were presented. Because of the continuing movement of the patient and the possibility of damage to the obturator nerve, the lithotripsy was interrupted in the sitting position and was performed successfully with patients in the prone position. One possible explanation for the above phenomenon

Extracorporeal shock wave lithotripsy induced stimulation of the obturator nerve.

Extracorporeal shock wave lithotripsy to a left lower third ureteral calculus, 1.4 x 0.9 cm. large, using an unmodified Dornier HM3* lithotriptor with the patient in the sitting position produced thigh adductor movement with each shock. The procedure was terminated. Extracorporeal shock wave lithotripsy was performed uneventfully 1 week later using a Medstone STS lithotriptor with the patient in the supine position. We believe that the sitting position caused increased intra-abdominal pressure in the pelvis resulting in apposition of the ureteral segment containing the calculus to the obturato

Obturator-to-pelvic nerve rerouting to restore bladder function after radical pelvic nerve injury.

BACKGROUND: Damage to the pelvic nerve can occur at the time of a radical hysterectomy or during resection of a rectal cancer or sacrococcygeal teratoma and lead to a neurogenic bladder and an inability to volitionally void. We report on our experience with somatic-to-autonomic nerve regeneration by obturator-to-pelvic nerve rerouting and its results in terms of restoration of normal bladder function. METHODS: Eleven patients (2 male, 9 female; age range 4-58 years) with a neurogenic bladder following radical hysterectomy or resection of a sacrococcygeal teratoma underwent an obturator-to-pel

Ultrasound-guided obturator nerve block technique at the distal end of the obturator canal: case series and cadaver evaluation.

The obturator nerve variably gives off the anterior, posterior, and hip articular branches along its course; however, all branches invariably pass through the obturator canal. Herein, we describe our obturator nerve block technique, which promises to deliver local anesthetic directly into the obturator canal. We performed the obturator nerve block in six patients undergoing transurethral resection of bladder tumor under spinal anesthesia. We also evaluated the spread of dye in a cadaver. With patient placed in the lithotomy position, a linear ultrasound transducer was placed on the anterior pa

[Anatomical study on the repair of femoral nerve injury with anterior branch of obturator nerve].

OBJECTIVE: To investigate the feasibility of the anastomosis of the anterior branch of obturator nerve and the muscular branch of femoral nerve. METHODS: Five fresh frozen cadavers, including 3 males and 2 females, were included. Both of the obturator nerve, femoral nerve and their branches were dissected, then their routes and anatomical positions were observed. The diameter and the number of myelinated nerve fiber of the anterior branch of obturator nerve and femoral nerve muscular branches were measured, as well as the overlap distance between them. RESULTS: The diameter of myelinated ner

Anatomical variations of the external obturator muscle and their impact on injectate spread to the obturator canal: a cadaveric study related to ultrasound-guided proximal obturator nerve block.

We anatomically investigated whether the superior fasciculus (SF) of the external obturator muscle (EOM) influences the spread of dye into the obturator canal, in a cadaveric model simulating a proximal ultrasound-guided obturator nerve block. Thirteen sides from seven Thiel-embalmed cadavers (five male cadavers, nine sides; two female cadavers, four sides) were studied. Blue dye was injected either between the pectineus muscle and the EOM, the pectineus muscle and the SF, or the SF and the main belly of the EOM. After injection, gross dissection was performed to evaluate dye spread along the

Life-threatening haemorrhage following obturator artery injury during transurethral bladder surgery: a sequel of an unsuccessful obturator nerve block.

In spite of prior blockade of the obturator nerve with 1% mepivacaine (8 ml) utilizing a nerve stimulator, violent leg jerking was evoked during transurethral electroresection of a bladder tumour approximately 1 h after the blockade in a 68-year-old man. The patient became severely hypotensive immediately following the jerking, and a large lower abdominal swelling concurrently developed. The urgent laparotomy indicated that the left obturator artery was severely injured by the resectoscope associated with the bladder perforation, causing acute massive haemorrhage. The patient recovered unevent
