Comparability of two commonly used automated office blood pressure devices in the severely obese.

The accuracy of automated office blood pressure (BP) devices in severely obese individuals is unknown. In an exploratory study, we compared standardized BP measurements using a BpTRU device with those taken using a WatchBP Office monitor. Paired t-tests, Bland-Altman plots, and Pearson's correlations were used to compare devices. A total of 62 patients were analyzed: their mean age was 39.5±9.3 years and their mean BMI was 45.1±5.7 kg/m. Fifty-five patients (89%) were women and 21 (34%) had a diagnosis of hypertension. The mean BpTRU values were 117.2±12.7/76.7±9.7 mmHg and the mean WatchBP me

Thresholds for Diagnosing Hypertension Based on Automated Office Blood Pressure Measurements and Cardiovascular Risk.

The risk of cardiovascular events in relation to blood pressure is largely based on readings taken with a mercury sphygmomanometer in populations which differ from those of today in terms of hypertension severity and drug therapy. Given replacement of the mercury sphygmomanometer with electronic devices, we sought to determine the blood pressure threshold for a significant increase in cardiovascular risk using a fully automated device, which takes multiple readings with the subject resting quietly alone. Participants were 3627 community-dwelling residents aged >65 years untreated for hypertens

Comparison of Office Blood Pressure, Automated Unattended Office Blood Pressure, Home Blood Pressure, and 24-Hour Ambulatory Blood Pressure Measurements.

BACKGROUND: Although previous studies have reported differences of blood pressure (BP) according to BP measurement methods, studies in Korean population were scarce. This study aimed to compare BP differences according to different BP measurement methods and assess hypertension phenotype. METHODS: This prospective study recruited 183 individuals (mean 55.9 years; 51.4% males). The BP measurements included office BP (auscultatory attended office BP [ausAOBP], automated attended office BP [aAOBP], and automated unattended office BP [aUAOBP]) and out-of-office BP (home BP [HBP] and ambulatory BP

Agreement between resting heart rate measured by unattended automated office and office blood pressure measurement, ambulatory blood pressure monitoring, or electrocardiography.

The application of unattended blood pressure measurement (uAOBPM) for resting heart rate (RHR) assessment is unknown. To assess the agreement between RHR measured during uAOBPM and other methods, the authors conducted a comparability study with office blood pressure measurement (OBPM), ambulatory blood pressure monitoring (ABPM), and electrocardiogram (ECG) in a group of 110 participants referred for ABPM. RHR measured with uAOBPM (70.8 ± 12.5 bpm) was significantly lower than OBPM (72.8 ± 12.6 bpm) but higher than measured by 24 h ABPM (67.5 ± 10.2 bpm). There was no significant difference wa

Identification of the Optimal Protocol for Automated Office Blood Pressure Measurement Among Patients With Treated Hypertension.

BACKGROUND: Automated office blood pressure (AOBP) involving repeated, unobserved blood pressure (BP) readings during one clinic visit is recommended for in-office diagnosis and assessment of hypertension. However, the optimal AOBP protocol to determine BP control in the least amount of time with the fewest BP readings is yet to be determined and was the aim of this study. METHODS: One hundred and eighty-nine patients (mean age 62.8 ± 12.1 years; 50.3% female) with treated hypertension referred to specialist clinics at 2 sites underwent AOBP in a quiet room alone. Eight BP measurements were t

Threshold for diagnosing hypertension by automated office blood pressure using random sample population data.

OBJECTIVE: Manual office blood pressure (BP) is still recommended for diagnosing hypertension. However, its predictive value is decreased by errors in measurement technique and the white-coat effect. The errors can be eliminated by automated office BP (AOBP) measurement taking multiple readings with the participant resting quietly alone. Therefore, use of AOBP in clinical practice requires a threshold value for hypertension diagnosis. The aim of the present study was to determine an AOBP threshold corresponding to the 140/90 mmHg manual office BP using data from a large random population sampl

Automated Office Blood Pressure Measurements in Waiting Room or Isolated Room for Diagnosis and Phenotyping of Hypertension.

BACKGROUND: Automatic office blood pressure measurement (AOBPM) in the waiting room could improve the performance of diagnosing hypertension without requiring an isolated room to carry out exploration. METHODS: In a cross-sectional study, we compared the performance to diagnose and phenotyping hypertension by 3 methods of blood pressure measurements, standard office blood pressure, unattended AOBPM in an isolated room or in an office waiting room AOBPM, compared with daytime ambulatory blood pressure measurement (daytime ambulatory blood pressure monitoring) from a 24-hour ambulatory blood pr

Comparison of automated office blood pressure measurement with 24-hour ambulatory blood pressure measurement.

PURPOSE: Although 24-hour ambulatory blood pressure measurement (24-h ABPM) is the most important method to establish true hypertension, in clinical practice often repeated automated office blood pressure (AOBP) measurements are used because of convenience and lower costs. We aimed to assess the agreement rate between a 30 and 60 min AOBP and 24-h ABPM. MATERIALS AND METHODS: Patients with known hypertension (cohort 1) and patients visiting the neurology outpatient clinic after minor stroke or transient ischaemic attack (cohort 2) were selected. We performed AOBP for 30-60 min at 5-min interv

Unobserved automated office BP is similar to other clinic BP measurements: A prospective randomized study.

