Perinatal Opioid-Related Disorder Care Across Nursing and Community Settings: A Scoping Review.

BACKGROUND: Pregnant and postpartum individuals affected by opioid use disorder (OUD) experience barriers to accessing medications for opioid use disorder (MOUD) and engagement in treatment. Nursing-led approaches that integrate community and pharmacy may help close care gaps, especially in rural areas. METHODS: This scoping review examined current U.S.-based evidence describing certified nurse midwives (CNMs), community-based pharmacists (CBPs), and community health support (CHS) in caring for perinatal OUD. Using PRISMA-ScR guidelines, four databases were searched for English-language studi

Disentangling the effects of racism-related stress on opioid use disorder and chronic pain outcomes.

In recent years, Black people in the U.S. have had one of the highest increases in opioid overdose mortality rates, despite being less likely to be prescribed opioids for pain. This population is also less likely to receive medications for opioid use disorder (MOUD). Chronic pain is a central factor in understanding this crisis, as minoritized people are more likely to live with undertreated pain, a major risk factor for developing opioid use disorder (OUD). Current practices fail to effectively treat pain among persons with OUD, a missed opportunity that is worse in minoritized populations an

The Associations Between Opioid Use Disorder and Healthcare-Related Outcomes in Vaso-occlusive Crisis.

BACKGROUND: In patients who experience frequent vaso-occlusive crises (VOC), opioid dependence may be due to a need for pain control as opposed to addiction; the implications of opioid use disorder (OUD) in this population are unclear. OBJECTIVE: To compare outcomes in hospitalizations for VOC in those with a history of OUD to those without a history of OUD. DESIGN: A retrospective assessment of hospitalizations for adults in the USA with a primary discharge diagnosis of VOC using the National Inpatient Sample database from 2016 to 2019. We also compared VOC hospitalizations to hospitalizati

Resting State EEG Activity Related to Impulsivity in People with Prescription Opioid Use Disorder.

Previous studies on EEG activity in prescription opioid use disorder (OUD) have reported neuronal dysfunction related to heroin use, most consistently reflected by increases in β-brain oscillations. As similar research has yet to examine EEG associated with non-medical use of prescription opioid and as inhibitory deficits are associated with OUD, this pilot study compared quantitative EEGs of 18 patients with prescription OUD and 18 healthy volunteers and assessed relationships between oscillatory activity and impulsivity with the Barratt Impulsiveness Scale (BIS-11). Spectral EEGs showed grea

Community social environment in relation to opioid use disorder across an urban-rural spectrum.

BACKGROUND: Addressing the opioid epidemic requires understanding environmental factors influencing population-level risk of opioid use disorder (OUD). To elucidate the role of social environmental conditions on opioid-related risk environments, we assessed two community-level measures-social disorganization and social capital-in relation to OUD across an urban-rural spectrum. METHODS: In a case-control study using medical records (2012-2020) from a Pennsylvania health system, 14,674 OUD cases were frequency matched (on age, sex, year, medical record duration) to 58,696 controls. Study indivi

Management of acute cancer-related pain in a patient with opioid use disorder: a case report.

BACKGROUND: Managing cancer-related pain in patients with opioid use disorder (OUD) poses significant challenges. We present this case of an OUD patient with severe intractable pain from metastatic testicular cancer to illustrate one approach to effective pain management. CASE PRESENTATION: A 31-year-old non-Hispanic White man with OUD experienced acute intractable cancer-related pain that was ultimately controlled with > 4700 daily oral morphine equivalents of patient-controlled fentanyl analgesia. He successfully weaned to oral methadone with oxycodone for breakthrough pain. CONCLUSIONS: E

Analgesic opioids in pregnancy and placental malperfusion-related disorders: a population-based cohort study.

BACKGROUND: Analgesic opioid use in pregnancy could increase the risk of disorders related to placental malperfusion, but this relationship is incompletely characterized. We aimed to study the causal association between analgesic opioids in pregnancy and placental abruption, pre-eclampsia, preterm birth, and fetal growth restriction (FGR). METHODS: We conducted a population-based cohort study of pregnancies resulting in birth at ≥20 weeks of gestation between July 2013 and December 2019 in New South Wales, Australia. Linked data on pregnancy, births, medication dispensation, and health servic

Public Health Interventions and Overdose-Related Outcomes Among Persons With Opioid Use Disorder.

