Occipital Bone Defect With Meningoencephalocele and Plexiform Neurofibroma in Neurofibromatosis-1.

Neurofibromatosis type 1 (NF-1) is the most common phakomatosis with autosomal dominant inheritance, caused by mutations in the NF-1 gene on chromosome 17q11.2, which codes for neurofibromin protein, a negative regulator of the RAS/MAPK pathway. A defect in this can lead to altered cellular growth and tumor development. NF-1 is a complex multi-system neurocutaneous disorder with some of the common manifestations being café au lait macules, axillary/inguinal freckling, and neurofibromas. Musculoskeletal manifestations include scoliosis, sphenoid wing dysplasia, tibial pseudoarthrosis, and, rare

Failure of ossification of the occipital bone in mandibuloacral dysplasia type B.

Mandibuloacral dysplasia with type B lipodystrophy is a rare autosomal recessive disease characterized by atrophic skin, lipodystrophy, and skeletal features. It is caused by mutations in ZMPSTE24, a gene encoding a zinc metalloproteinase involved in the post-translational modification of lamin. Nine distinct pathogenic variants have been identified in 11 patients from nine unrelated families with this disorder. We report a 12-year-old boy with mandibuloacral dysplasia with type B lipodystrophy and a novel homozygous c.1196A>G; p.(Tyr399Cys) mutation in ZMPSTE24. The patient had typical dermat

Mapping occipital bone thickness using computed tomography for safe screw placement.

OBJECT Safe and effective insertion of occipital bone screws requires morphological analysis of the occipital bone, which is poorly documented in the literature. The authors of this study present morphological data for determining the area of screw placement for optimal internal fixation. METHODS The subjects of this institutional review board-approved retrospective study were 105 individuals without head and neck disease who underwent CT imaging at the authors' hospital. There were 55 males and 50 females, with a mean age of 57.1 years (range 20-91 years). Measurements using CT were taken acc

[Cloverleaf skull syndrome with occipital bone cristae. Report of two cases].

Two cases of cloverleaf skull syndrome are reported. The patients were born with trilobed deformity of the skull associated with synostosis of the lambdoidal, coronal, and sagittal sutures in Case 1 and of the lambdoidal suture in Case 2. Both patients had hydrocephalus, low-set ears, and ocular hypertelorism. Ventriculoperitoneal shunting had been performed for hydrocephalus in both cases in early infancy. Thereafter, the skull anomaly caused mental and physical retardation and elevated intracranial pressure. In both cases, surgery was successful in terms of cosmetic and neurological improvem

[Compound depressed fracture of occipital bone causing laceration of left occipital lobe and injury of superior sagittal sinus--case report].

A case of compound depressed fracture of occipital bone in 52-years old man caused by crane hook is presented. Bone fragments were depressed 40 millimetres causing blindeness and huge hemorrhage with symptoms of hemorrhagic shock. Laceration of left occipital lobe and penetration of superior sagittal sinus were treated during the operation. Primary elevation, debridement and primary bone replacement with additional covering of bone defect by bone cement were performed. Primary wound healing was achieved. Cortical blindeness subsided to the stage of contralateral homonymous hemianopia.

Ehlers-Danlos syndrome-associated craniocervical instability with cervicomedullary syndrome: Comparing outcome of craniocervical fusion with occipital bone versus occipital condyle fixation.

INTRODUCTION: Ehlers-Danlos syndrome (EDS) predisposes to craniocervical instability (CCI) with resulting cranial settling and cervicomedullary syndrome due to ligamentous laxity. This study investigates possible differences in radiographic outcomes and operative complication rate between two surgical techniques in patients with EDS and CCI undergoing craniocervical fusion (CCF): occipital bone (OB) versus occipital condyle (OC) fixation. METHODS: A retrospective search of the institutional operative database between January 07, 2017, and December 31, 2019, was conducted to identify EDS patie

Large occipital aneurysmal bone cyst causing obstructive hydrocephalus in a pediatric patient.

