One-Lung Ventilation in a Pediatric Tracheostomy Patient: A Case Report.

A 20-month-old male with congenital lobar emphysema presented for right middle lobectomy. He had previously undergone right upper lobectomy and tracheostomy placement and remained ventilator-dependent. Due to recent accidental decannulation necessitating emergent tracheostomy revision, otolaryngology advised that stoma patency be maintained throughout the perioperative period. As such, orotracheal intubation techniques were not practical options for lung isolation. This case report describes a unique method of achieving one-lung ventilation in a young pediatric patient with a tracheostomy.

A novel method for right one-lung ventilation modeling in rabbits.

There is no standard method by which to establish a right one-lung ventilation (OLV) model in rabbits. In the present study, a novel method is proposed to compare with two other methods. After 0.5 h of baseline two-lung ventilation (TLV), 40 rabbits were randomly divided into sham group (TLV for 3 h as a contrast) and three right-OLV groups (right OLV for 3 h with different methods): Deep intubation group, clamp group and blocker group (deeply intubate the self-made bronchial blocker into the left main bronchus, the novel method). These three methods were compared using a number of variables:

[One lung ventilation in cases with unsuccessful double-lumen endobronchial intubation].

Three cases of thoracotomy with unsuccessful double-lumen endobronchial intubation were reported. Two were first intubated with an endotracheal tube. In the first patient a Fogarty catheter was inserted alongside the endotracheal tube into the right main bronchus to act as a bronchial blocker. The second patient employed a tracheal tube exchanger to change the endotracheal tube into an Univent tube for one lung ventilation. Nasal fiberoptic intubation was performed in the third case and a Fogarty catheter was passed through the lumen of the endotracheal tube into the left main bronchus as the

Optimizing Lung Collapse During One-Lung Ventilation: Physiological Mechanisms and Clinical Strategies: A Narrative Review.

Effective thoracic surgery requires timely, predictable operative lung collapse. During one-lung ventilation (OLV), lung collapse is not merely a mechanical consequence of nonventilated lumen opening but a phase-dependent physiological process. Rapid phase I collapse is driven by elastic recoil and passive gas venting, whereas slower phase II collapse depends on residual alveolar gas absorption. Communication between the operative-side airway and the atmosphere before pleural opening may permit tidal gas movement, ambient air entrainment, and nitrogen re-entry during the closed-chest period, d

Preoperative Multivariable Model for Risk Stratification of Hypoxemia During One-Lung Ventilation.

BACKGROUND: Hypoxemia occurs with relative frequency during one-lung ventilation (OLV) despite advances in airway management. Lung perfusion scans are thought to be one of the most accurate methods to predict hypoxemia during OLV, but their complexity and costs are well-known limitations. There is a lack of preoperative stratification models to estimate the risk of intraoperative hypoxemia among patients undergoing thoracic surgery. Our primary objective was to develop a risk stratification model for hypoxemia during OLV based on preoperative clinical variables. METHODS: This is a single-cent

One-Lung Ventilation via End Tracheostomy for Thoracic Surgery: A Case Report.

Standard double-lumen tubes (DLTs) are unsuitable in patients with end tracheostomy due to the shortened tracheal length and atypical stoma angulation. This case highlights the challenges of achieving one-lung ventilation (OLV) in patients with end tracheostomies due to anatomical changes following total laryngectomy. A 68-year-old male with a history of total pharyngo-laryngo-esophagectomy, gastric pull-up, and end tracheostomy underwent video-assisted thoracic surgery (VATS) for left lung wedge resection and lymph node dissection. A #7.5 Portex cuffed tracheostomy tube and a Coopdech bronchi

Amlodipine and Atropine for Hypoxia During One-Lung Ventilation: A Case Report.

Anesthetists are concerned about the causes and management of hypoxia during one-lung ventilation (OLV). Here, we report a hypoxic case during OLV and video-assisted thoracic surgery (VATS) for pulmonary lobectomy. The preoperative management of hypertension with amlodipine was considered to be responsible for the hypoxia. As a calcium channel blocker, amlodipine may inhibit hypoxic pulmonary vasoconstriction (HPV) and contribute to the reduction of the ventilation/perfusion ratio (or V/Q ratio). The hypoxia efficiently was treated by atropine, where both tracheal effects and the enhancement o
