BACKGROUND AND OBJECTIVES: To describe a novel technique of transorbital access to the lateral wall of the cavernous sinus (CS) after exenteration.
METHODS: Cadaveric dissection study. Seven heads (13 orbits) were dissected after total orbital exenteration. The technique was centered on creation of an osteotomy within the greater wing of sphenoid, bordered by the superior and inferior orbital fissures to access the middle cranial fossa. V2 within the foramen rotundum was used as a guide to enter the interdural plane of the lateral CS wall. Results were expressed as the mean value ±1 SD.

Orbital exenteration is a radical and disfiguring surgery mainly performed in specialized tertiary care centers. Orbital exenteration has long been considered the treatment of choice for managing periocular tumors invading the orbit or primary orbital malignancies. Over the past decades, attention has been directed toward reducing the perioperative morbidity by developing new surgical devices and new strategies and promoting cosmetic rehabilitation by providing adequate facial prostheses. Despite these advances, several studies have questioned the role of orbital exenteration in improving over

OBJECTIVE: Various types of orbital implants are in use in the rehabilitation of anophthalmic patients. The latest is the expensive hydroxyapatite implant. The study objective was to evaluate the effectiveness of low cost glass spherical hollow implants, as primary and secondary implants.
SETTING: St Michaels and Frazer Private Hospitals, Colombo.
STUDY DESIGN AND METHOD: 65 patients had glass sphere orbital implants between 1987 and 1995; 51 primary (evisceration 46, enucleation 5) and 14 secondary (evisceration 2, enucleation 12). At 9 to 12 months follow up, patients were evaluated for mo

Malignant tumours arising in the paranasal sinuses or maxilla usually spread to the surrounding regions. The skull base and the anterior cranial fossa are frequently affected as well. When the resection of a tumour involves an orbital exenteration, a transconjunctival-perilimbic incision can be added to a coronal approach in order to preserve the eyelids and the conjunctiva, avoiding cutaneous midfacial incisions. Patients with a diagnosis of malignant tumour affecting the orbit, upper jaw, paranasal sinuses, and/or anterior skull base were eligible for this technique. Tumoural invasion of the

The literature-reported incidence of ophthalmic injuries occurring with facial fracture ranges widely from 0.8% to 30%. Ocular trauma necessitating enucleation or evisceration is less common, but it is not rare. The trauma and physical disability related to removal of the eye are extreme. Moreover, the loss of an eye causes severe changes to the anatomy and physiology of the orbit, resulting in deformities that affect the relationship between the socket and the prosthesis. Here, the authors present their own experience of 8 consecutive cases of trauma injuries with globe loss and emphasize the

PURPOSE: To histopathologically assess the extent and pattern of vascularization of explanted porous polyethylene (PP) and hyrdoxyapatite (HA) orbital implants. To compare the vascularization in PP implants harvested after enucleation versus after evisceration.
METHODS: This is a comparative case series of six orbital implants explanted between 11 months and 5 years and 4 months post implantation. The implants were subjected to histopathological examination with various stains, after complete decalcification.
RESULTS: There were 2 post evisceration, 3 post enucleation and 1 secondary implant

Hydrogel scleral buckles are a hydrophilic explant that may lead to significant delayed complications. They can insidiously enlarge over decades and may mimic an orbital tumor or cyst. The authors report a case of an expansive hydrogel scleral explant in a previously eviscerated socket. A 58-year-old male presented with a 2-week history of severe pain in his left anophthalmic socket. And 31 years prior, he had sustained OS trauma and undergone a series of surgeries. Details of these procedures were unavailable; however, he had ultimately proceeded to globe evisceration. MRI revealed a large lo

Orbital cysts in anophthalmic socket are infrequent. Most of the cysts reported are anterior and the inability to retain a formerly well-fitted prosthesis is the most common symptom. We present a posterior conjunctival cyst developed after a dermo-fat graft reconstruction in an eviscerated socket. Eleven years after evisceration surgery due to irreparable ocular trauma an extrusion of the orbital implant occurred. Removal of the orbital implant and a socket reconstruction with dermis fat graft was done. Three years later, the patient again returned back to the treating ophthalmologist with com

A patient with Addison disease developed fever, pain, and marked orbital inflammation 3 days after evisceration in the setting of perforated corneal ulcer. He was treated for presumed orbital cellulitis without improvement. Increasing the corticosteroid dose for his Addison disease resulted in complete resolution of the inflammation.

BACKGROUND: Evisceration surgery is an important part of an ophthalmologist's skillset but is an uncommon procedure. Therefore, it is important for training ophthalmologists to have adequate knowledge and confidence in this procedure to maximise learning opportunities. Mixed reality involves the intertwining of real and virtual environments and has been used successfully in surgical training.
OBJECTIVES: To improve participants' knowledge of the relevant anatomy, steps and confidence in performing evisceration surgery using an extended reality online webinar coupled with conventional teaching

The regulatory role of pressure on growth and differentiation of the craniofacial skeleton is largely unknown. We devised an experimental model to determine if the graded application of pressure could exert a trophic influence on craniofacial bone, allowing deliberate alteration and reshaping of the facial skeleton. To examine this question, 18 kittens were used to determine the adverse morphological sequelae after orbital evisceration and to compare the ability to remedy such facial deformity with the use of an inert implant or pressure-induced tissue expansion. In addition to detailed cranio

PURPOSE: To describe an anterior scleral capping technique using human donor sclera or preserved pericardium during ocular evisceration to enable the placement of larger orbital implants.
METHODS: Retrospective interventional case series. Six consecutive patients underwent evisceration for blind, painful eyes without infection or suspected intraocular malignancy. Following standard evisceration, the largest possible spherical implant was inserted. A human donor scleral button (5 cases) or preserved pericardium (1 case) was sutured to the anterior scleral surface as a cap over the implant. Mai

Orbital exenteration is a radical surgery which involves complete removal of the orbital contents along with all or parts of the eyelid. However, the eyelid complex can be totally spared in cases of necrotising disease such as mucormycosis without compromising on exposure, or fear of devascularisation of the lids. We describe a total eyelid complex preserving orbital exenteration. Preservation of the total eyelid complex has the potential to speed up the healing process and provide dynamic functional and cosmetic rehabilitation.

Orbital exenteration remains a significant disfigurement in the face. Many reconstructive options were reported for one stage covering the defects. Local flaps are used primarily in elderly patients who are not candidates for microvascular procedures. Local flaps generally close the gap without achieving 3-dimensional adjustment perioperatively. Secondary procedures or shrinking by time are needed for better orbital adaptation. In this case report, we describe a novel frontal flap design influenced by a Tumi knife, an ancient Peruvian trepanation instrument. The design helps us to create a con
