Dosimetric variations of target volumes and organs at risk in nasopharyngeal carcinoma intensity-modulated radiotherapy.

OBJECTIVE: The aim of this study was to evaluate the actual dose variability to the targets and organs at risk (OARs) during nasopharyngeal carcinoma (NPC) intensity-modulated radiotherapy (IMRT) and to investigate the significance of replanning. METHODS: 11 NPC patients were included in this study. Each patient had both a planning CT and weekly repeated CT. Simulated plans that were generated by using the same beam configurations mapped to the repeated CT represented the actual delivered doses to the target volumes and OARs. An IMRT replanning was performed with the fifth week CT scan. Doses

Risk reduction of radiation-induced aspiration by sparing specific aspiration-related-organs at risk; an in silico feasibility study.

PURPOSE: To assess the feasibility and benefit of NTCP optimized aspiration-prevention treatment planning by sparing specific aspiration related organs at risk, and to assess the impact of baseline complaints on the planning results. MATERIALS AND METHODS: This in silico planning study included 30 HNC patients who were previously treated with definitive radiotherapy. New fully automated plans, allowing for sparing specific aspiration related organs at risk, were optimised directly on normal tissue complication probability (NTCP) models for common toxicities: xerostomia and dysphagia. Optimisa

A prospective multicenter trial of deep learning auto-segmentation for organs at risk in thoracic radiotherapy.

Widespread clinical implementation of rapidly evolving auto-segmentation tools remains constrained by a scarcity of high-quality prospective evidence. Here we show the results of a prospective, multicenter, observational trial (NCT05787522) evaluating the clinical performance of a deep learning model (iCurveE) for artificial intelligence (AI)-assisted delineation of organs at risk (OARs) in thoracic and breast cancer radiotherapy. Computed tomography images from 500 patients across five centers are annotated by 37 physicians using manual, AI-generated, and AI-assisted methods. Eleven thoracic

Variable-interval temporal feathering to optimize organs-at-risk repair for head and neck adaptive radiotherapy.

Background and Purpose: Temporally feathered radiation therapy (TFRT) for head-and-neck cancer (HNC) combines variable-dose intensity modulated radiation therapy (IMRT) subplans to increase organs-at-risk's (OAR) recovery. This study evaluated whether incorporating OAR-specific radiosensitivity into variable rest-time planning can reduce side effects. Materials and Methods: Using radiosensitivity profiles of five OARs (larynx, esophagus, parotid, spinal cord, brainstem), we developed an optimized TFRT (oTFRT) model to maximize OAR recovery by optimizing rest times, guided by the linear-quadrat

Inter-Observer variability in organs at risk contouring among radiation therapy students and qualified radiation therapists.

INTRODUCTION: Contouring organ-at-risk structures (OARs) remains an important skill that must be developed through education and training. This study assessed variations in OAR contouring among undergraduate radiation therapy students and qualified radiation therapists (RTs). METHODS: Four planning computed tomography datasets (Brain, Lung, Parotid, and Prostate), which included 21 clinician-validated OAR contours, were utilised as references in this contouring study. Participants included 2nd to 4th year students, as well as qualified RTs. A quantitative comparison of contours was performed

Deep learning based automatic segmentation of organs-at-risk for 0.35 T MRgRT of lung tumors.

BACKGROUND AND PURPOSE: Magnetic resonance imaging guided radiotherapy (MRgRT) offers treatment plan adaptation to the anatomy of the day. In the current MRgRT workflow, this requires the time consuming and repetitive task of manual delineation of organs-at-risk (OARs), which is also prone to inter- and intra-observer variability. Therefore, deep learning autosegmentation (DLAS) is becoming increasingly attractive. No investigation of its application to OARs in thoracic magnetic resonance images (MRIs) from MRgRT has been done so far. This study aimed to fill this gap. MATERIALS AND METHODS:

Multichannel vaginal cylinder brachytherapy-Impact of tumor thickness and location on dose to organs at risk.

PURPOSE: Multichannel vaginal cylinder brachytherapy (MCVCB) has the potential to sculpt dose distribution, although this is typically reserved for lesions <5-mm thick. The aim of this study was to investigate the dosimetric consequences of treating lesions with MCVCB of varying locations, ≥5 mm in thickness. METHODS AND MATERIALS: Patients previously treated with MCVCB were randomly selected to each fill one of six categories based on location (lateral, anterior, or vaginal cuff and/or apex) and size of cylinder (2.5 or 3.0 cm). Based on magnetic resonance image, each patient's target lesion

Variations in the contouring of organs at risk: test case from a patient with oropharyngeal cancer.

PURPOSE: Anatomy contouring is critical in radiation therapy. Inaccuracy and variation in defining critical volumes will affect everything downstream: treatment planning, dose-volume histogram analysis, and contour-based visual guidance used in image-guided radiation therapy. This study quantified: (1) variation in the contouring of organs at risk (OAR) in a clinical test case and (2) corresponding effects on dosimetric metrics of highly conformal plans. METHODS AND MATERIALS: A common CT data set with predefined targets from a patient with oropharyngeal cancer was provided to a population of

Deep learning-based classification of organs at risk and delineation guideline in pelvic cancer radiation therapy.

