Septic and nonseptic olecranon bursitis. Utility of the surface temperature probe in the early differentiation of septic and nonseptic cases.

Forty-six consecutive cases of olecranon bursitis were prospectively analyzed during a 1-year period. Eleven cases were septic; 35 cases were nonseptic. In addition to bursal fluid analysis, the surface temperature over the involved olecranon bursa and the contralateral (control) olecranon process was obtained by using a surface temperature probe. We compared the temperature difference between the involved and control sides in all septic and nonseptic cases. In nonseptic cases, the mean surface temperature difference was 0.7 degree C vs 3.7 degrees C in septic cases. In all septic cases, the t

Allograft reconstruction of olecranon after traumatic bone loss: a case report.

Despite 2.2 million bone allografts conducted annually, their complication rate remains high, with recipients incurring infection, fracture, instability, and failure to incorporate. Nonunion rates in massive bone allografts-a bone segment ≥5 cm in length that also contains the total circumference of replaced bone-have been documented as high as 50%. However, if early complication can be avoided, a 75% success rate at 20 years postoperatively has been reported. Nonmassive allografts may yield decreased nonunion rates, as massive bone allografts must overcome a greater metaphyseal to diaphyseal

Tension Band Suturing Versus Tension Band Wiring for Management of Pediatric Olecranon Fractures: Systematic Review and Meta-analysis.

BACKGROUND: Pediatric olecranon fractures require optimal fixation to prevent long-term morbidity. Tension band wiring (TBW) has been the gold standard, but tension band suturing (TBS) has emerged as a potential alternative. This study systematically reviews and compares outcomes of TBS and TBW in pediatric olecranon fractures. MATERIALS AND METHODS: A systematic review and meta-analysis were conducted per Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines. Nine studies were included, with three directly comparing TBS to TBW. RESULTS: No statistically significant d

Olecranon osteotomy for exposure of fractures and nonunions of the distal humerus.

Although olecranon osteotomy provides excellent exposure of the distal humerus, enthusiasm for this approach has been limited by reports suggesting numerous complications. It has been suggested that specific techniques for creating and repairing an olecranon osteotomy may help limit complications. This paper describes a technique for olecranon osteotomy using an apex, distal, chevron-shaped osteotomy, Kirschner wires directed out the anterior ulnar cortex distal to the coronoid process and bent 180degrees and impacted into the olecranon proximally, and two 22- gauge, figure-of-eight, stainless

Shape of articular surface of crocodilian (Archosauria) elbow joints and its relevance to sauropsids.

The determination of area and shape of articular surfaces on the limb bones of extinct archosaurs is difficult because of postmortem decomposition of the fibrous tissue and articular cartilages that provide the complex three-dimensional joint surfaces in vivo. This study aims at describing the shape of the articular cartilages in the elbow joints of six crocodilian specimens; comparing its structure with that of four birds, three testudines, and five squamates; and comparing the shapes of the surfaces of the calcified and the articular cartilages in the elbow joints of an Alligator specimen. Fasciocutaneous flaps based on fascial feeding vessels for defects in the periolecranon area.

Skin defects over the olecranon process are usually a result of wound dehiscence following trauma or following joint replacement. Rarely, breakdown occurs over the olecranon in degenerative joint diseases and as the result of pressure necrosis. We have used a fascial feeding vessel-based fasciocutaneous flap to reconstruct 10 defects in nine patients. The technique provides a thin and supple local flap that is easy to raise and is reliable.

Anatomical localization of motor endplate bands in the human biceps brachii.

OBJECTIVES: : Botulinum neurotoxin (BoNT) reduces disabling muscle contraction in numerous disorders with muscle overactivity. While motor endplates are the primary site of action for BoNT, their location is unknown in most human muscles. This study localized the motor endplate bands (MEB) within human biceps brachii (BB) muscles in relation to external landmarks. METHODS: : Five human BB muscles were obtained from autopsy. Three muscles were processed for Sihler's stain, a nerve staining technique, and 2 were processed by whole-mount acetylcholinesterase stain. RESULTS: : The major MEB in B

Partial posteromedial olecranon resection: a kinematic study.