Results of the SPRINT study have been disputed, based on the assumption that unattended BP measurements do not correlate with usual BP measurements. In this study, the authors investigated the correlation of unattended SPRINT-like measurements with other conventional measurements. All BP measurements were taken with the patient seated in a comfortable chair with the legs uncrossed and not speaking during the procedure. For the purpose of this study, sixty-five patients, mostly male (93%), were recruited from our hypertension clinic and all were on antihypertensive medication (av 3.0 ± 1.1). Pa

Detection of tuberculosis using digital chest radiography: automated reading vs. interpretation by clinical officers.

SETTING: A busy urban health centre in Lusaka, Zambia. OBJECTIVE: To compare the accuracy of automated reading (CAD4TB) with the interpretation of digital chest radiograph (CXR) by clinical officers for the detection of tuberculosis (TB). DESIGN: A retrospective analysis was performed on 161 subjects enrolled in a TB specimen bank study. CXRs were analysed using CAD4TB, which computed an image abnormality score (0-100). Four clinical officers scored the CXRs for abnormalities consistent with TB. We compared the automated readings and the readings by clinical officers against the bacteriologi

Automated office blood pressure measurement: a Hypertension Australia and National Hypertension Taskforce of Australia position statement.

Hypertension is the most common problem managed in Australian general practice, yet most adults with hypertension do not have their blood pressure (BP) treated to target. Hypertension diagnosis and management rely upon accurate BP measurements performed using a standardised protocol. However, health system barriers prevent doctors from following measurement protocols, leading to inaccurate BP assessments. A practical BP measurement protocol that can be widely implemented is urgently warranted. Automated office BP (AOBP) is the recommended measurement standard for the diagnosis and management o

Revisiting Unattended Versus Attended Automated Office Blood Pressure Measurements: A Systematic Review and Meta-Analysis.

BACKGROUND: Standardized automated office blood pressure (AOBP) measurement is widely accepted as the preferred method for monitoring blood pressure (BP) in health care settings. However, disagreements persist as to the need to perform AOBP unattended, where the patient is left alone in a quiet room. Thus, this study aimed to assess the BP differences between unattended and attended AOBP. METHODS: This systematic review and meta-analysis was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and registered on PROSPERO (International Prospe

The use of automated sequential blood pressure in hypertension clinics compared with office and ambulatory blood pressure measurements.

BACKGROUND: Office blood pressure (OBP) measurement is the most common method of blood pressure measurement. However, it is associated with several pitfalls as white coat effect and masked hypertension. Ambulatory blood pressure monitoring (ABPM) is usually used for diagnosis of hypertension and elimination of white coat effect. This study aimed to assess the correlation and degree of agreement of the automated sequential blood pressure (ASqBP) with OBP and ABPM. Patients presented to hypertension clinic were included. Each patient had his blood pressure recorded by three methods: OBP using a

Cardiovascular Risk in Hypertension in Relation to Achieved Blood Pressure Using Automated Office Blood Pressure Measurement.

The SPRINT (Systolic Blood Pressure Intervention Trial) reported that some older, higher risk patients might benefit from a target systolic blood pressure (BP) of <120 versus <140 mm Hg. However, it is not yet known how the BP target and measurement methods used in SPRINT relate to cardiovascular outcomes in real-world practice. SPRINT used the automated office BP technique, which requires the patient to be resting quietly and alone, with multiple readings being recorded automatically using an electronic oscillometric sphygmomanometer. We studied the relationship between achieved automated off

Diagnostic Performance of Unattended Automated Office Blood Pressure Measurement for Hypertension Screening Among People With and Without HIV.

BACKGROUND: The diagnostic performance of automated office blood pressure (AOBP) in screening for hypertension in people with HIV (PWH) is not known. METHODS: We conducted a cross-sectional analysis of baseline data from PWH and people without HIV (PWoH) from the Mwanza HIV&CVD cohort study. We conducted unattended AOBP and 24-hour ambulatory BP monitoring as recommended by international guidelines. Using average 24-hour BP as the reference standard, we estimated the prevalence of hypertensive diagnostic phenotypes and calculated measures of diagnostic performance at different diagnostic cuto

Optimal duration and number of readings for unattended automated office blood pressure measurements in patients with type 2 diabetes.

OBJECTIVE: Unobserved automated office blood pressure (uAOBP) measurement is better correlated to daytime ambulatory blood pressure monitoring (dABPM) than traditional office blood pressure (BP) measurements. However, prolonged uAOBP duration may underestimate BP levels. We aimed to determine the duration of uAOBP that has the lowest proportion of white-coat hypertension (WCH) or masked hypertension (MH) compared with the gold-standard using dABPM in patients with type 2 diabetes (T2DM). Additionally, we examined variables associated with discrepancy between uAOBP and dABPM. METHODS: A total