Aneurysmal bone cyst (ABC) is an uncommon benign, tumorlike lesion of bone that is usually located in the long bones and spine. On rare occasions, ABCs are found in the bones of the cranial vault and skull base, occasionally causing mass effect and cranial nerve findings. In this report the authors detail the case of a patient who presented with incidentally discovered hydrocephalus due to a large ABC of the occipital bone that produced mass effect and obstruction of CSF. The diagnosis, imaging findings, and surgical management of this interesting and rare case are discussed.

Developmental abnormalities of the occipital bone in human chondrodystrophies (achondroplasia and thanatophoric dwarfism).

Specific developmental malformations have been demonstrated in the occipital bone of two chondrodysplastic disorders (achondroplasia and thanatophoric dwarfism). Analysis of these malformations indicates that the occipital bone is primary affected in these disorders. In both cases, the endochondral-derived components of the occipital bone (the basioccipital, the two lateral parts, and the planum nuchale of the squama occipitalis) have failed to grow properly and are smaller and shorter than normal. On the other hand, the planum occipitalis of the squama, which derives from intramembranous ossi

Modified interfacet technique using shaped autologous occipital bone mass for basilar invagination.

PURPOSE: In economically undeveloped areas, surgery for basilar invagination (BI) is still a serious economic burden for people. This study introduces a modified interfacet technique for the treatment of BI using shaped autologous occipital bone mass to reduce BI and to save economical expenditure. METHODS: The data of 6 patients with BI who underwent modified interfacet technique using shaped autologous occipital bone mass in our hospital from April 2020 to February 2021 were retrospectively analyzed. During the operation, osteotomy at the external occipital protuberance was performed using

Post-traumatic transient cortical blindness in a child with occipital bone fracture.

Cortical blindness as sequelae of trauma has been reported in literature but mostly in the setting of occipital cortex or visual tract damages. We present a case of transient cortical blindness in a child following a closed head injury with a non-displaced occipital bone fracture and underlying occipital lobe contusion. We discuss the pathophysiology behind Post-traumatic transient cortical blindness, relevant investigations, and current management.

Histological study of the occipital bone from patients with Chiari I malformation.

BACKGROUND: This study is focused on the histologic characteristics of occipital bone removed during Chiari I decompression in the hope of discovering unique features that may be related to the pathogenesis of this condition. METHODS: Ten consecutive pediatric patients with Chiari I malformation underwent standard posterior fossa decompression surgery. Bone that was removed from the posterior fossa was sent for histological examination. Bone from age-matched controls also underwent histological analysis. RESULTS: For all study and control specimens, bony samples were found to be made up of d

A case of primary Ewing's sarcoma of the occipital bone presenting with obstructive hydrocephalus.

BACKGROUND: Ewing's sarcomas account for approximately 10% of primary malignant bone tumors. While most of the primary Ewing's sarcomas occur in the long bones, pelvis, or ribs, approximately 1-6% of these sarcomas arise in the skull. We found approximately 50 cases of Ewing's sarcoma of the calvarium reported in our search of the literature. Of these, 36 cases were reported in detail. CASE REPORT AND DISCUSSION: In this article we describe a rare case of primary Ewing's sarcoma of the occipital bone presenting with acute obstructive hydrocephalus and review the literature.

Fibrous dysplasia in combination with aneurysmal bone cyst of the occipital bone and the clivus: case report and review of the literature.

OBJECTIVE AND IMPORTANCE: Fibrous dysplasia of the cranium is a relatively uncommon disorder that affects primarily the anterior cranial region; its occurrence in the cranial base in combination with aneurysmal bone cyst (ABC) constitutes an extremely rare condition, only two cases of which have been reported previously in the literature. It is important to recognize and treat these cases properly because of the special location in the cranial base and the possibility of neural structure impingement. CLINICAL PRESENTATION: We report the case of a 19-year-old man with a slowly enlarging mass o
