Preventing osteoporotic fractures in older people.

While fractures at the spine, wrist and hip are regarded as classical osteoporotic fractures, all fragility fractures in the elderly should be considered as osteoporotic once pathological fracture (e.g. metastatic disease) has been excluded. The assessment of fracture risk should take account of specific risk factors in addition to bone mineral density (BMD). The WHO has produced FRAX, a well validated tool that estimates the probability of a major osteoporotic fracture in the next 10 years. The algorithm is specifically designed for primary care. After age and prior fragility fracture, BMD is

Treatment of osteoporotic distal radius fractures.

Fracture of distal radius is most commonly an injury of the fit osteoporotic patient. As the population and health of older individuals continue to expand, osteoporotic distal radius fractures will become increasingly common. While many older patients have limited functional demands and can accept some deformity and wrist dysfunction, others remain very active into older age and desire optimal wrist alignment and function. The difficulty obtaining reliable fixation in osteoporotic bone presents a challenge to the surgeon that has been partially addressed by newer implants with screws that dire

Cost-equivalence of different osteoporotic fractures.

METHODS: Among 985 Olmsted County, Minnesota, residents who experienced an osteoporotic fracture (distal forearm, humerus, clavicle/scapula/sternum, ribs, vertebrae, pelvis, hip, other femur or tibia/fibula [the latter in women only]), we estimated the incremental cost of direct medical care in the following year compared with age- and sex-matched controls without a fracture randomly sampled from the same community. RESULTS: The overall median incremental (case minus control) cost in the succeeding year was $2,390, with a particularly high incremental cost for hip fractures ($11,241). There w

Osteoporotic Fractures of the Pelvic Ring.

In the past, pelvic ring fractures were rare injuries that were classically caused by high-energy trauma. However, due to demographic change, most pelvic ring fractures today arise on the basis of osteoporosis, usually due to low-energy accidents or without an accident at all. The incidence of pelvic ring fractures has increased significantly in recent decades. Today, predominantly elderly patients are affected. Osteoporosis-related pelvic ring fractures represent a separate entity amongst fractures of the pelvis. Their treatment differs significantly from high-energy pelvic fractures, especia

Multidisciplinary management of osteoporotic vertebral fractures.

Vertebral fractures represent the most frequent complication associated with osteoporosis. Patients harboring a vertebral fracture complain physical impairment including low back pain and spine balance alteration, i.e., kyphosis, leading to subsequent systemic complication, with an increase in morbidity and mortality risk. Different strategies are available in the management of osteoporotic vertebral fractures: medical therapy acts as a prevention strategy while surgical vertebral augmentation procedures, when correctly indicated, aim to reduce pain and to restore the physiological vertebral h

Association of gout with osteoporotic fractures.

PURPOSE: Previous studies have shown that serum uric acid levels and inflammation are associated with bone mineral density. Gout, a disease characterized by hyperuricemia and inflammation, contributes to the risk of osteoporotic fractures. However, this association is controversial. Therefore, this study investigated whether gout in older people (age > 55 years) is associated with osteoporotic fracture risk. METHODS: This population-based, cross-sectional study included 2674 participants (147 cases of gout and 388 fractures). Standardized and self-administered questionnaires were employed and

Concomitant and previous osteoporotic vertebral fractures.

Background and purpose - Patients with osteoporosis who present with an acute onset of back pain often have multiple fractures on plain radiographs. Differentiation of an acute osteoporotic vertebral fracture (AOVF) from previous fractures is difficult. The aim of this study was to investigate the incidence of concomitant AOVFs and previous OVFs in patients with symptomatic AOVFs, and to identify risk factors for concomitant AOVFs. Patients and methods - This was a prospective epidemiological study based on the Registry of Pathological Osteoporotic Vertebral Fractures (REPAPORA) with 1,005 pat

Vertebral Augmentation for Osteoporotic Compression Fractures.

Vertebral augmentation procedures such as vertebroplasty and kyphoplasty were developed to reduce pain and improve quality of life for patients with osteoporotic vertebral compression fractures. However, the use of vertebral augmentation has been debated and questioned since its inception. This article addresses some of these issues.

Osteoporotic fractures and vitamin D deficiency.

A caucasian woman aged 79 years, with a history of hypertension, hyperlipidaemia, and osteoarthritis, was referred for assessment of osteoporosis and generalised musculoskeletal pain after surgery for a right midshaft femoral fracture. Further history revealed she had suffered nonspecific musculoskeletal pain, particularly of the pelvic girdle, and unsteady gait for many weeks, but denied suffering any falls. She had limited mobility due to osteoarthritis of both knees. She had been mostly housebound and was on an animal product restricted diet in view of her history of hypercholesterolaemia.