Deep learning (DL) models for radiation therapy (RT) image segmentation require accurately annotated training data. Multiple organ delineation guidelines exist; however, information on the used guideline is not provided with the delineation. Extraction of training data with coherent guidelines can therefore be challenging. We present a supervised classification method for pelvis structure delineations where bowel cavity, femoral heads, bladder, and rectum data, with two guidelines, were classified. The impact on DL-based segmentation quality using mixed guideline training data was also demonst

Dosimetric impact of bladder volume on organs at risk during external beam radiotherapy of cervical cancer.

This study aimed to investigate the effect of bladder volume on the dosimetry of pelvic organs at risk (OARs) in patients treated with external beam radiation therapy. Twenty patients with locally advanced cervical cancer were selected. Two computed tomography-simulation scans were obtained, one with an empty bladder followed by one with a full bladder. The acquired images were transferred to the treatment planning system. Target and OARs were contoured in both images, and treatment plans were performed for each computed tomography image. The delivered doses to target and OARs were determined

Bladder distension improves the dosimetry of organs at risk during intracavitary cervical high-dose-rate brachytherapy.

PURPOSE: To evaluate dose-volume histograms (DVHs) and dose-surface histograms (DSHs) to analyze bladder distension during cervical brachytherapy. METHODS: Twenty brachytherapy fractions from five cervical cancer patients were selected. For each fraction, empty and full (200cc of contrasted saline) bladder simulation CT scans existed, one of which was used to plan treatment. An alternative plan was then created with the unused scan. DVH for each fraction was generated for the bladder, rectum, sigmoid colon, and small bowel. Mean DVH dose, D0.1cc, and D2cc were calculated for each organ at ris

Evaluation of intrafraction motion of the organs at risk in image-based brachytherapy of cervical cancer.

PURPOSE/INTRODUCTION: To assess the variation in the doses received by the organs at risk (OARs) that can occur during treatment planning of cervical cancer by image-based brachytherapy. METHODS AND MATERIALS: After intracavitary application, two sets of images-CT and MRI-were obtained. The two sets of images were fused together with respect to the applicator. Contouring was done separately on CT and MR images. Dose received by the OARs on CT images with respect to the plans made on the MR images was estimated and compared with those on the MR images. RESULTS: Although there was always a dif

Dose variations in tumor volumes and organs at risk during IMRT for head-and-neck cancer.

Many head-and-neck cancer (HNC) patients treated with radiotherapy suffer significant anatomical changes due to tumor shrinkage or weight loss. The purpose of this study was to assess dose changes over target volumes and organs at risk during intensity-modulated radiotherapy for HNC patients. Sixteen HNC IMRT patients, all requiring bilateral neck irradiation, were enrolled in the study. A CTplan was performed and the initial dose distribution was calculated. During the treatment, two subsequent CTs at the 15th (CT15) and 25th (CT25) fractions were acquired. The initial plan was calculated on

Performance of multi-vendor auto-segmentation models for thoracic organs at risk trained on a single dataset.

INTRODUCTION: This study evaluates the delineation quality of artificial intelligence (AI)-based models for auto-segmentation trained on the same dataset, as the intrinsic performance cannot be evaluated for commercial solutions due to differences in training datasets. A diverse set of challenging thoracic organs-at-risk (OAR) were chosen, to reveal potential limitations of AI-based tools which are relevant for their clinical adoption. MATERIALS & METHODS: A structure set with 16 OAR was delineated and reviewed by radiation oncology experts for 250 patients with lung tumours (200/50 for train

Evaluation and failure analysis of four commercial deep learning-based autosegmentation software for abdominal organs at risk.

PURPOSE: Deep learning-based segmentation of organs-at-risk (OAR) is emerging to become mainstream in clinical practice because of the superior performance over atlas and model-based autocontouring methods. While several commercial deep learning-based autosegmentation solutions are now available, the implementation of these tools is still at such a primitive stage that acceptance criteria are underdeveloped due to a lack of knowledge about the systems' segmentation tendencies and failure modes. As the starting point of the iterative process of clinical implementation, this study focuses on the

Automated confidence estimation in deep learning auto-segmentation for brain organs at risk on MRI for radiotherapy.

PURPOSE: We have built a novel AI-driven QA method called AutoConfidence (ACo), to estimate segmentation confidence on a per-voxel basis without gold standard segmentations, enabling robust, efficient review of automated segmentation (AS). We have demonstrated this method in brain OAR AS on MRI, using internal and external (third-party) AS models. METHODS: Thirty-two retrospectives, MRI planned, glioma cases were randomly selected from a local clinical cohort for ACo training. A generator was trained adversarialy to produce internal autosegmentations (IAS) with a discriminator to estimate vox
