Le Fort III osteotomy: a new internal positioned distractor.

PURPOSE: The treatment of midfacial retrusion, due to congenital midfacial hypoplasia or trauma, is a challenging problem. Conventional Le Fort III osteotomies, with the surgical mobilisation of the entire midface, offers one solution. Distraction osteogenesis has become an accepted, and often preferred, technique for the treatment of severe midfacial hypoplasia. Existing distraction devices have several disadvantages. To overcome some of these disadvantages, an internal and temporal positioned distraction device was developed. PATIENTS AND METHODS: The new, internal positioned distractor dev

Carotid-cavernous sinus fistula following Le Fort I osteotomy.

A case report of a carotid-cavernous sinus fistula, a rare complication following orthognathic surgery, is presented. The anatomy and pathophysiology of the condition is discussed as they relate to the development of clinical signs and symptoms, and an attempt is made to explain this complication on the basis of a spectrum of possible internal carotid injuries following orthognathic surgery.

Advancement of the midface, from conventional Le Fort III osteotomy to Le Fort III distraction: review of the literature.

Since its introduction in about 1950, the Le Fort III (LF III) procedure has become a widely accepted treatment for correction of midface hypoplasia and related functional and esthetic problems. As long-term surgical experience grows and improvements are made in technique, equipment and peri-operative care, the number of LF III procedures performed worldwide is increasing. A number of fundamental questions concerning the technique remain unclear, and large, conclusive studies are lacking owing to the relative rarity of severe midface hypoplasia. This literature review aims to address problems,

Peri- and postoperative complications in Le Fort I osteotomies.

This retrospective study was performed to report the peri- and postoperative complications encountered by patients who underwent Le Fort I osteotomy, as well as predictor variables affecting the risk of complications. Patients who underwent only Le Fort I osteotomy were included in the study. Information on peri- and postoperative complications were collected from the patient data records. The effects of certain predictor variables on complication rates were also studied. Twenty-four per cent of the patients suffered from complications, six (6.1%) of whom were reoperated. Most of the complicat

The Use of Brainlab Navigation in Le Fort III Osteotomy.

Le Fort III osteotomy is commonly used in the surgical correction of midface hypoplasia, specifically in patients with syndromic craniosynostosis. These osteotomies can be associated with significant complications, which are often the result of incomplete or inaccurate osteotomies. Brainlab, a technology first developed for neurosurgery, has been applied to numerous surgical subspecialties. The aim of this study was to report our initial experience using the Brainlab VectorVision2 and Brainlab Curve (Brainlab, Westchester, IL) as an intraoperative guidance system for osteotomy placement during

Rhinoplasty and Le Fort I Maxillary Osteotomy in Cleft Patients.

INTRODUCTION: Cleft patients often need orthognathic surgery to correct maxillary hypoplasia and rhinoplasty to correct nasal deformity. Rhinoplasty can be performed as a staged procedure after orthognathic surgery or simultaneously with maxillary osteotomy. AIM: The authors evaluated need for and complications of staged and simultaneous rhinoplasties in patients with different cleft types undergoing maxillary osteotomy. PATIENTS AND METHODS: This retrospective study examined 99 (54 females) consecutive nonsyndromic patients with cleft lip/palate [23 bilateral cleft lip and palate (BCLP), 51

Le Fort III osteotomy or distraction osteogenesis imperfecta: your choice.

BACKGROUND: Traditional Le Fort III osteotomies have been described by several authors; however, few have reported the distances advanced and stability over time. METHODS: The authors assessed their last 30 Le Fort III osteotomies with respect to the immediate incisor advancement achieved at surgery and long-term advancement as measured at the A-point from preoperative and postoperative cephalometric tracings. The authors then compared their results with those in the literature for Le Fort III distraction techniques. Comparisons were made with respect to distraction distance maintained, the n

Subcranial procedures in craniofacial surgery: the Le Fort III osteotomy.

The esthetic stigmata and functional impairments associated with craniofacial deformities have long challenged surgeons in the management of deficiencies of the middle third of the face. By their very nature, craniofacial anomalies are repetitive patterns of deformity that affect the different functional and esthetic subunits of the facial hard and soft tissues. Although numerous procedures exist for the management of the various craniofacial malformations in the middle third of the facial skeleton, only the subcranial Le Fort III osteotomy addresses all of the functional and esthetic componen

Ocular palsy following Le Fort 1 osteotomy: a case report.

A 33-year-old female patient developed an ipsilateral sixth nerve palsy and partial third nerve palsy following a Le Fort 1 osteotomy. Complete resolution occurred at 10 weeks. The likely mechanism of injury secondary to pterygo-maxillary dysjunction is highlighted, with description of the relevant anatomy. Previous cases of ocular motility complications following Le Fort 1 osteotomy are discussed. We recommend that significant care be taken in osteotome placement in the pterygo-maxillary fissure, particularly in those prone to unpredicted fractures such as older patients, or where the anatomy

Simultaneous Le Fort I osteotomy and computer-guided implant placement.

BACKGROUND: Since 1977, osseointegrated implants have played a strong role in oral rehabilitation. Despite the large success of this technique for the edentulous maxilla and mandible with sufficient bone height, patients with atrophic jaws continue to be the challenging cases for an adequate reconstruction of aesthetics and functionality. Orthognathic surgical procedures have been developed to reposition the jaws and have been traditionally used in the dentate patient to correct a skeletal malocclusion; these procedures can be used on the edentulous patient to correct the discrepancies between