Osteoporotic vertebral fractures in postmenopausal women.

Back pain is a common symptom in postmenopausal women. As in younger age groups, most cases of back pain in postmenopausal women do not represent serious disease and resolve spontaneously within four weeks. However, acute back pain in postmenopausal women may be caused by vertebral fracture, and "red flags" in the history and physical examination can help clinicians decide on the appropriate work-up. When findings suggest vertebral fracture, anteroposterior and lateral radiographs of the thoracolumbar spine should be obtained. The diagnosis of existing vertebral fractures is critical because t

The pathophysiology of osteoporotic vertebral fractures.

Osteoporosis is an increasingly prevalent chronic metabolic bone disease, characterized by low bone mass that subsequently leads to bone fractures. Vertebral fractures are the most common and the most debilitating of osteoporotic fractures. People with osteoporotic vertebral fractures present a unique challenge to rehabilitation nurses. A better understanding of the complex pathophysiological processes that contribute to this disability will help nurses provide more comprehensive care and education to this growing segment of the population. This article presents an overview of normal physiolog

Osteoporotic fractures: background and prevention strategies.

OBJECTIVES: To review current knowledge of the epidemiology, pathogenesis, prevention and treatment of osteoporosis, with particular reference to issues related to the menopause. METHODS: Peer-reviewed publications were assessed. RESULTS: Much international variation exists in the prevalence of osteoporosis and the incidence of fracture. Risk fractures for osteoporosis are numerous. The menopause and other causes of hypogonadism in both women and men strongly predispose to osteoporosis. Various endocrinopathies, especially glucocorticoid excess, also are important. The contribution of family

Osteoporotic sacral fractures: a clinical study.

Traumatic sacral fractures are most often due to motor vehicle or industrial accidents and are commonly associated with pelvic, urogenital, and neurological injuries. In recent years, a more subtle type of sacral fracture, not associated with major trauma, has been described. It is an osteoporotic insufficiency fracture that presents as low back pain in elderly patients, especially postmenopausal Caucasian women. It may escape detection unless radionuclide bone scans, tomograms, or computed tomograms are obtained. The radiographic features have been detailed in various publications, but little

Prognostic value of the "modified Osteoporotic Fracture score" for treatment decision in osteoporotic vertebral fractures.

BACKGROUND: The mOF score shows promise as a reliable tool to guide treatment decisions of osteoporotic vertebral fractures (OVFs). However, the prognostic value is lacking. This study aims to assess the prognostic value of the mOF score considering clinical and radiological outcomes in accordance with compliant or non-compliant management. METHODS: A retrospective cohort study included all patients aged ≥50 years and diagnosed with acute thoracolumbar OVF between 2017 and 2021. Final treatment was compared with the recommendations according to the mOF score. Clinical and radiological outcome

["Paradoxical fractures": pathological fractures under anti-osteoporotic and antirheumatic treatment].

Pathological fractures under anti-osteoporotic and antirheumatic treatment are very rare events. Nevertheless, atypical femoral fractures occur during antiresorptive treatment with bisphosphonates or denosumab, the latter especially in patients previously treated with bisphosphonates. Treatment with teriparatide can be helpful. While glucocorticoids have a well-known influence on the development of osteoporosis and thus also fractures, the probably unproblematic use in the low-dose range has so far found little acceptance. Methotrexate-induced osteopathy is also a rare phenomenon but is now we

"Occult" osteoporotic vertebral fractures: vertebral body fractures without radiologic collapse.

STUDY DESIGN: Retrospective observational study. OBJECTIVES: We report our experience with patients who presented with osteoporotic vertebral fractures with no visible deformation of vertebral body. SUMMARY OF BACKGROUND DATA: The diagnosis of osteoporotic vertebral fractures largely relies on the observation of vertebral deformations on plain radiographs, termed vertebral collapses. There are no data on the characteristics, or indeed of the reality, of osteoporotic vertebral fractures with no significant deformation of the vertebral body. METHODS: We retrospectively analyzed cases that pre

Olecranon Fractures Have Features of Osteoporotic Fracture.

BACKGROUND: To determine whether olecranon fractures have osteoporotic features such as age-dependent, low bone attenuation and low-energy trauma as a cause of injury. METHODS: Elbow computed tomography (CT) and medical record review were performed in 114 patients (53 males and 61 females) with acute olecranon fractures. The mean age was 57 years. Bone attenuation was measured on the central part of the olecranon on sagittal CT images avoiding the fracture, and on the distal humerus (distal metaphysis and medial and lateral condyles) on coronal CT images. We compared bone attenuation and caus
