Mucus layer covers the ocular surface, and soluble mucins are also present in the tear fluid. After topical ocular drug administration, the drugs and formulations may interact with mucus layer that may act as a barrier in ocular drug delivery. Ophthalmic surgeons often apply phenylephrine topically to effect pupillary dilatation. We describe a paediatric patient in whom cardiac arrhythmias, severe hypertension and pulmonary oedema occurred following intraoperative ocular phenylephrine administration. We believe that systemic absorption of the drug was responsible and discuss ways in which this might be reduced when ocular phenylephrine is used in this context.

Even though subconjunctival injections are used in clinics, their quantitative pharmacokinetics has not been studied systematically. In a study of the ocular absorption and elimination of a topically applied non-steroidal anti-inflammatory drug (NSAID), flurbiprofen, the compound was well absorbed into rabbit ocular tissues and was highly concentrated in the rabbit cornea. Chronic liver pathologies may lead to vitamin A deficiency (VAD) through impairment of vitamin A absorption, storage, and distribution. VAD can contribute to ocular pathologies, and in the article, we present 2 patients with alcohol-associated cirrhosis admitted for liver transplant presenting with nonhealing central corneal epithelial defects in the eye without other known ocular pathologies. We screened patients with episcleritis or scleritis to detect syphilis by using either fluorescent treponemal antibody absorption testing or microhemagglutination assay for antibodies to Treponema pallidum. Four patients, two with nodular episcleritis and two with scleritis, had ocular involvement as the initial manifestation of late syphilis. Topical or oral corticosteroid therapy provided minimal initial clinical benefit, but ocular inflammation resolved within one to three weeks after administration of systemic penicillin.

Ocular syphilis has a wide range of presentations; however, the most common findings are ophthalmalgia, blurry vision, and erythematous conjunctiva. This report presents a case of ocular syphilis in a human immunodeficiency virus-positive patient with an allergy to penicillin. On presentation, the patient was diagnosed using a fluorescent treponemal antibody absorption test, in addition to rapid plasma reagin. In this case, the patient received alternative treatment with doxycycline prior to penicillin desensitization, with marked improvement of his symptoms.

Soemmering's ring and spontaneous lens absorption are two distinct conditions that are uncommon and unlikely to occur simultaneously. We report a case of a 67-year-old man who presented with blurred vision in his left eye and has had poor eyesight since birth. His right and left eye visual acuities were 6/7.5 and hand movement (HM), respectively. There was no relative afferent pupillary defect (RAPD). The right eye's examination was unremarkable. The left eye revealed a spontaneous rupture of the anterior lens capsule with partially absorbed lens material and the presence of Soemmering's ring.
