[Triple juxtacotyloid osteotomy].

The authors have developed a technique of triple innominate osteotomy derived from that describe by Steele. The three osteotomy divisions are made very close to the acetabulum. With this technique the acetabulum can more easily be re-orientated and the pubis and ischium are left intact with their muscle attachments.

Modified Salter osteotomy.

A modification of the Salter innominate osteotomy for hip dysplasia was developed. A posterior triangular area is removed from the proximal side of the osteotomy site to allow engagement of the distal iliac segment after displacement of the osteotomized bone. This leads to increased stability and prevents the medial and posterior displacement that occasionally is seen after a Salter osteotomy. Also, with this modification the added limb length seen with the Salter osteotomy is eliminated. Early experience shows that this modification has produced excellent results in patients with unilateral d

Double innominate osteotomy.

We performed double innominate osteotomy in twenty-five patients with acetabular insufficiency resulting from congenital dislocation of the hip and other lesions. Following iliac (Salter) osteotomy, the second osteotomy was carried out medial to the obturator foramen in the interval between the symphysis pubis and the pubic tubercle. In children more than six years old, adolescents, and adults, addition of the pubic osteotomy increased the amount of acetabular rotation and coverage of the femoral head that could be achieved. An additional benefit was that the femoral head could be shifted medi

Metatarsus Adductus Osteotomy.

Metatarsus adductus (MA) is an isolated, transverse-plane dominant deformity occurring primarily at the tarsometatarsal joints. The deformity is commonly associated with increased rates of fifth metatarsal base fractures, hallux valgus deformities, lesser metatarsophalangeal joint deformities, and multiplanar deformities of the hindfoot. Periarticular osteotomies of the metatarsals, cuboid, and cuneiforms have been discussed thoroughly in the literature, both isolated and in combination. Recent techniques have described intraarticular osteotomies with realignment arthrodesis of the tarsometata

Lateral metatarsal osteotomy.

Lateral metatarsal osteotomy (LMO) aims to reposition the affected metatarsals in a chosen position. The approach may be open or percutaneous. There are several types of LMO, according to displacement: shortening, raising, translation or lengthening. Preoperative planning covers type and extent of displacement, osteotomy location and type (open or percutaneous), and number of metatarsals concerned. In the 1990s, older concepts of non-fixed osteotomies gave way to preoperative planning and calculated shortening, including the development of Weil shortening osteotomy. Diaphyseal lengthening or s

Varus Derotational Osteotomy.

Legg-Calve-Perthes disease is a multifactorial process that can lead to debilitating femoral head deformity in children. Management can range from conservative (such as nonweightbearing protocols), bracing (such as A frames), to more invasive measures involving surgical interventions. First described by Axer in 1965, the varus derotational osteotomy (VDRO) has been a staple in the surgical management of Perthes disease. The goal of the VDRO is "containment" or prevention of extrusion of the head from the acetabulum. Many variations of the technique have been described over the years since its

Osteotomy for clinodactyly.

Clinodactyly, finger deviation in the coronal plane, affects the little finger most commonly. Patients typically tolerate clinodactyly without functional limitation, but with increasing deformity, they may have difficulties with specific tasks such as playing musical instruments. In those who do not tolerate the deformity, surgical options include physiolysis for young patients and osteotomy for older patients. An opening wedge osteotomy increases finger length while correcting the coronal deformity. Surgical results of the opening wedge osteotomy are satisfying and complications are rare, wit

[Tibial valgus osteotomy].

The tibial valgus osteotomy whatever its technique has a survival rate of about 85 % to 10 years, if we consider the reoperation as a criterion of failure, with a confidence index at 78%. The age, weight, sex and functional signs have no impact on the outcome. We have found no evidence in the preoperative radiographic assessment, neither the medial pinch, or varus epiphyseal neither varisant gap, which could be a failure and a reoperation before the tenth year. Good results were observed significantly when there is an over-valgus at least 3 degrees of global axis of the lower limb. This corres

High tibial osteotomy.

High tibial osteotomy is effective for managing a variety of knee conditions, including gonarthrosis with varus or valgus malalignment, osteochondritis dissecans, osteonecrosis, posterolateral instability, and chondral resurfacing. The fundamental goals of the procedure are to unload diseased articular surfaces and to correct angular deformity at the tibiofemoral articulation. Although the clinical success of total knee arthroplasty has resulted in fewer high tibial osteotomies being done during the past decade, the procedure remains useful in appropriately selected patients with unicompartmen

Biplanar chevron osteotomy.

Results of biplanar chevron osteotomy performed on patients with mild-to-moderate hallux valgus deformity with an increased distal metatarsal articular angle (DMAA) are shown. The study included clinical data of 32 patients (54 feet) who had completed a 2-year follow-up, and radiological data of these 32 and other 29 patients (50 feet) for a total of 61 patients (104 feet, 53 right and 51 left). There were 59 females and two males with ages varying from 11 to 66 years. According to the AOFAS Hallux Rating, the preoperative average score (50) improved to 90 (average score after the surgery). Proximal tibial osteotomy.

The indications for proximal tibial osteotomy are disabling osteoarthritis that must be mostly unicompartmental, minimal bone loss, and good stability. In the 60s we saw the development of osteotomy and in the 70s we saw the development of total knee replacement. These days we're seeing the two operations being put in proper perspective. At the moment we do about 100 upper tibial osteotomies a year and close to 800 knee replacements, either unicompartmental or bicompartmental. We have had 30 years of experience with proximal tibial osteotomy, and we know it relieves pain while preserving bone

High tibial osteotomy.

The results of high tibial osteotomy performed in a 12-year period in 75 patients (88 knees) were followed for at least five years or until failure occurred. Of the 86 knees available for subjective follow-up evaluation, 64 were in men and 22 were in women. Roentgenographic data were available for 75 knees. The results were rated good (no or minimal pain, occasional analgesics required, slight limitation of activity), fair (regular analgesics required, noticeable decrease in activity), poor (moderate to severe pain, marked decrease in activity), or failed (arthroplasty required). Osteotomies Demystified.

Osteotomies are critical techniques in rhinoplasty. There are a variety of approaches, trajectories, and tools used with no widely standardized classification or nomenclature. Percutaneous osteotomies are gaining in popularity, and picture framing the nasomaxillary bone is crucial for predictable fracture and reproducible results. This is best accomplished with medial, lateral, and transverse osteotomies. Intermediate osteotomies are used less frequently, but provide more mobility. With a detailed understanding of anatomy and a thorough approach to nasal osteotomies, the contour and function o

Midfoot osteotomies.

Midfoot osteotomies are primarily used for reconstructive procedures for congenital or acquired deformities. Although used more in a younger population, they can used in most any patient population. With restoration of a more normal architecture, patients' symptoms resolve and prevention of long-term sequella can be avoided. Careful evaluation is necessary, but with proper use, midfoot osteotomies are useful procedures in the foot and ankle surgeons' repertoire.

Distal osteotomy of the lateral metatarsals: a series of 72 cases comparing the Weil osteotomy and the DMMO percutaneous osteotomy.

INTRODUCTION: A Weil osteotomy with internal fixation can match the preoperative plan by precisely setting the metatarsal length; however 10 to 30% of patients end up experiencing postoperative stiffness. A percutaneous distal metatarsal mini-invasive osteotomy (DMMO) is a purely extra-articular technique; metatarsal length is set automatically upon weight bearing of the foot. The goal of this study was to compare these two osteotomy techniques when performed on the three or four most lateral metatarsals. HYPOTHESIS: A DMMO will result in better joint motion than a Weil osteotomy. PATIENTS A