BACKGROUND: The posteromedial aspect of the olecranon process is a site of impingement and subsequent osteophyte development in throwing athletes. Treatment with débridement, with resection of osteophytes and varying amounts of normal olecranon bone, is common. We found no reports in the literature concerning the effects of resecting different amounts of normal bone from the posteromedial aspect of the olecranon. We hypothesized that excessive resection would increasingly alter elbow kinematics and that an optimum amount of olecranon resection could be identified. METHODS: We investigated the

Ultrasonographic findings in patients with olecranon bursitis.

BACKGROUND: Musculoskeletal ultrasonography (U.S.) is an important imaging technique in the diagnosis of olecranon bursitis, especially for early manifestation. It allows sensitive detection of small fluid collections as well as for differentiation between soft tissue and bone lesions. U.S. examination allows detection of effusions, synovial proliferation, calcifications, loose bodies, rheumatoid nodules, gout tophi and septic processes. AIM: To assess the role of ultrasonography in the diagnosis and management of patients with olecranon bursitis. METHODS: Ultrasound was used in 34 patients

Mycotic Olecranon Bursitis in a Diabetic Patient: A Rare Case Report.

Olecranon bursitis is a common clinical condition that is usually traumatic, inflammatory, or bacterial in origin. Fungal involvement of the olecranon bursa is exceedingly rare and may present a diagnostic challenge because of its indolent course and nonspecific clinical and radiological features. A 63-year-old female with diabetes mellitus presented with a gradually progressive, painless swelling over the posterior aspect of the left elbow of 1 year duration. Ultrasonography revealed a thick-walled cystic lesion with internal echoes and septations overlying the olecranon process, while plain

Monteggia fracture-dislocation: a case report, its' initial management and review of bado's classification.

Fracture of proximal ulna and dislocation of the proximal radio-ulna joint in the same arm is called Monteggia fracture- dislocation. Four clinical variants of this fracture- dislocation have been described in literature. This is a report and description of initial management of a fracture- dislocation, which was consequent to a sideswipe injury and consisted of a combination of the following injuries; lateral dislocation of the radial head, fractures of the olecranon and proximal ulna in the left forearm. The radial head dislocation and fracture of the olecranon were open. The head of the rad

[OLECRANON BURSITIS].

The olecranon bursa is a pocket-like structure located at the posterior aspect of the elbow that is responsible for the smooth movement of the surrounding tissues. Frequently, it is the source of elbow pain due to an inflammation which may be caused by local injury or penetration of bacteria through the skin. This can lead to an initial acute and possibly a late chronic inflammation. Chronic inflammation may originate from systemic diseases such as gout and rheumatoid arthritis as well. The treatment of olecranon bursitis may be conservative (non-surgical) or surgical. Recently, there is more

The effect of Ding's screws and tension band wiring for treatment of olecranon fractures: a biomechanical study.

Although tension band wiring (TBW) is popular and recommended by the AO group, the high rate of complications such as skin irritation and migration of the K-wires cannot be ignored. Ding's screw tension band wiring (DSTBW) is a new TBW technique that has shown positive results in the treatment of other fracture types. The objective of this study was to evaluate the stability of DSTBW in the treatment of olecranon fractures by biomechanical testing. We conducted a Synbone biomechanical model by using three fixation methods: DSTBW, intramedullary screw and tension band wiring (IM-TBW), and K-wir

Anatomic evaluation of the triceps tendon insertion at the proximal olecranon regarding placement of fracture fixation devices.

PURPOSE: Olecranon fractures, especially with a small proximal fragment, remain a surgical challenge. Soft tissue irritation and affection of the triceps muscle bear a risk of complications. In order to find an area for a soft-tissue sparing placement of implants in the treatment of olecranon fractures, we aimed to define and measure the segments of the proximal olecranon and evaluate them regarding possible plate placement. METHODS: We investigated 82 elbow joints. Ethical approval was obtained from the local ethics committee, After positioning in an arm holder and a posterior approach we de