IMPORTANCE: Given the high number of opioid overdose deaths in the US and the complex epidemiology of opioid use disorder (OUD), systems models can serve as a tool to identify opportunities for public health interventions. OBJECTIVE: To estimate the projected 3-year association between public health interventions and opioid overdose-related outcomes among persons with OUD. DESIGN, SETTING, AND PARTICIPANTS: This decision analytical model used a simulation model of the estimated US population aged 12 years and older with OUD that was developed and analyzed between January 2019 and December 20

Differences in Inpatient Management of Cancer-Related Pain Among Patients with Opioid Use Disorder.

The management of cancer-related pain in patients with opioid use disorder (OUD) remains complex and often challenging for clinicians and patients. There is currently a paucity of data to guide best practices, and the evidence that exists is variably applied. In this hospital-based questionnaire, we sought to understand the variation in practice patterns among clinicians in palliative medicine, addiction medicine, and hospital medicine, in caring for this complex patient population. Sixty-two questionnaire responses were analyzed and variation was found in management of pain, as well as initia

Prevalence of Opioid Use Disorder among Patients with Cancer-Related Pain: A Systematic Review.

BACKGROUND: The opioid use disorder is an international public health problem. Over the past 20 years it has been the subject of numerous publications concerning patients treated for chronic pain other than cancer-related. Patients with cancer-related pain are also at risk of opioid use disorder. The primary objective of this literature review was to determine the prevalence of opioid use disorder in patients with cancer-related chronic pain. Its secondary objective was to identify the characteristics of these opioid users. METHODS: This is a literature review of studies published over the la

Long-Acting versus Short-Acting Opioids and Opioid-Related Disorders in Chronic Non-Cancer Pain: A Claims Data Analysis in Germany.

BACKGROUND: Chronic non-cancer pain is common in primary care and may be managed with long-term opioid therapy. The German guideline "Long-term Use of Opioids for Chronic Non-Cancer Pain" provides recommendations for such treatment. The role of opioid formulation (long-acting vs. short-acting) and the association with opioid-related disorders remains underexplored. This study aims to explore associations between opioid formulation and opioid-related disorders in patients with chronic non-cancer pain receiving long-term opioid therapy in German general practices. METHODS: We conducted a retros

Perioperative opioid related disorders on outcomes following lower extremity fracture fixation: Comparative analysis from a multicenter national database.

BACKGROUND: Opioid use can be common in patients who require surgical fixation of lower extremity fractures. While common, these medications place patients at risk for developing opioid-related disorders (OD) which can in turn affect bony healing and propagate endocrinopathies. This study aims to investigate the impact of perioperative opioid-related disorders on short- and long-term outcomes following open reduction and internal fixation (ORIF) of lower extremity fractures. METHODS: This retrospective study utilized the multicenter database TriNetX to identify patients who underwent ORIF of

Mood and anxiety disorders and their association with non-medical prescription opioid use and prescription opioid-use disorder: longitudinal evidence from the National Epidemiologic Study on Alcohol and Related Conditions.

BACKGROUND: Non-medical use of prescription opioids represents a national public health concern of growing importance. Mood and anxiety disorders are highly associated with non-medical prescription opioid use. The authors examined longitudinal associations between non-medical prescription opioid use and opioid disorder due to non-medical opioid use and mood/anxiety disorders in a national sample, examining evidence for precipitation, self-medication and general shared vulnerability as pathways between disorders. METHOD: Data were drawn from face-to-face surveys of 34 653 adult participants in

Management of loperamide-related opioid use disorder with buprenorphine within an Australian context: A case report.

Loperamide is an over-the-counter peripheral mu-opioid agonist commonly used to manage diarrhoea. When taken in an excessive dose, loperamide's penetration of the central nervous system increases, leading to euphoria, respiratory depression and other opioid effects. A health warning was released in the United States in 2016 with increased reports of excessive loperamide use leading to cardiac conduction abnormalities such as ventricular tachycardia, prolonged QTc and Torsades des Pointes. The growing number of cases is likely due to the increased restrictions on other opioids. Loperamide use d
